Grup A Streptokok ve İnfluenza A/B’nin sebep olduğu üst solunum yolu enfeksiyonlarının karşılaştırılması

dc.description.abstractAmaç: İnfluenza koenfeksiyonu şeklinde olan bakteriyel enfeksiyonlar özellikle yaşlılar ve küçük çocuklar gibi yüksek riskli gruplar arasında ciddi bir morbidite ve mortalite nedenidir. Çalışmamızda olgularımızda saptanan grup A streptokok ile influenza virüs tip A ve tip B nedenli üst solunum yolu enfeksiyonlarının klinik ve laboratuvar bulgularının karşılaştırılması ile her iki etkenin koenfeksiyon sıklığının irdelenmesi amaçlanmıştır. Yöntem: Çalışmaya üçüncü basamak hastanemiz pediatri kliniğine üst solunum yolu enfeksiyonları şikayetiyle 2019 Ocak - 2020 Ocak tarihleri arasında başvuran ve streptokok hızlı antijen testi ile influenza A ve B hızlı testleri uygulanan toplam 1884 çocuk hasta dahil edildi. Eş zamanlı olarak çocukların tam kan sayımı ve C-reaktif protein testleri de çalışılmış olup sonuçlar influenza virüsü ve grup A steptokok enfeksiyonları için karşılaştırıldı. Sonuçların istatistiksel analizi yapıldı ve p<0.005 değerleri anlamlı kabul edildi. Bulgular: Hastaların %54.6’sı erkek, %45.4’ü kız olup yaşları ortalaması yaşı 5.0±3.1 yıldır. Hastaların 1174’ünde (%62.3) grup A streptokok veya influenza virüsleri saptanmadı. Toplam 226 hastada (%12.0) grup A streptokok, 319 hastada (%16.9) influenza virüs tip A, 118 hastada (%6.3) influenza virüs tip B, 47 hastada (%2.5) ise grup A streptokok ve influenza virüs tip A birlikte saptandı. Gruplar arasında cinsiyet açısından istatistiksel farklılık yoktu (p=0.6). Ortalama lökosit ve nötrofil düzeyleri streptokok enfeksiyonu olan hasta grubunda diğer tüm gruplara göre anlamlı yüksek bulundu (p<0.001). Ortalama nötrofil düzeyi negatif grupta grup A streptokok dışındaki gruplara göre; grup A streptokok grubunda ise influenza A ile grup A streptokok+influenza A gruplarına göre anlamlı yüksekti (p<0.001). Ortalama monosit, trombosit ve C-reaktif protein düzeyleri hem negatif grupta hem de grup A streptokok grubunda diğer gruplara göre anlamlı yüksek bulundu (p<0.001). Gruplar arasında ateş, boğaz ağrısı, yutkunma güçlüğü, burun akıntısı ve tıkanıklığı, hırıltılı solunum, bulantı, kusma, karın ağrısı, iştahsızlık ve halsizlik düzeyleri açılarından anlamlı fark bulunamadı (p>0.005). Ancak influenza A grubunda öksürük düzeyi (%61.8) diğer gruplardan anlamlı yüksek bulundu (p<0.001). Sonuç: Sonuç olarak üst solunum yolu enfeksiyonu geçiren çocuk hastalarda etkenin viral mi bakteriyel mi olduğu hızlı ve doğru olarak saptanmalıdır. Çalışmamızın, hem grup A streptokokların hem de influenza virüslerin hızlı antijen testleriyle çalışılmış olması ve klinik ve laboratuvar bulguların karşılaştırılması yönüyle klinisyenler ve araştırmacılar için yol gösterici olabilecek veriler sağladığı düşüncesine ulaşılmıştır.
dc.description.abstractObjective: Coinfections of influenza virus with bacterial infections are an important cause of morbidity and mortality, especially among high-risk groups like young children and the elderly. This study aimed at the comparison between clinical and laboratory findings of upper respiratory tract infections caused by group A streptococcus and influenza virus type A and B to determine the frequency of coinfection. Methods: A total of 1884 patients who applied to the pediatrics clinic of our tertiary referral hospital with complaints of upper respiratory tract infections between January 2019 and January 2020 which were performed the antigen group a streptococcus and influenza A and B rapid tests were included to our study. Complete blood counts and C-reactive protein tests were compared in between the patients with influenza virus and group A streptococcus. Values of <0.005 p were considered statistically significant. Results: Total of the patients of 54.6% were male and 45.4% were female. Mean age was 5 ± 3.1 years (range, 2-26 years). 1174 (62.3) of patients were tested negative for the group A streptococcus and the influenza virus. Group A streptococcus was found in 226 patients (12.0%), the influenza virus type A was found in 319 patients (16.9%), the influenza virus type B was found in 118 patients (6.3%) and the coinfection of group A streptococcus and influenza virus type A were found positively in 47 patients (2.5%). There was no significant statistical difference between the groups regarding gender. Mean leukocyte and neutrophil levels were significantly higher in patients with the streptococcal infection than in all the other groups (p<0.001). The mean neutrophil levels in the negative group were higher than other groups, except the group A streptococcus group. Also the neutrophil levels of group A Streptococcus were significantly higher than the influenza A and the influenza A + group A streptococcus (p<0.001). Mean monocyte, platelet, and C-reactive protein levels in the negative group and the group A streptococcus group were determined significantly higher. (p<0.001). There was no significant difference between the groups in terms of fever, sore throat, difficulty in swallowing, runny nose, nasal congestion, wheezing, nausea, vomiting, abdominal pain and loss of appetite and weakness (p>0.005). The cough levels in the influenza A group (61.8%) were significantly higher compared to the other groups (p<0.001). Conclusion: Whether the agent of the cause of the upper respiratory tract infections is viral or bacterial should be determined rapidly and accurately in children. It was concluded that our study provided data that could be a guide for clinicians and researchers by rapid antigen tests of both group A streptococci and influenza viruses and by comparing clinical and laboratory findings.
