Öğe Associations between chronotype, sleep quality, suicidality, and depressive symptoms in patients with major depression and healthy controls(Informa Healthcare, 2010) Selvi, Yavuz; Aydın, Adem; Boysan, Murat; Atlı, Abdullah; Ağargün, Mehmed Yücel; Beşiroğlu, LütfüllahResearch interest concerning associations between sleep characteristics and suicidality in psychopathology has been growing. However, possible linkages of suicidality to sleep characteristics in terms of sleep quality and chronotypes among depressive patients have not been well documented. In the current study, the authors investigated the possible effects of sleep quality and chronotype on the severity of depressive symptoms and suicide risk in patients with depressive disorder and healthy controls. The study was conducted on 80 patients clinically diagnosed with major depression and 80 healthy subjects who were demographically matched with the patient group. All participants completed a questionnaire package containing self-report measures, including the Beck Depression Inventory (BDI), Pittsburgh Sleep Quality Index (PSQI), Morningness-Eveningness Questionnaire (MEQ), and Suicide Ideation Scale (SIS), and subjects were interviewed with the suicidality section of the Mini-International Neuropsychiatric Interview (MINI). Results are as follows: (a) logistic regression analyses revealed that poor sleep quality and depression symptom severity significantly predicted onset of major depression; (b) morningness-type circadian rhythm may play as a significant relief factor after onset of major depression; (c) sleep variables of chronotype and sleep quality did not significantly predict suicide ideation after controlling for depressive symptoms in the major depression group; and (d) suicide ideation and poor sleep quality were antecedents of depression symptom severity in patients with major depression, and in healthy controls. Findings are discussed under the theoretical assumptions concerning possible relations between chronotype, sleep quality, depression, and suicidality.Öğe Ongoing or re-emerging subjective insomnia symptoms after full/partial remission or recovery of major depressive disorder mainly with the selective serotonin reuptake inhibitors and risk of relapse or recurrence: A 52-week follow-up study(Elsevier, 2011) Güleç, Mustafa; Selvi, Yavuz; Boysan, Murat; Aydın, Adem; Beşiroğlu, Lütfullah; Ağargün, Mehmet YücelBackground: Many patients who remit fully/partially or recover from an episode of major depression continue to suffer from sleep problems, mainly insomnia. Our study assesses the frequency and types of ongoing or re-emerging subjective insomnia symptoms and their relationship to subsequent depressive relapse or recurrence in a group of full/partial remitters or recoverers.Method: Sixty patients, 18 to 65 years of age, with current MDD defined using DSM-IV-TR criteria were participated in the study. They began a 24-week course of open-label acute plus continuation phase treatment mainly with SSRIs. Maintenance treatment was not planned for not affecting natural relapse or recurrence rates. All participants were evaluated by CGI-S, BDI, HAMD-17, PSQI, and ISI at weeks 0. 4, 12, 24, and 52.Results: Majority of the participants (83.33%) had subjective sleep disturbance, insomnia prior to relapse or recurrence. Repeated measures ANOVA was used to evaluate changes in scores of depression and sleep scales over time between recurred (N =12) and non-recurred groups (N = 48). Differences between means were calculated with post hoc Bonferroni comparison test. The changes in scores of entire clinical scales over time between recurred and non-recurred groups were statistically significant Limitations: The limitations of this study include the use of a relatively small, mostly young female sample and the lack of an objective sleep measure to corroborate self-report scales.Conclusions: Early recognition and treatment of disturbances of the sleep-wake cycle may be important for treatment and prevention of recurrence of depression.Öğe Phenomenal self and dream self(Küre İletişim Grubu A.