Antimicrobial susceptibility results and characterization of skin and soft tissue infections caused by staphylococcus aureus in children
(2024) Yakut, Nurhayat; Ergenç, Zeynep; Bayraktar, Sezin; Akbolat, İrem; Sayın, Elvan; İlki, Arzu; Kepenekli, Eda
Introduction: Staphylococcus aureus is a major cause of skin and soft tissue infections (SSTIs). This study aimed to determine the antimicrobial susceptibility and clinical and epidemiological characteristics of community-acquired SSTIs caused by methicillin-susceptible and methicillin-resistant S. aureus (MSSA-MRSA) in children. Materials and Methods: This was a retrospective, single-center study of pediatric SSTIs caused by S. aureus at a tertiary care hospital in Türkiye between January 2014 and November 2019. Results: Demographic, clinical, and microbiological data of 431 patients were examined during the study period. Overall, 333 (77.3%) isolates were MSSA, and 98 (22.7%) were MRSA. Antibiotic courses and hospital stays were significantly longer in patients with MRSA infection. The antimicrobial susceptibility patterns for 17 antibiotics were assessed in both MSSA and MRSA isolates. Penicillin resistance rate was 91%, while fosfomycin, gentamicin, mupirocin, trimethoprim-sulfamethoxazole, and fusidic acid resistance rates were 1.2%, 3.2%, 2.7%, 4.2%, and 8.1%, respectively. All S. aureus isolates were susceptible to teicoplanin, vancomycin, linezolid, and tigecycline, and 335 (77%) isolates showed susceptibility to daptomycin. A statistically significant increase was detected in resistance of MSSA isolates to trimethoprim-sulfamethoxazole in 2019 compared to 2014, 2015, 2016 and 2018 (p1= 0.029 (2014 vs. 2019); p2= 0.008 (2015 vs. 2019); p3= 0.019 (2016 vs. 2019); p4= 0.032 (2018 vs. 2019). Conclusion: S. aureus strains causing SSTIs showed a continued high prevalence of MSSA and multi-drug susceptibility. A striking result was the detection of increased resistance to trimethoprim-sulfamethoxazole, which was frequently used in oral therapy against MSSA strains in 2019 compared to the other years. These results may provide guidance for clinical management of SSTIs in children.

First case of intravenous drug use-related candidemia in Türkiye
(2024) Kaya, Abdurrahman; Zerdali, Hasan; Koç, Buse; Tozalgan, Ümit; Çiftçi, Can; Mert, Ali
Although Candida species are normal flora elements of the body, they can cause various diseases from mucocutaneous infections to candidemia. The presence of a central venous catheter, use of broad-spectrum antibiotics, parenteral nutrition, dialysis, neutropenia, malignancy, and immunosuppressive treatments are the most reported risk factors for candidemia. Complications that develop during follow-up increase both morbidity and mortality. The incidence of fungal infections has been increasing in recent years. This increase is also seen in intravenous drug users. In these patients, candidemia can cause serious life-threatening complications including septicemia and end-organ failure. To date, no cases of intravenous drug use-related candidemia have been reported in our country. This patient represents the first documented case. In this case, we aim to draw attention to the relationship between intravenous drug use and candidemia.

Effects of pinealectomy and melatonin application on Serum Melatonin, Nesfatin-1 and ghrelin levels
(Pera Publishing Services, 2023) Sönmez, Saadet; Menevşe, Esma; Baltacı, Saltuk Buğra; Ünal, Ömer; Moğulkoç, Rasim; Baltacı, Abdulkerim Kasım
Objective: In this study, it was aimed to investigate the relationship between the pineal gland and ghrelin and nesfatin-1 hormones in rats. Methods: A total of 36 male rats were used in the study, and the animals were divided into 4 groups. Group 1, Control; Group 2, Pinealectomy (Px); Group 3, Px+Melatonin; Group 4 Melatonin. After the end of the experimental applications, melatonin, ghrelin and nesfatin-1 levels (ELISA) were determined in the blood samples taken from the animals. Results: While pinealectomy resulted in suppression of melatonin levels, melatonin supplementation led to a significant increase in blood melatonin levels (p<0.01). Melatonin supplementation suppressed ghrelin levels, while pinealectomy increased ghrelin levels (P<0.01). On the other hand, Nesfatin-1 levels, which increased with melatonin support, were significantly suppressed by pinealectomy (p<0.01). Conclusion: The findings of the study draw attention to an important relationship between the endocrine activity of the pineal gland and the hormones ghrelin and nesfatin-1, which play a critical role in nutrition. Consequently, administration of melatonin inhibits ghrelin but increases nesfatin-1.

