    Compartment syndrome following arthroscopic removal of a bullet in the knee joint after a low-velocity gunshot injury

    Gunshot injuries are getting more frequently reported while the civilian (nongovernmental) armament increases in the world. A 42-year-old male patient presented to emergency room of Istanbul Medipol University Hospital due to a low-velocity gunshot injury. We detected one entry point on the posterior aspect of the thigh, just superior to the popliteal groove. No exit wound was detected on his physical examination. There was swelling around the knee and range of motion was limited due to pain and swelling. Neurological and vascular examinations were intact. Following the initial assessment, the vascular examination was confirmed by doppler ultrasonography of the related extremity. There were no signs of compartment syndrome in the preoperative physical examination. A bullet was detected in the knee joint on the initial X-rays. Immediately after releasing the tourniquet, swelling of the anterolateral compartment of the leg and pulse deficiency was detected on foot in the dorsalis pedis artery. Although the arthroscopic removal of intra-articular bullets following gunshot injuries seems to have low morbidity rates, it should always be considered that the articular capsule may have been ruptured and the fluids used during the operation may leak into surrounding tissues and result in compartment syndrome.

    Does intra-articular fracture change the lubricant content of synovial fluid?

    Background: Lubrication function is impaired and the lubricant content of synovial fluid (SF) changes immediately after plateau tibia fractures. Here, we aimed to analyze the lubricant content of SF at chronic term following plateau tibia fracture. Methods: Forty-eight surgically treated patients without joint incongruency (<2 mm displacement) were included in the study. Joint aspiration had been possible in 16 of the participants. However, sampling could be made from healthy knees in only ten of these patients. Twenty-six SF samples (16 injured knees, 10 healthy knees) were analyzed for concentrations of hyaluronic acid (HA), proteoglycan-4 (PRG4), TNF-alpha, IL-1 beta, and IL-6. Results: The group of experimental samples were obtained at a mean of 31 (12-66) months after injury from patients with a mean age of 45.1 (32-57) years. There were no relationships detected between biochemical analysis results and patient ages, sexes, postoperative time, and fracture type. After excluding six patients for whom we could not sample from their healthy knee, ten patients' values were compared with paired Wilcoxon signed rank test and no significant differences detected between the healthy and injured knee in terms of the SF concentrations of HA and PRG4 (p = 0.225 and 0.893, respectively). Similarly, there were no statistically significant differences in SF sample concentrations of TNF-alpha, IL-1 beta, and IL-6 between healthy and injured knees. Conclusions: Despite acute changes, the long-term concentrations of HA and PRG4 were similar after plateau tibial fracture. We could not detect any concentration level differences between healthy knees and injured knees regarding HA and PRG4 in the long-term follow-up.

    Does pelvic injury trigger erectile dysfunction in men?

    Purpose: Pelvic ring fractures constitute 3%-8% of all fractures of the skeletal system and are generally related with high energy trauma. Sexual dysfunction following pelvic fracture has a high incidence, and affects the male patients both physically and psychologically. In this study, we aimed to investigate the impact and frequencies of comorbidities such as erectile dysfunction (ED) with adverse sociocultural and psychological consequences for the patient who had a pelvic ring fracture. Methods: This study included 26 men who corresponded to the inclusion criteria and agreed to participate our study. Results: According to fracture types, most of our cases were Tile type A1 and type A2. Severe and moderate ED were detected in 46.1% (12/26) of these patients via the International Index of Erectile Function-5 questionnaire. Conclusion: ED develops following pelvic fractures, especially in Tile type B and C pelvic fractures.

    Effective of postoperative shoulder imbalance on patient stisfaction with surgial treatment of adolescent idiopathic scoliosis

