    A contraindication to metfromin therapy: Renal impairment - Adherence to prescribing guidelines at a hospital in Turkey

    Objective: The aim of this study was to investigate whether renal functions of the patients were monitored and checked before and during metformin treatment as recommended in guidelines or whether they were disregarded and metformin was prescribed despite the contraindication of renal impairment in a hospital in Turkey. Method: This retrospective cross-sectional study was conducted among the patients who were hospitalized at a university hospital, diagnosed with type 2 diabetes mellitus and had metformin included in their treatment between 2015-2016. The total number of patients with this diagnosis and treatment between these years was determined as 66 and all the patients were taken into the study. Renal functions of these patients were assessed by measuring serum creatinine levels and calculating GFR using the Cockcroft- Gault formula. Results: During the duration of metformin treatment 10 patients (15%) were not monitored for their serum creatinine. The 56 patients who were monitored for their serum creatinine were at 1, 2, 3, 4, 5 (end-stage) renal failure stages with the rates of 30, 36, 30, 4, 0% respectively. Conclusion: Assessment of renal function, adjustment of drug doses accordingly and termination of the treatment when contraindicated, are essential strategies for metformin therapy to prevent medication errors. However, this study showed that adherence to these prescribing rules are low and in some patients renal function was not monitored, placing them in increased risk of lactic acidosis.

    A prospective study concerning the effect of pharmaceutical care services on patients with heart failure

    Objective: Heart failure (HF), caused by an abnormality in cardiac function, is the inability of heart tissue to pump blood or deliver sufficient oxygen, resulting in abnormal diastolic volume. Drug-Related Problems (DRPs) can cause significant but preventable morbidity and mortality once specific medication errors and their contributing factors are identified. The aim of this prospective study is to determine the effect of pharmaceutical care in patients with heart failure in a Turkish hospital. Methods: A total of 160 patients with heart failure (80 patients in the control group, 80 patients in the intervention group) were examined at a university hospital. The results of the Pharmaceutical Care Survey were evaluated in accordance with the objective of the study. In addition, using the Pharmaceutical Care Network Europe (PCNE) classification system V8.01, the role and importance of the clinical pharmacist in identifying, preventing and resolving drug-related problems encountered during the treatment of two groups was assessed. The number and causes of potential DRPs were taken into scrutiny. Results: Comparing the results of the Pharmaceutical Care Survey in both groups at the end of the 6th month, the study group shows a significant improvement in the rates of "forgetting to take medication" (2.9%) and "experiencing any side effects from your drug" (4.5%). Compared to other problems, ineffectiveness of the drugs used in treatment was reported as the most common drug-related problem (n=23; 28.7%) in the study group (p<.05). 72.5% of the proposed interventions were accepted and the problem was found to be resolved in 31% of the patients. Conclusion: In this study, it is discussed that clinical pharmacists can play an active role in resolving DRPs in heart failure patients. It is therefore can be predicted that the training of information and warnings conveyed by the clinical pharmacist to the intervention group will make a significant contribution to the health of the patient within the framework of pharmaceutical care.

    A survey on awareness, knowledge, and attitudes toward epilepsy in an urban community in turkey

    Background and Aim: Epilepsy is one of the most common chronic neurological disorders with a high prevalence. Epileptic people and their family members suffer more from social stigma than the disorder itself. Among various complex reasons knowledge and awareness about epilepsy are the two important factors underlying discriminatory attitudes towards epileptic people. Community pharmacists play a major role in the care of these patients. In this study we mainly aimed to gain insights into the knowledge and awareness of and attitudes (AKA) towards epilepsy both in epileptic and healthy individuals in an urban community. To this end we also aimed at developing a reliable and valid measurement tool to assess AKA levels. Materials and Methods: This study was conducted in 13 community pharmacies with 219 respondents. Factor analysis yielded three clear subscales. Results: It was found that a vast majority of the participants were familiar with epilepsy; yet only 18 of them had detailed information. The community pharmacists were indicated as a main source of information about epilepsy at the same rate to that of physicians. Although most of the respondents knew that epilepsy was not a form of mental illness only about one forth of them knew the real cause. More than half of the respondents supported the epileptics' socialization in the community. Conclusion: We believe that the questionnaire developed in the study is a promising instrument for determining educational needs and offering guidance to healthcare professionals in developing standardized educational tools and programs.

