Kalp cerrahisinde kullanılan santrifugal ve roller başlıkların preoperatif ve postoperatif etkilerinin karşılaştırılması

Abstract

Günümüzde açık kalp cerrahisinde kardiyopulmoner bypass (KPB) esnasında yaygın olarak santrifugal ve roller olmak üzere iki farklı pompa tipi kullanılmaktadır. Santrifugal pompalar, santrifuj yani merkezkaç kuvvet prensibi ile çalışır. Bu pompalar, hızlı dönen konsantrik koni şeklinde bir çark içerir. Böylece kan hızla çevrilerek pompanın çıkışına doğru iletilir. Roller pompalar ise, pozitif yer değiştirme usulü çalışan silindir pompalardır. Kanı çekme ve itme prensibi ile çıkışa doğru taşırlar. Her iki pompanın da birbirlerine üstünde konusunda görüş ayrılıkları vardır. Bu çalışmada amacımız, kalp cerrahisinde kullanılan santrifugal ve roller pompa başlıkların preoperatif ve postoperatif etkilerinin karşılaştırılmasıdır. Çalışmaya roller pompa kullanılan (grup 1) n=15, santrifugal pompa kullanılan (grup 2) n=15 toplam 30 hasta dahil edildi. Her iki grup için hemogram ve kan gazı testlerinin değerlendirilmesi için preoperatif, postoperatif 2.saat, postoperatif 24.saat ve postoperatif 72.saat kan örnekleri alındı. Alınan bu örneklerden hemoglobin, hemotokrit, lökosit, eritrosit ve trombosit değerleri kayıt edildi. Kan gazı örneklerinden ise laktat değerleri kayıt altına alındı. Her iki grup için kardiyopulmoner bypass sonrası ve postoperatif 24.saat idrar çıkışları ve drenaj miktarları, yoğun bakım ekstübasyon ve kalış süreleri, taburculuk gün sayıları ve kullanılan kan ürünleri miktarları karşılaştırıldı. Elde ettiğimiz veriler istatistiksel olarak değerlendirildi. İki grup arasında post-op 2.saat eritrosit ve kross klemp 5.dk laktat değerleri arasında anlamlı bir fark tespit edildi (p<0.05 ). Diğer parametrelerde istatiksel olarak anlamlı bir fark bulunamadı (p>0.05). Elde ettiğimiz veriler doğrultusunda KPB' da kullanılan roller ve santrifugal başlıkların değişkenler üzerinde farklı etkileri olduğu tespit edilmiştir.

Currently, two different types of pumps are used in the open heart surgeon during cardiopulmonary bypass (CPB). Centrifugal pumps work with centrifugal force principle. These pumps contain a rotating concentric cone-shaped impeller. Thus, the blood moves at high speed and moves to the outlet of the pump. Roller pumps are positive displacement cylinder pumps. They carry the blood to the outlet with the principle of pulling and pushing. They carry the blood to the outlet with the principle of pulling and pushing. Both pumps have differences of opinion on each other. The aim of this study was to compare the preoperative and postoperative effects of centrifugal and roller pump heads used in cardiac surgery. Group 1 (n=15) using roller pump, Group 2 (n=15) using centrifugal pump totally 30 patient were included in the study. Preoperative, postoperative 2nd hour, postoperative 24th hour and postoperative 72th hour blood samples were taken for the evaluation of hemogram and blood gas tests for both groups. Hemoglobin, hematocrit, leukocyte, erythrocyte and platelet values were recorded. Lactate values were recorded from blood gas samples. Both groups were compared with cardiopulmonary bypass and postoperative 24th hour urinary output and drainage quantities, intensive care extubation and residence time, number of days of discharge, and blood products used. The data we obtained were evaluated statistically. There was a significant difference between the two groups of post-op 2nd hour erythrocyte and cross-clamp 5.min lactate values (p<0.05 ). No statistically significant difference was found in other parameters (p > 0.05). Roles and centrifugal headings used in CPB have different effects on variables according to our data.