    Objective: We aimed to evaluate whether there is a relation between the level of shoulder imbalance after scoliosis surgery and patients' satisfaction with treatment. Materials and methods: Twenty-three patients with adolescent idiopathic scoliosis (18 females, 5 males; mean age, 15.9 years; age range, 13-24 years), who were treated by posterior instrumentation and fusion and followed up for an average of 35.4 months (range, 24-67 months) postoperatively, were included in this retrospective clinical study. In order to evaluated shoulder balance on coronal plane, three parameters were measured on postoperative radiography: first rib angle, radiographical shoulder height, and clavicle angle. For the assessment of patients' satisfaction with scoliosis surgery, Scoliosis Research Society (SRS)-22r Patient Questionnaire was used.Results: Fusion was obtained in all patients. On radiography, mean first rib angle was 2.5º±2.8º, shoulder height was 6.0±5.4 mm, and clavicle angle was 1.7º ± 1.5º. The mean values for SRS22r domain scores were between 3.3 and 3.8, being lowest for mental health and highest for pain and self-image. There was no significant correlation between radiographic parameters and total or domain scores of SRS-22r.Conclusions: Shoulder imbalance is a common undesirable effect of correcting thoracic curve in surgical treatment of adolescent idiopathic scoliosis. However, unless it is severe, shoulder imbalance does not cause patient dissatisfaction.

    Erken ve geç evre osteoartritli hastalarda ağrı şiddeti ile beden algısı, benlik saygısı, depresyon ve fonksiyonel düzey arasındaki ilişkinin incelenmesi: Pilot çalışma

    Amaç: Bu çalışma, erken ve geç evre kalça ve diz osteoartrit (OA) tanılı hastalarda, ağrı şiddeti ile beden algısı, benlik saygısı,depresyon ve fonksiyonel düzey arasındaki ilişkiyi incelemek amacıyla planlandı.Yöntem: Çalışmaya diz ve kalça OA tanılı 55 gönüllü hasta dahil edildi. Hastalar, Kellgren Lawrence ölçütlerine göre erken evreve geç evre olmak üzere iki gruba ayrıldı. Hastaların ağrı şiddetleri Görsel Analog Ölçeği (GAÖ) ve Katastrofik Ağrı Ölçeği (KAÖ)ile, beden algıları Vücut Algısı Ölçeği (VAÖ) ile, benlik saygıları Rosenberg Benlik Saygısı Ölçeği (RBSÖ) ile, ruhsal durumlarıBeck Depresyon Ölçeği (BDÖ) ile ve fonksiyonel düzeyleri Zamanlı Kalk Yürü Testi (ZKYT) ile değerlendirildi.Bulgular: Çalışma bulguları, tüm OA’lı hastaların beden algılarının ve benlik saygılarının azaldığını, minimal seviyededepresyonlarının olduğunu gösterdi. Erken evre ve geç evre OA’lı hastalar arasında ağrı şiddeti ve fonksiyonel düzey bakımındanistatistiki fark bulundu (p<0.001). Erken evre OA’lı hastalarda, KAÖ ile VAÖ arasında pozitif yönlü düşük düzeyde bir ilişki olduğu(p=0.016) görüldü. Geç evre OA’lı hastalarda, KAÖ ile değerlendirme ölçekleri arasında bir ilişki bulunmadı.Sonuç: Çalışma sonuçları, hastalığın radyolojik evresinin ilerlemesiyle ağrı şiddetinin arttığını ve fonksiyonel düzeyin olumsuzetkilendiğini gösterdi. Ayrıca OA’lı hastalarda, ağrı şiddetinin hastalığın erken döneminden itibaren yarattığı olumsuzpsikosomatik etkileri nedeniyle beden algısını etkileyebileceği düşünüldü.

    Gelişimsel kalça displazisi için yapılan ultrasonografik tarama sonuçları ve risk faktörlerinin karşılaştırılması