    Bronşit tanısı almış pediyatrik hastalarda potansiyel ilaç-ilaç etkileşimlerinin incelenmesi ve değerlendirilmesi

    Akut bronşit, çocuklarda en sık görülen ve hastaneye yatış sebeplerinin başında gelen enfeksiyon hastalıklarından biridir. Akut bronşit tedavisi genellikle semptomatiktir, analjezik ve antipiretik ilaçlar sıklıkla kullanılmaktadır. İlaçların eş zamanlı kullanımı beraberinde ilaç etkileşimlerini getirmektedir. Bu etkileşimler tedavinin başarısı açısından önem arz etmektedir. Araştırmamızın amacı bronşit tanısı olan pediyatrik hastalarda görülen ilaç-ilaç etkileşimlerinin değerlendirilmesidir. Çalışmamız, 2019-2021 yılları arasında Rize Özel Şar Hastanesi'nde yatarak tedavi gören bronşit tanılı 48 erkek ve 51 kız hasta olmak üzere toplamda 99 birey üzerinden yürütülmüştür. Hastaların yaş ortalaması 5,17±2,31 olarak bulunmuştur. Hasta kayıtları geriye dönük taranarak ilaç etkileşimleri belirlenmiştir. Bu etkileşimler Medscape veri tabanından yararlanılarak ortaya çıkartılmıştır. Yaptığımız bu çalışma sonucunda toplamda 69 ilaç etkileşimi bulunmuş ve bunların çoğunluğunun (%63,6) monitör düzeyde etkileşim olduğu belirlenmiştir. Etkileşim sayısının, orderdeki ilaç adedi ile pozitif yönde istatistiksel olarak anlamlı ilişkili olduğu bulunmuştur (p<0,001). Etkileşim sayısı ve yaş arasında anlamlı ilişki saptanmamıştır (p=0,271). Çalışmamızda kimi hastalarda hiç etkileşim görülmezken, en fazla 4 etkileşim görülen hastalarımız olmuştur. Bu çalışmapediyatrik hastalarda ilaç etkileşimleri ve bu etkileşimlerin önlenmesinde akılcı ilaç kullanımının önemini vurgular niteliktedir.

    Chronobiological efficacy of combined therapy of pelargonium sidoides and melatonin in acute and persistent cases of COVID-19: A hypothetical approach

    Since the outbreak of the first SARS-CoV-2 epidemic in China, pharmacists have rapidly engaged and developed strategies for pharmaceutical care and supply. According to the guidelines of the International Pharmaceutical Federation (FIP), clinical pharmacists/hospital pharmacists, as members of care teams, play one of the most important roles in the pharmaceutical care of patients with COVID-19. During this pandemic, many immuno-enhancing adjuvant agents have become critical in addition to antivirals and vaccines in order to overcome the disease more easily. The liquid extract obtained from the Pelargonium sidoides plant is used for many indications such as colds, coughs, upper respiratory tract infections, sore throat, and acute bronchitis. The extract obtained from the roots of the plant has been observed to have antiviral and immunomodulatory activity. In addition to its anti-inflammatory and antioxidant effects, melatonin plays a role in suppressing the cytokine storm that can develop during COVID-19 infection. Knowing that the severity and duration of COVID-19 symptoms vary within 24 hours and/or in different time periods indicates that COVID-19 requires a chronotherapeutic approach. Our goal in the management of acute and long COVID is to synchronize the medication regimen with the patient’s biological rhythm. This chapter provides a comprehensive review of the existing and emerging literature on the chronobiological use of Pelargonium sidoides and melatonin during acute and prolonged COVID-19 episodes.

    Comparison of 3 doses of aloe vera and burn drugs in market on burnt rat models

    Burn injuries are an incident that can have serious traumatic consequences. Degree of tissue damage depends on height of heat and duration of contact. It can be classified as 4 different burn degrees. Herbal treatments can be applied for alleviating the symptoms alongside with pharmacological treatments in clinic. Aloe vera are used for burn cases for so many years because of its antiinflammatory, antiviral, antifungal, antibacterial, antidiabetic and rapid burn wound healing effects. Our aim in this study is to compare different doses of Aloe vera gel with burn medications in market. 48 female Sprague-Dawley rats divided into 8 groups (negative control, positive control, Silverdin®, Bepanthol®, Sudocrem®, Aloe vera 30 mg, Aloe vera 60 mg, Aloe vera 90 mg). For burn modelling, back of the rats were exposed to 90 o C hot water which in the 1,5 cm diameter falcon tube for 10 seconds (burnt area: 7,069 cm2). After the modelling, serum physiologic was applied to each rat. Their treatments were always given at the same time. Experimental study was carried out with 14 numbered approval of Istanbul Medipol University Animal Experiments Local Ethical Committee. Obtained data were evaluated with SPSS v21 programme. After 15 days treatment, 30 mg of Aloe vera was more effective than Silverdin® cream (Aloe vera decreased burnt area: 3,064 cm2, Silverdin® decreased burnt area: 3,731 cm2). The most effective drug among these medications is Sudocrem® with decreased burnt area 2,302 cm2. Consequently, it can be seen that Sudocrem® is the most effective drug when comparing with these medications for burn treatment, and 30 mg dose of Aloe vera is better than Silverdin® cream.

