Öğe Evaluation of stroke risk factors and characteristics in the comorbidity of cancer(Inönü University Faculty of Medicine, 2021) Arıcı Düz, Özge; Helvacı Yılmaz, Nesrin; Aslan, Asile Seval; Polat, BurcuAim: Stroke and cancer can be followed together and in the presence of cancer, there may be changes in classical stroke characteristics. The aim of this study was to determine the effect of cancer on stroke risk factors, radiological and clinical features. Materials and Methods: Patients who were hospitalized with the diagnosis of stroke between 2014-2020 were included in the study retrospectively. By examining the stroke and cancer characteristics of the patients; The differences in demographic findings, stroke risk factors, and radiological features of stroke between patients with and without cancer were examined. The anti-cancer treatments used and the characteristics of cancer were evaluated in stroke patients. Results: 281 stroke patients were evaluated and cancer was detected in 52 patients. There was no significant difference in terms of demographic characteristics of the patients. No significant difference was observed in stroke patients with cancer in terms of risk factors except smoking and alcohol use. Multiple ischemic lesions were more evident on magnetic resonance imaging (MRI) in stroke patients with cancer. The most common cancer was lung cancer. Conclusion: Stroke and cancer are increasingly common comorbidities. As the time between stroke and cancer development decreases, while the risk factors for cancer are more prominent, classical risk factors for stroke are observed less frequently.Öğe Parkinson's disease and the COVID-19 pandemic: Do quarantine affect the motor and non-motor symptoms of patients with and without deep brain stimulation?(Galenos Publishing, 2022) Helvacı Yılmaz, Nesrin; Bolluk Kılıç, Başak; Zırh, Tahsin Ali; Aslan, Asile Seval; Hanoğlu, LütfüAim: Patients with chronic diseases were forced into lockdown due to the coronavirus disease-2019 pandemic. Extended quarantine could lead to physical inactivity and psychiatric problems. We investigated the effects of quarantine and social isolation during the pandemic on the motor and non-motor symptoms (NMS) of Parkinson's disease (PD) patients with and without deep brain stimulation (DBS). Methods: This study included 168 patients with PD who were in quarantine for 2 months (between April 1, 2020 and May 31, 2020). Eighty-three patients had undergone bilateral subthalamic DBS surgery. A questionnaire with three parts was administered via phone: 1) motor symptoms, 2) NMS, and 3) the reasons for impairment. Results: Of the patients, 54.7% reported impairment in at least one motor symptom and 58.9% reported impairment in at least one NMS. Increased tremors, difficulty with turning in bed, and insomnia were significantly more pronounced in patients without DBS. Patients with DBS complained less of being bored due to staying at home and had less deterioration due to lack of exercise and slower disease progression. Conclusion: Half of the symptoms of patients with PD worsened during quarantine, but patients with DBS tolerated the lockdown better. Telemedicine and online physiotherapy programs should be recommended to prevent rapid disease progression.Öğe Parkinson’s disease - Its pathophysiology to etiology(Nova Science Publishers, Inc., 2022) Kılıç, Şeyma; Aslan, Asile Seval; Helvacı Yılmaz, NesrinThe symptoms of Parkinson’s disease are well-known, but there are still little known about pathophysiology to etiology. Alpha-synuclein and Lewy body accumulation in neurons of substansia nigra, mitochondrial dysfunction and oxidative stress, neuro-inflammation caused by microglia, glutamate excitotoxicity and gut microbiota are the possible pathogenetic factors. The motor symptoms, dopamine depletion due to alpha-synuclein neurotoxicity leads to bradykinesia and rigidity while dysfunction of the cerebellumthalamo- cortical circuit is the underlying mechanism for tremor. Age, pesticides, head trauma, malignant melanoma are the major environmental risk factors while smoking, caffeine, Mediterranean diet and exercise are the main protective factors. By avoiding the risk factors and providing disease-modifying therapies, Parkinson’s Disease may become a curable disease.