Ş., 2017) Kara, Hayrettin; Selvi, YavuzThe concept of self is a complicated issue; however, a conceptualization of dream self even reflects a more complicated process. Various historical and contemporary approaches, which need reconciliation and refinement, exist toward the topic of the self; these approaches spread through a wide spectrum. On the one hand is the thought that self is fictional; on the other hand, self is considered a solid concept. Of these approaches, the phenomenological approach has its own unique place capturing the issue from a broader view. While this phenomenological view argues that self is the subject of the self’s experiences, it also emphasizes the accessibility of self through consciousness. A descriptive introspection might access the basic phenomenal characteristics of the self. The present article basically argues that there is a phenomenal self in dreams that can be accessed through consciousness and thus, dreams are basically personal experiences of the self. Research into the phenomenological similarities and differences between the awake and dreaming self might help to better understand subjectivity and consciousness. On the other hand, if there exists a phenomenal self in dreams, which can be accessed through consciousness psychological dream theories need a radical revision.Öğe Sleep-related violence, self-mutilation, and dissociative experiences(Küre İletişim Grubu A.S., 2016) Ağargün, Mehmet Yücel; Beşiroğlu, Lütfullah; Güleç, Mustafa; Aydın, Adem; Selvi, YavuzTwenty-eight subjects with and 46 subjects without sleep-related violence (VBS) were evaluated regarding the presence of self-mutilative behaviors (SMBs). The subjects with and without VBS were compared using dissociative experiences scale (DES). The subjects with VBS had higher mean DES score and more frequent SMBs than those without VBS. Yöntemler: Araştırmanın örneklemini 50 birey oluşturdu. Bu bireyler birinci derece yakınlarından sevdiği birini en az 1 ay önce, en fazla 2 ay önce kaybetmiş bireylerdi. Bu gruba “yas grubu” adı verildi. İkinci grup olarak, birinci dereceden sevdiği bir yakınını 5 yıl içerisinde kaybetmiş olmayan 50 birey çalışmaya dahil edildi. Ölçeğin iç tutarlılığı Cronbach-· istatistiği ve Pearson bağıntı analizi ile araştırıldı. İki farklı uygulama zamanında CBI toplam puanlarının test-tekrar test güvenirliği bağımlı gruplarda t testi ve Pearson bağıntı analizi uygulanarak değerlendirildi. Yas grubu ve 2. grubun toplam puanları bağımsız gruplarda t testi kullanılarak geçerliğe ilişkin kanıtlar araştırıldı. Bulgular: Ölçek toplam puanı yeterli bir test-tekrar test güvenirliği göstermiş ve iç tutarlığı yüksek (?=0.94) bulunmuştur. İç tutarlıkta Pearson bağıntı katsayısı da her bir madde için 0.46-0.87 arasında bulundu. Geçerlik analizinde 4., 8., 9. ve 10. maddeler dışında maddelerin tümü ve toplam puanları açısından ayırt edici bir geçerliğe sahip olduğu görülmüştür. Sonuç: Sonuçlar, CBI’nın Türkiye örneklemi için yas sürecindeki bireylerde yasın Şiddetini ve gelişimini değerlendirmede geçerli ve güvenilir bir araç olarak kullanılabileceğini göstermektedir.Öğe The influence of the sleep-wake cycle on primary monosymptomatic nocturnal enuresis: A non-randomized comparative study(Taylor and Francis Ltd., 2016) Erdoğan, Fırat; Kadak, Muhammed Tayyip; Selvi, Yavuz; Kartal, Vural; Şenkal, Evrim; Ateş, Burçin; Akgül, MuratBackround: Enuresis implies severe stress in affected children, and impairs quality of life and sleep. Children with enuresis experience difficulties in their arousal from sleep, possibly associated with disturbances of the circadian rhythm. In this study, we aimed to evaluate the sleep-wake cycle and sleep disturbances in children with monosymptomatic enuresis nocturna (MEN). Method: The study comprised 70 children with MEN who were admitted to the pediatrics and urology outpatients department and 94 age-matched healthy controls. Parents completed "Strengths and Difficulties Questionnaire," Children's Sleep Habits Questionnaire (CSHQ), Children's Chronotype Questionnaire scale. Results: Children with enuresis had significantly more sleep and psychological problem. Enuresis group reported higher bedtime resistance, parasomnias, breathing-related problems, and daytime sleepiness in CHSQ (p < 0.05). Although circadian preference did not differ statistically between the groups (p > 0.05), sleep duration on school days and awakening and mid-sleep points, both on scheduled and free days, were found to be significantly different in the enuretic group (p < 0.05). In logistic regression analysis, age, sleep period on scheduled days, sleep inertia on scheduled and free days were significant predictor for enuresis. Discussion: Children with enuresis were more likely to experience problematic sleep. This may reflect that enuretic children have impaired sleep-wake cycles, leading to dysregulation of daily functional changes of bladder capacity and related hormones such as ADH. These findings might imply a sleep-wake disturbance in enuresis.