    Evaluation of drug-drug interactions and side effects in COVID-19 patients in an intensive care unit

    BACKGROUND/AIM: Considering comorbidity rates in patients diagnosed with coronavirus disease 2019 (COVID-19), polypharmacy will be inevitable when the treatment for COVID-19 is added to the treatment of existing chronic disease conditions. In our study, we evaluated the effects of comorbidities, drug-drug interactions and complications on the clinical course of the disease in COVID-19 patients. MATERIALS AND METHODS: This study was conducted retrospectively with thirty five COVID-19 patients of various age and gender groups who had been admitted to the intensive care unit in a university hospital in March and April, 2020. The demographic, laboratory and clinical data were collected. RESULTS: In our study, the average number of days intubated in patients with acute respiratory distress syndrome and sepsis was found to be statistically significantly higher than those without complications. serious-use alternative interaction was detected in 85.7% of the patients, monitor closely interaction in 60%, and minor interaction in 34.3%. In 88.6% of the patients, at least one of these interactions was observed, while all three interactions occurred at the same time in 20% of them. CONCLUSION: According to the results of this study, managing the risks, interventions such as drug dosage adjustment, and drug changes and monitoring of any parameters that may indicate drug side effects for the patient may be necessary.

    Evaluation of probiotic use of the students of Istanbul Medipol University

    This research; is planned to determine the level of knowledge about probiotic nutri-tions and, the consumption of them by university students. The date range of the study was 2018. A total of 100 students were surveyed at the İstanbul Medipol University, 47 men and 53 women aged 18-30 years. General information and demographic status of participants were questioned. Also, height and weight data were recorded by their written notifications. In parallel with the ever-increasing work on the positive effects of probiotics on health; increased consumption of probiotics and increased knowledge. Microorganisms can directly or indirectly cause the formation of many diseases. 20% of the students consume these nutrients on the recommendation; and 24% of them did not consume it because they think did not need it. Although probiotic dairy products are mostly used for symptoms of constipation, there is not enough information in the context of other diseases.

    Evaluation of venous thromboembolism prophylaxis in cancer patients: A retrospective study

    This study aims to evaluate the prophylaxis of venous thromboembolism (VTE) in cancer patients. This retrospective study included 100 adult patients of different ages and genders who applied to the University Hospital between January and November 2021, who were diagnosed with cancer, admitted to the intensive care unit (ICU), and received chemotherapy. The patients were evaluated by clinical pharmacists during their ICU hospital stay in accordance with recommendations from the National Comprehensive Cancer Network, the American Society of Clinical Oncology, and the International Society for Thrombosis and Homeostasis Recommendations. Anticoagulant prophylaxis was indicated in all of our patients (100 patients) admitted to the ICU, and 38 (38%) of our patients received anticoagulant prophylaxis during their hospital stay. Enoxaparin sodium was the preferred anticoagulant drug for 38 patients. During outpatient cancer treatment of patients, 27 of our 100 patients had a Khorana risk score greater than 2 and these patients were recommended to receive prophylactic anticoagulant therapy during outpatient treatment. Results of this study showed that, oncology team members should be educated about factors that significantly increase VTE risk, and there is an urgent need to improve VTE awareness and practice of thromboprophylaxis in clinical practice.

    Hypertension-related knowledge, attitudes, and behavior among hypertensive patients in a community pharmacy in Sivas, Türkiye: A regional descriptive study

    This study scrutinized hypertension-related knowledge, attitudes, and behavior among patients with hypertension. It was performed with 88 hypertensive patients in a community pharmacy in Sivas for one month. Suitably designed and pretested questionnaire was used. Chi-square test was used and p<0.05 was accepted as significant. More than half of the participants correctly explained that people have a family history of hypertension, diabetes, and overweight people. Among preventive factors, only being physically active, reduced alcohol intake, and smoking cessation had significance between males and females. When examined attitude, participants opined that hypertension was a chronic disease that needs to be treated and can be controlled for life long and reduction of alcohol intake, and smoking cessation can prevent hypertension. Behavioral parameters such as regular monitoring of blood preessure, not smoking and reduction of alcohol intake showed significance between males and females. Participants had good knowledge, while they had poor attitudes and behaviors.