    Amaç: Çalışmamızın amacı ultrasonografik taramanın seçilmiş hastalarda mı (risk faktörleri ya da muayene bulguları olan) ya da genel popülasyon üzerinde mi yapılması gerektiğini değerlendirmektir. Gereç ve Yöntem: Kliniğimize 2010-2012 gelişimsel kalça displazisi (GKD) taraması amacıyla başvuran 634 bebek çalışmaya dâhil edildi. Risk faktörleri olarak aile öyküsü, makat gelişi, tortikollis, ilk gebelik, oligohidramniyos, çarpık ayak, kız cinsiyet olarak belirlendi. Graf metodu ile yenidoğan kalçaları taranarak, hastalardaki risk faktörleri ve klinik muayene bulguları istatistiksel olarak duyarlılık ve özgüllük açısından karşılaştırıldı. Bulgular: Çalışmaya alınan 634 bebeğin 403'ü kız, 231'i erkekti. Hastaların ortalama yaşı 87,45±49,6 gündü. Ortalama anne yaşı 29,6±5,4 yıldı. Ultrasonografik tarama sonucunda 66 bebeğin 71 kalçası displazik bulundu. Bu kalçalardan 30'u Tip 2b, 8 kalça Tip 2c, 3 kalça Tip d, 23 kalça Tip 3, 7 kalça Tip 4 olarak tespit edildi. Sonuç: Çalışmamızda klinik bulguların GKD taraması açısından yeterli olmadığı sonucuna ulaşılmıştır. Bu nedenle de özellikle ülkemiz gibi GKD prevelansının sık görüldüğü ülkelerde seçilmiş hastalara ultrasonografik tarama yapmaktansa tüm popülasyonun ultrasonografik taramasının yapılması gerektiği düşüncesindeyiz.

    Hamstring otogreftleri ile yapılan ön çapraz bağ rekonstrüksiyonunda teknik yöntemin propriosepsiyon, denge, kas gücü ve ağrı üzerine etkisi

    Çalışmamızın amacı, ön çapraz bağ rekonstrüksiyonunda (ÖÇBR) tamamı içerde tekniğinin konvansiyonel tekniğe göre propriosepsiyon, kas gücü, ağrı ve denge açısından farklarını araştırmaktır. Çalışma, Medipol Mega Üniversite Hastanesi'nde tamamı içerde tekniğiyle ÖÇBR olan 20 kişi, konvansiyonel teknikle ÖÇBR olan 20 kişi ve 20 sağlıklı kişi olmak üzere toplam 60 kişilik 3 gruptan oluşmuştur. Hastalar cerrahiden 6 ay sonra değerlendirmeye alınmıştır. Hastaların; ağrı durumunu değerlendirmek için Vizüel Analog Ölçek (VAS) , diz aktivite düzeylerini değerlendirmek için Tegner Aktivite Düzeyi Ölçeği, fonksiyonel durumlarını değerlendirmek için Lysholm Diz Skorlama Ölçeği kullanıldı. Cybex izokinetik dinamometre ile kuadriseps, hamstring kas kuvveti ve propriosepsiyon ölçüldü. Ölçümler opere olmayan bacağa da uygulandı. Postüral stabilite ve düşme riski Biodex denge sistemi kullanılarak değerlendirildi. Sağlıklı grubun tegner aktivite puanları (6,000 ± 1,589), konvansiyonel ÖÇBR uygulanan grup (3,750 ± 1,713) ve tamamı içerde ÖÇBR uygulanan gruptan (4,100 ± 1,861) anlamlı olarak yüksek bulunmuştur. Grubu sağlıklı olanların lysholm diz puanları (97,650 ± 3,884), tamamı içerde ÖÇBR uygulanan gruptan (87,900 ± 19,331) anlamlı olarak yüksek bulunmuştur. Tamamı içerde ÖÇBR uygulanan grubun düşme riski indeksi puanları (0,770 ± 0,266), grubu sağlıklı olanların düşme riski indeksi puanlarından (0,570 ± 0,243) anlamlı olarak yüksek bulunmuştur. Postüral stabilitede, VAS skorunda, 60º/s, 90º/s ve 180º/s açısal hızlarda ölçülen fleksör ve ekstensör kas kuvvetinde ve 15, 45, 75 derecelerde bakılan propriosepsiyon duyusunda, her iki cerrahi grup kendi arasında ve sağlıklı grupla karşılaştırıldığında anlamlı fark bulunmamıştır.     Hybrid external fixation via a minimally invasive method for tibial pilon fractures -Technical note