(Ankara University, 2024) Sağcan, Nihan; Sağcan, Hasan; Bozkurt, Fatih; Bulut Güneş, Ayşe Nur; Fakir, Hüseyin; Dertli, Enes; Sağdıç, Osman
Inulin and fructooligosaccharides (FOS) are prominent functional components in the food industry due to prebiotic and other pharmaceutical properties. Inulin is a storage polysaccharide in various plants. FOS are naturally present in various plants and can be obtained by partial hydrolysis of inulin. In this study, ground and sieved chicory roots (Cichorium intybus L.) were used as starting material for inulin extraction under optimized conditions determined by Response Surface Methodology (RSM) with a Box-Behnken design. Optimum inulin extraction conditions from chicory roots were; temperature of 90 °C, extraction time of 30 min., and liquid-to-solid (LS) ratio of 10:1 mL/g. Inulin extract was further hydrolyzed to FOS by enzymatic or acid treatment, separately. Purification of inulin extract and FOS hydrolysate was performed by ultrafiltration with a 10 kDa membrane under the pressure of 2 bar with continuous stirring. As a result, inulin and FOS were obtained at 90% and 76% purity, respectively.

Comparison of the efficiencies of intrathecal and intraganglionic injections in mouse dorsal root ganglion
(Turkiye Klinikleri, 2023) Aydın, Mehmet Şerif; Yiğit, Esra Nur
Background/aim: Dorsal root ganglia (DRG) are structures containing primary sensory neurons. Intraganglionic (IG) and intrathecal (IT) applications are the most common methods used for viral vector transfer to DRG. We aim to compare the efficiencies and pathological effects of IT and IG viral vector delivery methods to DRG, through in vivo imaging. Materials and methods: Mice were divided into four groups of six each: IT, IG, IT-vehicle, and IG-vehicle. Adeno-associated virus (AAV) injection was performed for EGFP expression in IT/IG groups. DRGs were made visible through vertebral window surgery and visualized with multiphoton microscopy. After imaging, spinal cords and DRGs were removed and cleared, then imaged with light sheet microscopy. Results: No neuronal death was observed after IT injection, while the death rate was 17% 24 h after IG injection. EGFP expression efficiencies were 90%–95% of neurons in both groups. EGFP expression was only observed in targeted L2 DRG after IG injection, while it was observed in DRGs located between L1-L5 levels after IT injection. Conclusion: IT injection is a more suitable method for labeling DRG neurons in neurodegenerative injury models. However, when the innervation of DRG needs to be specifically studied, IT injection reduces this specificity due to its spread. In these studies, IG injection is the most suitable method for labeling single DRG neurons.

The relationship between kappa angle and photic phenomena after trifocal intraocular lens implantation
(Duzce University, 2023) Koç, Hacı; Kaya, Faruk
Aim: This study aimed to investigate the relationship between photic phenomena and the kappa angle after trifocal lens implantations. Material and Methods: Fifty eyes of 35 cases, 17 female and 18 male, were included in the study. The kappa angle was calculated with the Lenstar LS900 low-coherence interferometry device using the pupil barycenter parameter. It was also calculated by using the iris barycenter parameters. According to the calculations using the pupil barycenter distance, the patients were divided into two groups with the preoperative pupil barycenter distance below 0.4 mm and above 0.4 mm. A questionnaire was applied to the patients to evaluate complaints and satisfaction in the postoperative period. Results: The mean preoperative pupil barycenter distance was 0.38±0.12 mm and 52.0% (n=26) of the measurements were below 0.40 mm, while the mean preoperative iris barycenter distance was 0.40±0.15 mm and 46.0% (n=23) of the measurements were below 0.40 mm. No significant correlation was found between the preoperative pupil barycenter distance and the preoperative iris barycenter distance (rs=0.086, p=0.553). Additionally, there was no statistically significant difference between the two groups concerning symptoms such as halo and glare (p=0.948). Conclusion: When considering a kappa angle upper limit of 0.6 mm, there is no discernible difference in the frequency of occurrence of photic phenomena. We believe that both iris barycenter parameters and pupil barycenter parameters, utilized for kappa angle calculations, can be effectively employed to determine the deviation distance.

