    Botulinum toxin a augmentation of strabismus surgery for large-angle strabismus a retrospective case series and literature review

    Purpose: To report the results of botulinum toxin A (BTA)–augmented bilateral recession or monocular recession/resection surgery in a case series of patients with large-angle strabismus and review the relevant literature. Methods: This retrospective study reviewed the medical records of 24 patients treated with BTA-augmented strabismus surgery for large-angle horizontal strabismus (55 prism diopters or greater) with a minimum 6-month follow-up. Patients underwent unilateral recession-resection or bilateral rectus muscle recession surgery with intraoperative BTA injection into the recessed muscle(s). Main outcome measures were postoperative angle of deviation and success rate, defined as a final deviation of 10 prism diopters or less. Results: Of 24 patients (20 esotropia, 4 exotropia), the mean age was 9.2 years (range: 1 to 64 years). Mean preoperative deviation was 61.2 prism diopters for esotropia and 66.2 prism diopters for exotropia. Mean final postoperative deviation was 6.6 prism diopters for esotropia and 14.5 prism diopters for exotropia. Successful outcomes were achieved in 75% of patients with esotropia and 50% of patients with exotropia. Conclusions: BTA augmentation of conventional strabismus surgery dosages is a promising approach for managing large-angle strabismus, particularly esotropia. Optimal patient selection and follow-up are needed to maximize surgical success rates.

    Hunting glioblastoma recurrence: glioma stem cells as retrospective targets

    Glioblastoma (GBM) remains one of the most aggressive and treatment-resistant brain malignancies in adults. Standard approaches, including surgical resection followed by adjuvant radio- and chemotherapy with temozolomide (TMZ), provide only transient control, as GBM frequently recurs due to its infiltrative nature and the presence of therapy-resistant subpopulations such as glioma stem cells (GSCs). GSCs, with their quiescent state and robust resistance mechanisms, evade conventional therapies, contributing significantly to relapse. Consequently, current treatment methods for GBM face significant limitations in effectively targeting GSCs. In this review, we emphasize the relationship between GBM recurrence and GSCs, discuss the current limitations, and provide future perspectives to overwhelm the challenges associated with targeting GSCs. Eliminating GSCs may suppress recurrence, achieve durable responses, and improve therapeutic outcomes for patients with GBM.

    Milk ladder: who? when? how? where? with the lowest risk of reaction

    The milk ladder (ML) approach, which is the gradual reintroduction of the milk allergen from the least allergenic forms to the most allergenic forms into the diet of the patients, has been utilized mostly in non-IgE-mediated but in some countries also in IgE-mediated-CMPA due to its possible benefits which include nutrition, quality of life and tolerance induction. Despite increasing interest, so far, there is no guideline on ML; thus, the use of this approach shows discrepancies among healthcare professionals as many factors such as dietary habits, patient history, test results, workload, and facilities of the hospitals, the anxiety of the parents/patients may affect the decision on how, when, where and whom to use ML. Here, we reviewed current data on implementing the ML, suggested a 4-step ML including receipts and amounts, and shared our experience on optimal patient selection, appropriate time and steps for initiating ML, and time intervals between the steps targeting the lowest risk of reaction. We also added the newly developed twice-baked biscotti cake to the ML. We presented the analyses of this product, showing its low allergenicity compared to conventional cake, which provides a safer introduction of milk into the diet.

    Do we know about dynapenia?

    Dynapenia is a condition characterized by decreased muscle strength and function in older adults that is not due to a spe-cific underlying disease or medical condition. Dynapenia is common among older adults and has significant health effects, including functional impairment, disability, increased risk of falls, hospitalization, and death. Oxidative stress, mitochondrial dysfunction and chronic inflammation are involved in the etiopathophysiology of dynapenia. Diagnosis of dynapenia is based on the evaluation of muscle strength and function using methods such as hand grip strength, timed up and go test and short physical performance battery. Management of dynapenia involves a multifaceted approach that includes exercise, nutrition, pharmacological interventions, management of underlying medical conditions, and fall prevention strategies. With appropriate interventions, older adults with dynapenia can improve muscle strength and function, reduce the risk of falls and disability, and maintain their independence and quality of life.

