Abdominoplastide oluşan suprapubik skarınpostoperatif migrasyonunun değerlendirilmesi

Abstract

Abdominoplasti en sık uygulanan estetik ameliyatlardan biridir. Abdominoplasti sonrası horizontal skar, zamanla vertikal olarak superiora doğru migrasyona uğramaktadır. Skarın nihai olarak ne kadar yukarı taşınacağının öngörülebilmesi, iyi bir preoperatif planlamaya bağlıdır. İstanbul Medipol Üniversitesi Tıp Fakültesi Plastik, Rekonstrüktif ve Estetik Cerrahi Kliniği'nde Eylül 2017-Eylül 2019 tarihleri arasında, yaş aralığı 20-60 (yıl) olan ve abdominoplasti operasyonu geçiren 31 hastayı içeren retrospektif bir çalışma yapıldı. Hastaların postoperatif 1 yıl boyuncaki takipleri değerlendirildi. Planlamada inferior insizyonun yeri karın cildi yukarı gerilerek orta hatta vulvar komissürden 7 cm yukarı mons pubis üzerinde işaretlendi. Burası merkez olacak şekilde planlama yapıldı. Cerrahi skar değerlendirilmesi 1. haftada ve 1. yıl kontrolünde gerçekleştirildi. Bu değerlendirmede, supin pozisyonda anterior labial komissür ile skar arasındaki mesafe orta hatta ölçüldü. İstatiksel analizler için SPSS® istatistik programı kullanıldı (version 23.0 for Mac OS; SPSS Inc., Chicago, IL, USA). Postop 1. haftada komissür ile skar arasındaki mesafe ortalama 7.90±0.39 (7.00-8.60) cm olarak ölçüldü. Hastaların post-op 1.yıl takiplerinde ise komissür-skar mesafesi ortalama 9.20±0.84 (7.20-11.00) cm olarak ölçüldü. Analizler sonucunda post-op 1. haftadaki her 1 cm değişimin, 1. yıldaki değişimi 1.47 cm olacak şekilde etkilediği görüldü (R2 : 0.48). Abdominoplasti ameliyatlarında bırakılan skarın ne kadar vertikal migrasyona uğrayacağının öngörülebilmesi, planlamanın uygun olması açısından dikkat edilmesi gereken bir konudur.

Abdominoplasty is one of the most common aesthetic surgeries. Horizontal scar after abdominoplasty migrates vertically towards the superior over time. Predicting how high the scar will ultimately migrate depends on good preoperative planning. A retrospective study was planned in Istanbul Medipol University Faculty of Medicine Plastic, Reconstructive and Aesthetic Surgery Clinic between September 2017 and September 2019, involving 31 patients aged 20-60 (years) who underwent classical abdominoplasty operation. The postoperative follow-ups of the patients for 1 year were evaluated. During planning, while determining the location of the inferior incision, the skin was kept streched and a mark was placed on mons pubis 7 cm above the vulvar commissure in the midline. Planning was made so that this mark is the center. Surgical scar evaluation was performed at the 1st week and at the 1st year control. In this evaluation, the distance between the anterior labial commissure and the scar in the supine position was measured at the midline. SPSS® statistics program was used for statistical analysis (version 23.0 for Mac OS; SPSS Inc., Chicago, IL, USA). The distance between commissure and scar was measured as 7.90 ± 0.39 (7.00-8.60) cm at the postoperative 1st week. The average commissure-scar distance was measured as 9.20 ± 0.84 (7.20-11.00) cm in the postoperative 1st year follow-up of the patients. As a result of the analysis, it was seen that every 1 cm change in the postop 1st week affected the change in the 1st year to be 1.47 cm (R2 : 0.48). The ability to predict how much vertical migration the scar left in abdominoplasty surgeries will undergo is an issue that should be considered in terms of proper planning.