(Turkiye Klinikleri, 2023) Açıkgöz, Özgür; Bilici, Ahmet; Tataroğlu Özyükseler, Deniz; Göktaş Aydin, Sabin; Selçukbiricik, Fatih; Rzazade, Rashad; Ölmez, Ömer Fatih; Başak Çağlar, Hale
Background/aim: Oligometastatic disease for nonsmall cell lung cancer (NSCLC) patients is generally thought to represent a better prognosis with a quieter biology, limited number of disease sites and long-term disease control. In this study, we aimed to determine the efficacy of radical treatment options for patients with oligometastatic NSCLC. Materials and methods: This retrospective trial included totally 134 patients with oligometastatic NSCLC. The presence of oncodriver mutation, tumor stages and nodal status, the number of metastases and involved metastatic site, treatment of primary tumor and oligometastasis, response rate, overall survival (OS) and progression-free survival (PFS) were evaluated. Results: Of 134 patients 66.4% were defined as adenocarcinoma, 26.1% were squamous cell carcinoma and 7.5% of patients were in other histology. Based on the treatment of primary tumor, in 36 patients (26.9%) curative surgery has undergone, in addition, 19 (14.2%) patients were received chemotherapy, 73 (54.5%) were treated with chemoradiotherapy, while immunotherapy and targeted therapy were used in 1 (0.7%) and 2 (1.4%), respectively. The preferred treatment for oligometastatic lesions were SBRT in 72.4% of patients, surgery in 10.5%, and both SBRT and surgery in 17.1% of patients. At the median follow up of 31.3 months (range: 9.5–48.5), the median PFS and OS times were 17 and 24.4 months, respectively. Moreover, OS-2 after progression was also 7.2 months. Conclusion: Based on our real-life experience, we demonstrated a significant correlation between good response to first treatment and survival in oligometastatic disease, we also understand that local ablative treatment modalities prolong and also delay both OS and PFS in oligometastatic NSCLC patients OS-2.

A Prospective Study: Can physical examination identify palpable undescended testes more accurately than ultrasonography?
(Logos Medical Publishing, 2022) Akman, Mustafa
Introduction: The cremasteric reflex, which makes the testicle mobile, and excessive scrotal adipose tissue may mimic scrotal emptiness, leading to the evaluation of cases as undescended testes. Although it has been suggested that the location of the testes cannot be determined by imaging tests, suspicious localization may lead the physician to imaging tests. This study reveals the needlessness of ultrasonography (USG), especially in palpable cases. Methods: In our study, patients who had their initial evaluation and USG performed by a pediatrician were targeted. Finally, a pediatric surgical evaluation was performed by a single pediatric surgeon, and the findings were compared. The cases were divided into two groups, normal and abnormal testicles, based on the pediatrician's distinction. According to this distinction, all cases were evaluated as a whole. Testes that could not maintain their position within the scrotum were considered abnormal. Consistency was sought in intergroup relations with Cohen's Kappa. Results: The study was conducted with 75 patients who met the criteria. In the group with normal localization in the pediatrician examination, pediatric surgical evaluation was compatible in one (2.6%) of the cases reported as abnormal on USG and 38 (100%) of the cases reported as normal on USG. In the group with abnormal localization in the pediatricians’ examination, pediatric surgical evaluation was compatible with 32 (40%) of the cases reported as abnormal on USG and four (100%) of the cases reported as normal on USG. Discussion: Radiologic examination of testicles, the localization of which cannot be ascertained by USG, began in the 1970s and is still being practiced. Some institutional bodies and independent researchers express opposing views, and some authors suggest that USG should be used. Conclusion: The popularity of USG does not seem to be ending. In our study, the findings showed that the number of patients referred to surgery by USG was significantly higher. The findings obtained in this study suggest that imaging should be confined to non-palpable cases and USG only, and diagnostic laparotomy should be accepted as the gold standard. Relying on normal pediatric exams and being skeptical of abnormal reports revealed by USG will mean staying on the safe side.

Parenting perceptions and experiences of healthcare professionals employed in contact tracing teams
(ANI Publishing, 2023) Gürsoy, Figen; Eraslan, Ayşe Nihal; Argüt, Neslihan; Öz, Sena; Çakcak, Damla; Yavuzekinci, Mustafa; Sezer, Fikriye; İkiz, Selin
The pandemic has mandated some healthcare workers to provide contact tracing services. The present study aimed to discuss the parenting perceptions and experiences of healthcare professionals assigned to contact tracing teams. We carried out this phenomenological study with 29 mothers and 26 fathers selected using purposive sampling techniques. We collected the data using a demographic information form and a parent interview form and analyzed them using the content analysis method. The results revealed that the parents attempted to balance the risks of their tasks with their parenting roles. Also, their perception of protecting their families was the basis of their efforts to protect themselves. Moreover, the social distance to family members revealed longing and intimacy expectations among the participants. The pandemic has significantly impacted families, which is exponentially dramatic for contact tracing teams as health workers. Therefore, it seems significant to carry out counseling work with such families.

