Vücut kitle indeksi ile diz patolojileri ve pes planus ilişkisi

Çalışmanın amacı; farklı Vücut Kitle İndeksine (VKİ) sahip bireylerin ayak arklarının vücut ağırlığından nasıl etkilendiğiyle ilgili bilgi edinip, bir etkilenme varsa bu durumun diz eklemi üzerinde meydana getireceği değişiklikler ve diz patolojileri açısından risk faktörü olup olmayacağı hakkında daha detaylı bilgi edinmektir. Çalışmaya 45-55 yaşları arasında 75 kadın, 18 erkek toplam 93 olgu dahil edildi. Olguların VKİ, MLA, AAVA, QA, DVA ölçüldü. Pes planusun (medial longitudinal ark) değerlendirilmesinde Harris Tabakası, diğer açısal ölçümlerde ise Üniversal Gonyometre kullanıldı. Harris tabakası kullanılarak elde edilen ayak tabanı grafisi Staheli İndeksi(Sİ) kullanılarak yorumlandı. Bütün EHA ölçümleri standart tekniklerle KENDALL – MC CREARY kriterlerine uyularak yapıldı. İstatistiksel analiz için Minitab® 17.1.0 istatistik paket programı kullanıldı. İstatistik değerlendirmeler bağıntı (Korelasyon) analizi ile (Pearson ve Spearman) test edildi. Yapılan değerlendirmeler sonunda VKİ ile Sİ, Sİ ile AAVA, Sİ ile DVA ve QA, AAVA ile DVA ve QA arasındaki bağıntı anlamlıydı (p< 0,05). VKİ ile AAVA, VKİ ile QA ve DVA arasında anlamlı bağıntı olmadığı görüldü (p>0,05). VKİ ile Ağrı arasında ise zayıf anlamlı bağıntı olduğu belirlendi. Çalışma sonunda elde edilen bulgular; Pes planus VKİ den doğrudan etkilenirken, DVA, QA, AAVA Vücut Kitle İndeksinden ayak arkının değişmesine bağlı dolaylı olarak etkilendiğini gösterdi. VKİ arttığında diz ağrısı ortaya çıkmakta ancak bu durumda ayak arkı değişikliğine bağlı değişen alt ekstremite dizilimiyle ilgili olduğu düşünüldü. VKİ, ayak arkını değiştirmesine sekonder ortaya çıkacak hatalı diz eklemi dizilimine sebebiyet vermesi bakımından diz patolojileri için önemli bir risk faktörü olmasına karşın diz eklemi patolojilerinden birincil etkenin Pes Planus olabileceği sonucuna varıldı. Objectives of the Study; To obtain relevant information about how foot arch is affected by body weight of individuals with different Body Mass Index (BMI) and to learn about whether a risk factor for knee pathologies and changes occurring on the knee joint .75 women and 18 man, in total 93 cases between the ages of 45-55 were included in the study. BMI of the patients, the medial longitudinal arch (MLA), the hindfoot valgus angles(HVA) , the Q angle (QA), Knee Valgus Angles(KVA) , inversion and eversion angles, dorsiflexion and plantar flexion angles were measured. To evaluate Pes Planus (medial longitudinal arch), Harris Mat was used, and for the other angular measurements Universal Goniometer was used. Staheli Index(SI) was used to interpret the graph of foot arch found using the Harris Mat. All Range of Motion measurements were made with standard techniques complied with KENDALL - MC CREARY criteria. Minitab® 17.1.0 statistic program was used for statistical analyses. At the end of the evaluation there are significant correlation between BMI with SI, SI with HVA, SI with KVA, SI with QA, HVA with KVA and HVA with QA (p <0,05). BMI with HVA and QA with BMI showed no significant correlation between KVA (p> 0,05). There is a weak significant correlation between BMI and Knee Pain (P =0,049). At the end of the study, evidence showed that; as the Foot arch (pes planus) was directly affected from BMI; KVA, QA, HVA was affected due to the changes of foot arch indirectly. Pain occurs when BMI increased but it is thought to be related with the altering of the allingnment of lower extremity due to the changes of foot arch. Although the BMI is an important risk factor for knee pathologies due to causing malalingnment of the knee joint seconderly revealing from the altering of the foot arch, it is thought that Pes Planus is the key factor for the knee pathologies.