    İlaç tedavi karmaşıklık puanının tedavi uyumuna etkisi

    Bu çalışmada Morisky Uyum Ölçeği kullanarak en az 3 ay süreyle ilaç kullanmış 18 yaş üstü hastaların tedavi uyumunun değerlendirilmesi ve İTKÖ (İlaç Tedavisi Karmas?ıklık Ölçeg?i) ile ilaç tedavisi karmaşıklığının puanlandırılması, aralarındaki ilişkinin incelenmesi amaçlanmıştır. Bu çalışmaya en az 3 ay süre ile ilaç kullanan, çalışmaya katılmaya gönüllü olan 18 yaşından büyük hastalar dahil edilmiştir. Çalışmaya katılan hastaların çalışmaya dahil edilmesinde az 3 ay süre ile kullandığı ilaçları, yararlandıkları sağlık sigortası sisteminden belgelendirme şartı aranmıştır. Çalışmaya katılan hastaların %62,4'ü (n=126) kadın, %37,6'sı (n=76) erkektir. Bilgi düzeyi yüksek olan hasta grubunda astım tanı oranı istatistiksel olarak anlamlı düşüktür (p=0,047). Bilgi düzeyi yüksek olan hastalar motivasyon açısından kendi içinde değerlendirildiğinde motivasyonu yüksek olan hastaların yüzdesi, motivasyonu düşük olan hastaların yüzdesine göre istatistiksel olarak anlamlı düzeyde yüksektir (p<0,001). Motivasyonu yüksek/düşük hastaların Tedavi Rejim Karmaşıklık İndeksi düzeylerinde istatistiksel olarak anlamlı fark saptanmadı (p=0,842). İTKÖ puanı yaş ve ek hastalık sayısı ile pozitif yönde istatistiksel olarak anlamlı ilişkili saptandı (her ikisi için p<0,001). Erkek cinsiyette KAH oranı kadınlara göre istatistiksel olarak anlamlı yüksek, depresyon ve diğer hastalık oranı kadınlarda erkelere göre istatistiksel olarak anlamlı yüksek saptandı (p<0,001 p=0,031 p=0,002). Hastalıklara göre İTKÖ puanı değerlendirildiğinde DM, HL, KAH, glokom, KOAH, astım hastalıklarına sahip olan hastalarda istatistiksel olarak anlamlı fark bulundu (p<0,001). Sonuç olarak; tedaviye uyum veya uyumsuzluk ya hep ya hiç şeklinde olmayıp tam uyum, kısmi uyum veya hiç uymama şeklinde görülebilir. Tedavi uyumunu için tek bir form yok, tedavi uyumu kişiye göre çok değişkeni olan bir dengedir. Her değişken her hastayı farklı etkileyebilir. Multidisipliner çalışma uyumdaki başarıyı arttırabilir.

    Investigation of the diabetic patients’ knowledge on diabetes and the role of clinical pharmacist in providing pharmaceutical care to them

    Type 2 Diabetes is a common incurable metabolic disorder with worldwide increasing prevalence. International Diabetic Association noticed that there is a very fast and alarming increase in the worldwide prevalence of Diabetes. In 2012, diabetic popu-lation around the world has reached 371 million people. Untreated or undertreated Type 2 Diabetes does affect the quality of life of the patients both directly by symp-toms and indirectly by short term and long-term complications. In addition to the direct effects of type 2 diabetes mellitus, affects the patient’s life in a bad way, if the treatment process is not followed carefully, the quality of life of the patient will be re-duced. At this point, clinical pharmacist has an extremely important role in determin-ing and executing the correct clinical treatment. Besides that, the importance of the patient’s level of knowledge about the disease and its impact on the treatment process is very important. The major goal of this thesis is evaluating the diabetic patients` knowledge on diabetes and the effect of this point on treatment process. This study has been done in Merve Pharmacy of Muğla/Turkey between January and March of 2018. A questionnaire consisting of 20 multiple choice questions has been used to evaluate 50 participants who are clinically diagnosed diabetics. SPSS 23.0 statistical program was used for the statistical analysis of the data and the findings were evaluated at and 5% significance level. It has been statistically observed that knowledge levels of patients affected the treatment process. Recognizing the importance of this factor by clinical pharmacists and other health professionals will increase the training for diabetes and prevent the problems arising from the patient’s low-level knowledge.