    Introduction: We aimed to present the clinical and radiological outcomes of patients with tibial pilon fractures who were treated with hybrid external fixators. Shanz screws were applied synchronously and used as joysticks for fracture reduction. Radiological evaluations were conducted on roentgenograms at the second week, sixth week, third month and first year. Material and methods: The study group included 42 patients with tibial pilon fractures that were classified as 43C according to the AO/OTA classification system. We used 2-hydroxyapatite-coated Schanz screws for the tibial pilon reduction. Schanz screws fixated to two separate motors were synchronously passed through the fracture fragments. An external fixator was applied after the fracture was stabilized with Schanz screws. Results and discussion: The mean operation duration was 45 min Fracture healing was observed in all patients, and the mean fracture healing time was 17 (range, 12-32 weeks) weeks. The mean lateral distal tibial angle was 89°. Joint surface irregularity was not observed in any patient. All of the patients had 0-15° of ankle dorsiflexion. None of the patients had restricted ankle plantar flexion. No wound complications were observed. According to the AOFAS scoring system, the clinical evaluation was excellent in 26 patients, good in 14 patients and fair in 2 patients. Malunion and nonunion may necessitate additional surgical procedures, delay the return to activities of daily living, and increase treatment costs. Good alignment was achieved, with a mean lateral distal tibial angle of 89 (range, 84-92) degrees. Conclusion: Permanent hybrid external fixator applied using Schanz screws via a mini open technique is a fast, easily applied alternative with low morbidity and satisfying results.

    Is cold therapy really efficient after knee arthroplasty?

    Introduction: Knee arthroplasty is a frequently used surgery. The purpose of this study is to evaluate the effects of cold therapy after knee arthroplasty. Methods: 60 patients who were operated in our clinic between the years 2013 and 2014 were evaluated prospectively, and the patients were randomized into two groups. The cold therapy by Cryo/Cuff application was started 2 h before the surgery in Group-2 patients; it was repeated in a postoperative 6th hour and continued for postoperative four days. We analyzed the visual analog scales (VAS) for pain, Knee Society Scores (KSS), hemoglobin, and bleeding parameters. Results: The data of the group that did not receive the Cryo/Cuff application (Group-1) are as follows; the number of the patients was 33; the mean age value was 68,4 (53-78). Group-2 data was as follows; the number of the patients was 27; the mean age value was 67.2 years (range 57-78). The mean KSS scores of the patients increased from 79.6 (75-83) and to 90.5 (88-92) postoperatively (p < 0.05). Discussion: In our study, the pain and functional knee scores of the patients who received Cryo/Cuff application were significantly different and as expected, the use of analgesics was much lower. Cryotherapy did not affect the amount of bleeding during the surgery (p > 0.05), which was not reported previously in the literature. Conclusion: After knee arthroplasty, the preoperative and postoperative use of cryotherapy is effective in terms of the pain control and functional knee scores without a significant change in surgical blood loss.

    Is kinesiotherapy effective in relieving pain and reducing swelling after shoulder arthroscopy?

    PURPOSE OF THE STUDY The purpose of this study is to investigate the acute and long term effectiveness of kinesio taping applied following shoulder arthroscopy in relieving pain and reducing swelling. MATERIAL AND METHODS This study included 50 patients undergone shoulder arthroscopy between June 2016 and December 2017 in our clinic. Patients were randomly assigned into two groups. Group I consisted of patients who had kinesiotherapy; while Group II consisted of control patients whom we applied sham taping with no effect. Pre and postoperative pain and swelling status of patients were recorded and groups were compared. RESULTS Comparing two groups, we found that kinesio taping significantly reduced pain levels in the early post-operative period. However, we didn't detect any significant difference in reduction of shoulder swelling between two groups. CONCLUSIONS Kinesio taping could be an alternative treatment in relieving pain after shoulder arthroscopy. However, we didn't find a significant reduction in swelling in shoulder.

    Is there a relation between plantar fasciitis and total cholesterol levels?