    Depressor labii inferioris muscle palsy after chin augmentation

    The depressor labii inferioris (DLI) muscle helps to lower and turn the lower lip outward and receives innervation from the marginal mandibular branch of the facial nerve. Paralysis of this muscle is a rare but potential complication of chin augmentation injections. Paralysis of the DLI causes symptoms such as difficulty smiling on the affected side, difficulty speaking clearly, and facial asymmetry, especially when smiling. In this case report, we present two female patients, aged 29 and 36 years, who were evaluated for impaired lower lip movement, asymmetry in smiling, and slurred speech following submandibular liposuction and chin augmentation. The patients were found to have DLI palsy, closely followed with facial exercises, and recovered completely within three months. Although isolated DLI palsy is rare, it may occur as a complication of various maxillofacial surgeries. Careful technique and good knowledge of facial anatomy are important to prevent complications.

    Acute pancreatitis (AP) is a clinical condition that arises acutely in the pancreas through various inflammatory pathways due to multiple causes. Turkish Society of Gastroenterology Pancreas Working Group developed comprehensive guidance statements regarding the management of AP that include its epidemiology, etiology, clinical presentation, diagnostic criteria, disease severity, treatment, prognosis, local and systemic complications. The statements were developed through literature review, deliberation, and consensus opinion. These statements were ultimately used to develop a conceptual framework for the multidisciplinary management of AP.

    Botulinum toxin injection versus lateral internal sphincterotomy for chronic anal fissure: a meta-analysis of randomized control trials

    Background: Anal fissures, tears in the epithelium of the anal canal that cause pain and bleeding, have a lifetime prevalence of 11%. While surgical treatments, such as lateral internal sphincterotomy are traditional, they pose postoperative complications. Recent studies investigated less invasive options involving botulinum toxin injection, showing promise with fewer adverse effects. The aim of this study is to compare the outcomes of botulinum toxin injection to lateral internal sphincterotomy for chronic anal fissures. Method: Up to October 2023, an extensive literature search was conducted in PubMed, Scopus, and Google Scholar to identify relevant papers. This systematic review and meta-analysis examined the comparative effectiveness of lateral internal sphincterotomy and botulinum toxin injection in the treatment of chronic anal fissures. The methodology adheres to PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) criteria, and the study protocol has been registered with the Open Science Framework (OSF). Results: A total of 1,839 patients from 18 trials were included in the meta-analysis. Patients undergoing lateral internal sphincterotomy had higher healing compared to botulinum toxin injection (pooled effect = 0.77; 95% CI= [0.69– 0.83]; I2 = 90.95%; P = 0.00). Conclusion: Our study revealed the efficacy of lateral internal sphincterotomy over botulinum toxin injection in promoting healing for chronic anal fissures. These findings emphasize the clinical advantage of traditional surgical interventions in the management of this condition. However, further studies with long-term follow-up are required to confirm our observations.

    Optimizing outcomes in heart transplantation: the role of high-intensity statin therapy

    Heart transplantation is a vital procedure for patients with end-stage heart failure, but it faces significant challenges, including graft dysfunction, rejection, and cardiac allograft vasculopathy (CAV), which can compromise long-term graft success. Research suggests that statin therapy may offer significant benefits to heart transplant recipients, such as improved long-term survival and reduced rates of graft rejection and mortality. The aim of this review is to thoroughly examine the recent literature on this topic since 2005. Early use of high-dose statins appears to be particularly effective in preventing vasculopathy and improving outcomes, although a titrated approach may help to reduce side effects. High-dose statins may provide superior cardiovascular benefits, including lower rates of CVD, slower progression of CVD and improved long-term graft survival. Despite potential concerns about adverse effects, evidence suggests that high-intensity statins improve cholesterol levels without increasing serious adverse events after transplantation. The goal of statin therapy in heart transplant recipients is to balance the well-established benefits seen in the general population with the specific needs of this group, with the ultimate goal of improving both longevity and quality of life.