(Kare Publishing, 2022) Karcı, Ebru; Ürün, Yüksel; Köksoy, Elif Berna; Uçar, Gökhan; Bilgin, Burak; Yekedüz, Emre; Arslan, Çağatay; Alkan, Ali; Dede, İsa; Tuncay, Gülseren; Hocazade, Cemil; Yıldırım Özdemir, Nuriye; Şendur, Mehmet Ali Nahit; Utkan, Güngör; Yalçın, Bülent; Akbulut, Hakan
Objectives: To evaluate the predictive factors of lymph node involvement in non-metastatic colorectal adenocarcinomas (nmCRC). Methods: A total of 453 patients diagnosed with nmCRC were analyzed regarding T stage, lymphovascular invasion status, tumor grade and proposed risk score (RS), determined by the combination of these three factors for lymph node metastasis. Results: The median age was 62 (25-90 years), M/F ratio was 1.4:1 and majority of the patients had tumors localized on the left colon (70.6%). The number of excised lymph nodes was ?12 in 77% of the cases. The postoperative pathological assessments revealed that 57.2% of patie, nts had N0 disease, 29.1% had N1 disease, and 13.7% had N2 disease. The T stages (p=0.007), grade (p<0.001), lymphovascular invasion (p=0.002), RS (p<0.001), and number of excised lymph nodes (p=0.029) were significantly different between N0, N1, and N2 patients. Higher RS was associated with lymph node metastasis (p<0.001). Conclusion: The risk score may predict lymph node metastasis in patients with nmCRC and if validated may be helpful in the decision-making of adjuvant chemotherapy, especially in the elderly and patients with inadequate lymph node dissection.

The relationship between metabolic parameters, 18F-fluorodeoxyglucose/positron emission tomography, and ras mutation in patients with metastatic colorectal cancer: A descriptive cross-sectional study
(Kare Publishing, 2022) Karcı, Ebru; Soydal, Çiğdem
Objectives: This study aimed to predict the RAS mutation by using imaging techniques and routine clinical or laboratory findings without tissue samples. Methods: The study was conducted in a retrospective cross-sectional plan in a tertiary-care health center between January 2010 and December 2016. Data collection was done from the patient files using the hospital’s electronic patient registry. The primary outcome variable was the presence of RAS mutations as evaluated from the primary surgical specimens. Besides, data was collected on blood count parameters, serum CEA, and CA 19-9 levels. Neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) were calculated. Forty-five patients who underwent 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) with pathologically confirmed metastatic colorectal adenocarcinoma were included in the study. Results: In our study the presence of RAS mutation was 40%(n=18). When the findings were compared according to the presence of RAS mutation, a statistically significant difference was found only in age at diagnosis (p=0.038). TLG (Total Lesion Glycolysis) significantly correlated with all other variables and age at diagnosis (p<0.05). A logistic regression model with age at diagnosis and TLG as explanatory variables had a sensitivity of 70.6% and a specificity of 81.5% in detecting RAS mutation. Conclusion: Although data on TLG and RAS mutations are valuable, they should be supported by studies with a larger cohort.

The static and free vibration analyses of axially functionally graded elliptical beams via mixed FEM
(Golden Light Publishing, 2022) Ermiş, Merve; Akdoğan, Gökay; Kır, Oğuzhan; Arıbaş, Ümit Necmettin; Omurtag, Mehmet Hakkı
The objective of this study is to investigate the behavior of the static and free vibration analyses of axially functionally graded elliptical planar curved beams using a mixed finite element method (MFEM) based on the Timoshenko beam theory. A two-noded curved mixed finite element has 12 field variables at each node. These variables denote three displacements, three cross-sectional rotations, three forces, two bending moments, and torque, respectively. The functionally graded material is composed of ceramic-particle material and metal-matrix material. The volume fraction of ceramic and metal materials varies along the beam axis. The effective material properties (modulus of elasticity, Poisson's ratio, and density) of the functionally graded material are determined according to the rule of mixture. It is aimed in the benchmark examples to present the influence of ceramic-particle material and non-homogeneity index of material gradation, the minimum radius of the elliptical beam, and boundary condition to the results of static and free vibration analysis in detail.