    Background: The main objective of this study is to investigate the relationship between hypercholesterolemia and Plantar Fasciitis (PF). Methods: The study includes patients who presented to the orthopedics outpatient clinic with heel pain and were diagnosed with PF. The control group was composed of patients who came to the orthopedics outpatient clinic, with complaints other than heel pain. The two groups were compared in terms of epidemiological data, total cholesterol (TC) levels, and hypercholesterolemia prevalence. We also performed an in-group analysis of PF patients in terms of age, sex, body mass index, and duration of symptoms. Results: There were 238 patients (mean age, 46.7) in the PF group and 240 patients (mean age, 47.9) in the control group. There was a significant difference between the PF group and the control group in TC levels (207.6 +/- 47.5 versus 195.1 +/- 30.1, p = 0.001). Hypercholesterolemia (TC level > 240 mg/dL) was found in 22.7% (n = 54) of the patients in the PF group whereas in the control group this rate was 10.8% (n = 26) (p < 0.001). It was seen that the TC levels were significantly higher in patients over the age of 45 in the PF group (p = 0.038). We also found that TC levels were higher in PF patients with symptoms for longer than a year (p = 0.026). Conclusion: Significantly higher TC levels were found in PF patients in comparison with other orthopedic outpatients. Besides, being over the age of 45 and having a duration of symptoms longer than a year is associated with higher cholesterol levels for PF patients.

    Kinesiotaping reduces knee diameter but has no effect on differences pain and edema following knee artroscopy

    PURPOSE OF THE STUDY Investigating the efficacy of kinesiotaping for the reduction of postoperative pain and swelling after invasive knee arthroscopy, which tend to develop in almost every patient undergoing this procedure. MATERIALS AND METHODS Kinesiotaping or sham taping was performed in a total of 42 patients who underwent invasive knee arthroscopy. Pain intensities; mid-thigh, knee, mid-calf, and ankle diameters; and VAS (Visual Analogue Scale) were recorded preoperatively and postoperatively. RESULTS However knee diameter was significantly greater in the control group compared to the group there is no significant differences were observed between the two groups´ pain scores at the postoperative period. There were also no significant statistical differences between the two groups with respect to mid-thigh, calf, and ankle diameters. CONCLUSIONS Kinesiotherapy could be used as an effective treatment method to relieve knee effusion after knee arthroscopy. However, both groups did not have significant differences with respect to pain and edema levels.

    Long-term results of microfracture in the treatment of talus osteochondral lesions

    Osteochondral lesions of the talus are common injuries, and many clinicians consider arthroscopic debridement and microfracture as the first-stage treatment. This study assessed the long-term clinical and radiographic outcomes of arthroscopic debridement and microfracture for osteochondral lesions of the talus. A total of 82 patients (48 males, 34 females) who were treated with arthroscopic debridement and microfracture for osteochondral lesions of the talus between 1996 and 2009 with a minimum 5-year follow-up were included in our study group. Functional scores (AOFAS, VAS) and ankle range of motion were determined, and an arthrosis evaluation was performed. Subgroup evaluations based on age, lesion localization, and defect size were performed using functional outcome correlations. The mean age of the patients was 35.9 +/- A 13.4 years (14-69 years), and the mean follow-up period was 121.3 months (61-217 months). The mean defect size was 1.7 +/- A 0.7 cm(2) (0.25-5). The mean pre-operative AOFAS score was 58.7 +/- A 5.2 (49-75), and the mean post-operative AOFAS score was 85.5 +/- A 9.9 (56-100). At the last follow-up, 35 patients (42.6 %) had no symptoms and 19 patients (23.1 %) had pain after walking more than 2 h or after competitive sports activities. Radiological assessments of arthrosis revealed that no patient had grade 4 arthritis but that 27 patients (32.9 %) had a one-stage increase in their arthrosis level. Subgroup analyses of the lesion location demonstrated that lateral lesions had significantly better functional results (p = 0.02). Arthroscopic debridement and microfracture provide a good option for the treatment of osteochondral lesions of the talus over the long term in select patients. Functional outcomes do not correlate with defect size or patient age. Orthopaedic surgeons should adopt the microfracture technique, which is minimally invasive and effective for treating osteochondral lesions of the talus. IV.

