Retinacular ligament anatomisinin manyetik rezonans görüntüleme ile değerlendirilmesi

Bu çalışmanın amacı, temporomandibular eklemin (TME) aksesuar ligamentlerinden biri olan retinacular ligamentin anatomisinin manyetik rezonans görüntüleme (MRI) ile ortaya konmasıdır. Retinacular ligamentin varlığı 1995'ten bu yana bilinse de literatürde ligamente ait çok az bilgi bulunmaktadır. Bunun yanı sıra ligament daha önce radyolojik herhangi bir yöntem kullanılarak görüntülenmemiştir. Yaşları 18 ile 28 arasında değişen 6'sı erkek 14'ü kadın toplam 20 kişinin TME'leri bilateral olarak MRI ile görüntülenmiştir. Çekimler için Siemens Magnetom Viva syngo MR XA20 3T MRI cihazı kullanımış olup görüntüler 3D CRANII ve bFFE sekansları ile elde edilmiştir. MRI çekimleri ağız kapalı pozisyonda yapılmıştır. Başın fiksasyonu için sabitleme pedleri ve bantlar kullanılmıştır. Görüntülerin elde edilmesinin ardından ligamentin görülebilir hale gelmesi için ham görüntülere 3D Synapse Fujifilm Tokyo Japan yazılımı kullanılarak renk kodlama ve segmentasyon yapılmıştır. Program post processing için kullanılmıştır ve ligamente ait ölçümler işlenen görüntüler üzerinden elde edilmiştir. Ligamentin uzunluğu, aksiyal kesitte başlangıç anterior ve posterior genişliği, izlenebilen son noktadaki bitiş genişliği ölçülmüştür. Aynı zamanda alan ölçümü yapılmıştır. Retinacular ligament 40 TME'de de retrodiscal dokuların hemen altından başlamış, ramus mandibula'nın arka kısmında ilerleyerek parotideomasseterik fasyaya karışarak sonlanmıştır. Eklem kapsülü ve discus articularis ile bağlantısı gözlenmemiştir. Literatürde varlığı bilinen lateral fibröz ve posterolateral fibröz parçalar MRI görüntüsünde seçilememiştir. Parçaları görebilmek için çalışmayı destekleyici olması amacıyla yapılan diseksiyonda ise ligamentin iki parçası ortaya çıkarılmıştır. The aim of this study is to reveal the anatomy of the retinacular ligament, one of the accessory ligaments of the temporomandibular joint (TMJ), by magnetic resonance imaging (MRI). Although the existence of the retinacular ligament has been known since 1995, there is little information about the ligament in the literature. In addition, the ligament has not been visualized using any radiological method before. The temporomandibular joints of a total of 20 individuals, 6 male and 14 female, aged between 18 and 28 years, were imaged bilaterally with MRI. Siemens Magnetom Viva syngo MR XA20 3T MRI device was used for shooting and images were obtained with 3D CRANII and bFFE sequences. MRI scans were performed with the mouth closed. Fixation pads and tapes are used for head fixation. After the images were acquired, the raw images were color-coded and segmented using 3D Synapse Fujifilm Tokyo Japan software to make the ligament visible. The program was used for post processing and the measurements of the ligament were obtained from the processed images. The length of the ligament, the initial anterior and posterior width in the axial section, and the ending width at the end point were measured. At the same time, area measurement was made. In 40 TMJ, the retinacular ligament started just below the retrodiscal tissues and progressed in the posterior part of the ramus mandible and ended by mixing with the parotideomasseteric fascia. Its connection with the joint capsule and discus articularis was not observed. Lateral fibrous and posterolateral fibrous fragments known to exist in the literature could not be detected in the MRI image. In order to support the study in order to see the parts, two parts of the ligament were revealed in the dissection.