    Kalp yetmezliği olan hastalarda farmasötik bakımın etkisi üzerine prospektif bir çalışma

    Bu çalışmanın amacı, kalp yetmezliği bulunan hastalarda klinik eczacının sunacağı farmasötik bakım hizmetleri kapsamında hasta eğitiminin ilaç uyuncuna, ilaçla ilgili problemler (İİP)'e ve yaşam kalitesine etkisini belirlemektir. Bu tez çalışmasında, İstanbul Medipol Üniversitesi Hastanesi'nde yatan 160 kalp yetmezliği hastası (çalışma grubu: 80 hasta; kontrol grubu: 80 hasta) klinik takibe alındı. Her iki grupta 'Morisky Ölçeği', 'SF-36 Yaşam Kalitesi', 'Farmasötik Bakım' anketi uygulandı ve PCNE V8.0 sınıflandırma sistemini kullanarak hastaların tedavi sırasında karşılaşılan ilaçla ilgili problemlerinin belirlenmesi, önlenmesi ve çözümlenmesindeki klinik eczacının rolü ve önemi değerlendirildi. 'Morisky Ölçeği' ile 6 ay sonunda her iki grup arasında tedaviye uyunç değerlendirildiğinde çalışma grubunda yüksek oranda uyunç sağlandığı sonucuna ulaşılmıştır. 'SF-36 Yaşam Kalitesi' anketinin sonuçlarına ilişkin genel sağlık değişim puanları kontrol ve çalışma grupları arasında anlamlı farklılık göstermektedir (p<0,05). 'Farmasötik Bakım' anketinin sonuçları 6. ay sonunda iki grupta da karşılaştırıldığında 'ilaç almayı unutma' (%2,9) ve 'ilacınızdan kaynaklanan herhangi bir yan etki yaşama' (%4,5) oranlarında çalışma grubunda önemli ölçüde iyileşme görülmektedir. Diğer sorunlar ile karşılaştırıldığında, tedavide kullanılan ilacın etkisizliği en yaygın (n=23; %28,75) ilaçla ilgili sorun olarak belirlenmiştir (p <0,001). Müdahale önerilerinin %72,5'i kabul edilmiş, %31,25 hastada problemin çözüldüğü saptanmıştır. Sonuç olarak, ilaca ilişkin sorunların hasta eğitimiyle çözülmüş olması önemli bir başarıdır. Çeşitli hastalıklara sahip bireylerin çoğunda ilaçla ilgili problemlere sık rastlanabilmektedir. Ancak tedavinin doğası gereği, kalp yetmezliği hastalarında, gelişme riski çok daha yüksek olarak bulunmuştur.

    Novel hybridized computational paradigms integrated with five stand-alone algorithms for clinical prediction of HCV status among patients: A data-driven technique

    The emergence of health informatics opens new opportunities and doors for different disease diagnoses. The current work proposed the implementation of five different stand-alone techniques coupled with four different novel hybridized paradigms for the clinical prediction of hepatitis C status among patients, using both sociodemographic and clinical input variables. Both the visualized and quantitative performances of the stand-alone algorithms present the capability of the Gaussian process regression (GPR), Generalized neural network (GRNN), and Interactive linear regression (ILR) over the Support Vector Regression (SVR) and Adaptive neuro-fuzzy inference system (ANFIS) models. Hence, due to the lower performance of the stand-alone algorithms at a certain point, four different novel hybrid data intelligent algorithms were proposed, including: interactive linear regression-Gaussian process regression (ILR-GPR), interactive linear regression-generalized neural network (ILR-GRNN), interactive linear regression-Support Vector Regression (ILR-SVR), and interactive linear regression-adaptive neuro-fuzzy inference system (ILR-ANFIS), to boost the prediction accuracy of the stand-alone techniques in the clinical prediction of hepatitis C among patients. Based on the quantitative prediction skills presented by the novel hybridized paradigms, the proposed techniques were able to enhance the performance efficiency of the single paradigms up to 44% and 45% in the calibration and validation phases, respectively.