    Ön çapraz bağ cerrahisinde RAMP lezyonu tamirinin spora dönüşe etkisi

    Bu çalışma izole ön çapraz bağ cerrahisi geçirmiş ve ön çapraz bağ cerrahisi ile eş zamanlı ramp lezyonu tamiri yapılmış katılımcıların spora dönüşlerinin kapsamlı bir şekilde incelenmesi amacıyla tasarlandı. Çalışmaya cerrahi sonrası 9. ayını dolduran yaşları 16-43 arasında, toplam 21 hasta dahil edildi. Hastalar geçirdikleri cerrahiye göre izole ön çapraz bağ grubu (n=11) ve ramp tamiri grubu (n=10) olmak üzere 2 gruba ayrıldı. Hastaların istirahat ve hareket esnasındaki ağrı şiddeti ''Vizüel Analog Skalası'', diz aktivite düzeyi ''Tegner Aktivite Düzeyi Ölçeği'', diz fonksiyonu ''Lysholm Diz Skorlama Ölçeği'', spora dönüşe psikolojik etmenlerin etkisi ''Ön Çapraz Bağ Yaralanması Sonrası Spora Dönüş Ölçeği (ACL-RSI)'', diz fonksiyonel kapasitesi ''Tek Bacak Hoplama Testleri'', dinamik denge değerlendirmesi ''Yıldız Uzanma Denge Testi'' ve statik denge ölçümü ''Flamingo Denge Testi'' ile değerlendirildi. Veriler SPSS version 22 yazılımı ile analiz edildi. Her iki grupta grup içi ve gruplar arası karşılaştırmada; istirahat ve aktivite esnasındaki ağrı şiddeti, diz aktivite düzeyi, diz fonksiyonel kapasitesi, sağlam ve opere bacak tek bacak hoplama testi, tek bacak hoplama testlerinin ortalama BSİ değerleri, sağlam ve opere bacak dinamik ve statik denge değerlerinde istatistiksel düzeyde anlamlı fark elde edilmedi (p>0,05). Gruplar arası ACL-RSI skorlarının karşılaştırılmasında ramp tamiri grubu lehine istatistiksel düzeyde anlamlı bir fark belirlendi (p<0,05). Sonuç olarak ramp lezyonu tamiri yapılmasının hastaların spora dönüşünde psikolojik durumlarına daha fazla katkı sağladığı bulundu.

    Ön çapraz bağ cerrahisinde, ek cerrahilerin kas gücü, propriosepsiyon ve denge üzerine etkisi

    Bu çalışmanın amacı, ön çapraz bağ (ÖÇB) yaralanmalarıyla birlikte sık görülen ve cerrahi tamir gerektiren menisküs yırtıklarının, ön çapraz bağ rekonstrüksiyon (ÖÇBR) cerrahisine etkilerini kas gücü, propriosepsiyon ve denge açısından değerlendirmektir. Çalışmada; aynı cerrahi işlemde hem ÖÇBR hem de menisküs veya ramp tamiri uygulanmış hastaların klinik sonuçlarıyla, sadece ÖÇBR uygulanmış hastaların klinik sonuçları karşılaştırılmıştır. Çalışmaya sadece ÖÇBR uygulanmış 15 hasta, menisküs veya ramp tamiriyle birlikte ÖÇBR uygulanmış 15 hasta ve 15 sağlıklı katılımcı dahil edilmiştir. Ayrıca ÖÇBR ve ek cerrahi uygulanmış hastalar; ramp tamiri uygulanmış 7 hasta ve medial menisküs tamiri uygulanmış 8 hasta olmak üzere altgruplara ayrılmıştır. Hastaların; izokinetik kas kuvveti, propriosepsiyon, denge, düşme riski, Lysholm Diz Skorlama ve Tegner Aktivite Düzeyi değerlendirmeleri preoperatif dönemde ve postoperatif 6. haftada yapılmıştır. Elde edilen bulgulara göre, postoperatif erken dönemde ÖÇBR+ek cerrahi uygulanmış grubun fleksör kas gücü, izole ÖÇBR ve kontrol grubuna göre anlamlı olarak düşük bulunmuştur (p<0,05). Ayrıca, ÖÇBR+ek cerrahi uygulanmış grupta istatistiksel olarak anlamlı fark tespit edilmiş olup (p<0,05), ÖÇBR+medial menisküs tamiri grubunun kas gücü ÖÇBR+ramp tamiri grubuna göre düşük bulunmuştur (p<0,05). ÖÇBR'ye ek cerrahilerin erken dönemde denge üzerindeki etkilerinin benzer olduğu, tüm hastaların iyileşme gösterdiği, ilaveten; ÖÇBR+ramp tamiri grubunun ÖÇBR+medial menisküs tamiri grubuna göre anlamlı olarak daha fazla iyileşme gösterdiği tespit edilmiştir (p<0,05). Lysholm ve Tegner skorlarına göre; tüm hastalar istatistiksel olarak anlamlı iyileşme göstermiştir (p<0,05). Çalışmamız sonucunda; sadece ÖÇBR uygulanmış hastaların, ÖÇBR+ek cerrahi uygulanmış hastalara göre ve ÖÇBR+ramp tamiri uygulanmış hastaların, ÖÇBR+medial menisküs tamiri uygulanmış hastalara göre postoperatif erken dönemde klinik sonuçlarının daha iyi olduğu saptanmıştır.

    Orthopedics and traumatology residency - Working conditions, training, and psychological stress

    PURPOSE OF THE STUDY : The specialty of orthopedics and traumatology that is completed in the 5 years period in our country is a challenging educational process and our purpose in this study is to demonstrate through a survey training conditions of the orthopedic assistants in our country and the effects of this process on assistants. MATERIAL AND METHODS : 524 (70.05%) of 748 assistants who receive specialization training in Turkey were reached. There were 20 multiple choice questions ( 1 mark each) and 3 questions (more than 1 mark each) in the survey consisting of twenty-three questions. Our study group was formed by doctors who have still worked as assistant in our country and have accepted to participate in the study. The doctors who finished assistantship with any reason and did assistantship for time less than 6 months and did not exactly fill the questionnaire form were excluded from the study. RESULTS:524 (71.97%) of 728 assistant who are in 40 (100%) of 40 provinces where assistant training given in Turkey were reached. 474 (90.45%) participants were satisfied to do orthopedic specialization. When considering working hours, it was observed that 337 (64.31%) participants had over 90 hours weekly including night shift and 521 (99.42%) participants had to work after night shift. The majority of participants (361 persons 68.89%) were receiving salaries between TL 4000-6000. When looking at the entire working group, the rate of participants who said that scientific training is weak or there is no scientific training was 427 (81.48%). CONCLUSIONS Our survey study is one of the first statistical study which investigating professional and social problems of orthopedic assistants. Some of important problems as training satisfaction, abuse by patients and/or manager, the average monthly income and psychological status assessment is emphasized. Orthopedics and Traumatology assistantship is a challenging process to cause physical and psychological problems with the hard working conditions in our Turkey.

    Procedural outcomes of double vs. single fluoroscopy for fixing intertrochanteric femur fractures

    Objective: This study compares reducing radiation and operation time between single and double C-arm fluoroscopy in the treatment of intertrochanteric femur fractures with intramedullary nails. Patients and methods: Forty four patients participated in the study. Patients were divided into two groups as single (23 patients) and double fluoroscope (21 patients). The time of preparation, the duration of the surgery, the total amount of blood loss, and the total duration of radiation exposure were compared, retrospectively. The collo-diaphyseal angle was compared with that of the contralateral hip on postoperative radiographs. Furthermore, the tip-apex distance and the position of the screws in the femoral head were recorded. Results: The mean preparation periods, collo-diaphyseal angles and blood loss did not differ between groups. In the double-fluoroscopy group, the duration of surgery was 15.9 min shorter (p <0.001), and the radiation time was 25.7 s shorter (p <0.001). Conclusion: The double fluoroscopy technique can significantly reduce surgical and radiation exposure times during surgery.

    Serious anterior cruciate ligament injuries at the individuals who have irregular sport activity on the artificial playing surfaces

    Objectives: The goals of this study that to present the individual’s features who have serious Anterior Cruciate Ligament (ACL) injuries of the knee that performing irregular sportive activities on the Artificial Playing Surface (APS). Methods: 87 patients who underwent arthroscopic ACL reconstruction Between April 2011 and March 2014, were evaluated. All patients questioned about the injury mechanisms, shoe types, frequency of sporting activity, time of warm-up exercises before sporting activity, types of APS, body mass index (BMI), return to work, frequency of return to previous sportive activity after surgery and rehabilitation period. Conclusion: Irregular sportive activity with unsufficient equipment on the artificial playing surface causes the serious ACL injuries that have to managed by surgical procedures resulting with loose of working time due to indispensable hospitalisation and rehabilitation period and significant economical losses.