    Obturator neuropathy combined with Guillain-Barre syndrome

    Obturator neuropathy caused by entrapment of obturator nerve is a disease with numbness, pain and weakness in legs. Guillain-Barre syndrome is an immune-me-diated peripheral polyneuropathy characterized by nearly same symptoms. 51-year-old man presented with severe shortness of breath. He was admitted to ICU and treated with mechanical ventilation followed complete blood count (CBC). We observed inflammation, started required medications and, we provided improvement in a short time. When patient was visited, he explained numbness, pain in hip area, muscle weakness in all extremities. Cervical MRI and nerve conduction tests were performed. MRI showed oedema in sciatic nerve and entrapment of obturator canal, resulted to obturator neuropathy and nerve conduction tests revealed GBS. He was treated with plasmapheresis and IVIG, he was discharged without neurological recovery. The patient consent was taken orally. In conclusion, treatments decreased complaints and provided to walk short distances. Reinnervation signals were observed in nerve conduction test.

    Role of clinical pharmacists in post-COVID management

    As the world begins to emerge from the COVID-19 pandemic, there is a critical need for healthcare providers to focus on post-COVID management. Clinical pharmacists can play a vital role in this effort, as they possess unique skills and expertise in medication management, patient education, and collaborative care. One of the key challenges in post-COVID management is addressing the long-term effects of the disease, commonly known as long COVID. Clinical pharmacists can help by assessing and managing the pharmacological treatment of these chronic conditions, as well as monitoring for potential drug interactions and adverse effects. In addition, clinical pharmacists can provide education to patients and other healthcare providers about the proper use of medications and the importance of medication adherence. This is especially important in the post-COVID era, as many patients may have experienced disruptions in their healthcare during the pandemic and may need support to resume their medication regimens. Finally, clinical pharmacists can work collaboratively with other healthcare providers, such as physicians and nurses, to optimize medication therapy and ensure that patients receive the best possible care. This includes reviewing medication orders, monitoring drug therapy, and providing recommendations for dose adjustments or alternative treatments. In conclusion, clinical pharmacists are well-positioned to play a critical role in post-COVID management. By leveraging their unique skills and expertise, they can help to address the long-term effects of the disease, provide education to patients and healthcare providers, and work collaboratively to optimize medication therapy.

    The determine of possible drug interaction of clarithromycin

    Phenytoin is an antiepileptic drug which is eliminated by CYP2C9, CYP2C19 (less important: CYP2C18, CYP3A4) enzyme. Therapeutic concentration range is very important due to the narrow therapeutic drug levels. Targeted therapeutic concentration range is 10–20 mg/L (40–80 lmol/L) in blood serum for adults and children older than 3 months. Phenytoin has highly variable pharmacokinetics due to its complex of binding to Albumin (%90). It is dif?cult to determine the correct dosage of this drug because of signi?cant dose related toxicity. It is also important for inpatient in intensive care unit.

    The importance of computerized drug interaction checker programs used in community pharmacies to avoid potential drug interactions: A preliminary study with clarithromycin

    Introduction: Drug-drug interactions (DDI) due to multiple drug use are the most important cause of adverse drug reactions. DDIs are among medication errors that can be prevented. The integrated computerized drug interaction checker programs, which medical professionals use in addition to their medical and practical knowledge, can help medical staff to reduce potential DDIs (PDDIs), although they are not sufficient alone. The aim of this study was to investigate the role of computerized drug interaction checker programs in the identification and prevention of PDDIs in clarithromycin prescribed pediatric outpatients. Methods: The study was a retrospective observational prescription analysis held in three community pharmacies operating in the province of Üsküdar-İstanbul during 12-month period. The prescriptions with oral clarithromycin medication were selected and PDDIs were analyzed using the RxMediaPharma® software on an active substance base. Results: During the 12-month period, 100 prescriptions containing 266 medicines were prescribed by 52 different physicians. The mean number of medicines per prescription was 2.66±1.11. Of the 16 PDDIs detected, five were clarithromycinrelated (31.25%) and 11 were non-clarithromycin-related (68.75%). When PDDIs were categorized by severity, 10 out of 16 (62.5%) were high-risk, two (12.5%) were moderate and four (25%) were low-risk. Clarithromycin interactions were moderate with lidocaine and low with both metronidazole and sultamicillin (ampicillin/sulbactam). Conclusion: Although evidence based computerized drug interaction checker programs that provide rapid access is not sufficient alone, they can help health care professionals in preventing PDDIs. The use of this program in the community pharmacies could minimize PDDIs during the dispensing of medicine.
