Araştırma, kolorektal cerrahi hastalarında hızlandırılmış iyileşme protokolü uygulamasının hemşirelik bakım konforuna etkisini belirlemek amacıyla klinik deneysel bir çalışma olarak planlandı. Araştırma 1 Ocak -31 Aralık 2019 tarihleri arasında Sağlık Bilimleri Üniversitesi Kocaeli Derince Eğitim ve Araştırma Hastanesi'nde uygulandı. Örnekleme elektif şartlarda kolorektal cerrahi geçiren ve en az 72 saat serviste kalan deney grubunda 30 ve kontrol grubunda 30 olmak üzere 60 hasta alındı. Veri toplamada Hasta Tanıtıcı Özellikler Formu, Hızlandırılmış İyileşme Protokolü Uygulama Kontrol Listesi, Standart Bakım Uygulama Kontrol Listesi, Perianestezi Konfor Ölçeği (PKÖ), Genel Konfor Ölçeği (GKÖ) ve Görsel Kıyaslama Ölçeği kullanıldı. Deney grubundaki hastalara hızlandırılmış iyileşme protokolü uygulandı, kontrol grubundaki hastalara standart bakım uygulandı. Veriler ortalama, standart sapma, frekans, yüzdelik, t testi, Pearson korelasyon analizi, Mann Whitney U testi ve Spearman korelasyon analizi ile test edildi. Çalışmada PKÖ puan ortalaması deney grubunda 4,8±0,521, kontrol grubunda 3,967±0,761 bulundu. Deney grubunda PKÖ puan ortalamasının kontrol grubundan anlamlı olarak daha yüksek olduğu belirlendi(U=246; p=0,001). Çalışmada GKÖ toplam puan ortalaması deney grubunda 3,067±0,254, kontrol grubunda 2,7±0,466 bulundu. Deney grubunda GKÖ toplam puan ortalamasının kontrol grubundan anlamlı olarak daha yüksek olduğu belirlendi (U=294; p=0,001). Deney grubunda GKÖ'nin tüm düzey ve boyut puan ortalamalarının kontrol grubundan anlamlı olarak yüksek olduğu belirlendi (p<0,05). Deney grubunda GKÖ puanları ile PKÖ puanları arasında pozitif yönde zayıf derecede anlamlı ilişki bulundu (r=373, p=0,042). Kontrol grubunda GKÖ puanları ile PKÖ puanları arasında pozitif yönde, zayıf düzeyde, anlamlı ilişki bulundu (r=445, p=0,014). HİP uygulanan grubun ağrı düzeyi kontrol grubuna göre daha düşük bulundu. Deney grubundaki hastaların ameliyat sonrası ilk gaz ve gaita çıkış zamanı,ilk oral alım, ilk mobilizasyon süresi, kataterlerin çıkış süresi ve taburculuk süresinin kontrol grubuna göre daha kısa olduğu tespit edildi. Sonuç olarak kolorektal cerrahi hastalarında HIP uygulaması genel ve perianestezi konfor düzeylerini arttırmaktadır. The study was planned as a clinical experimental study to determine the effect of enhanced recovery protocol on nursing care comfort in colorectal surgery patients. It was conducted between 1 January and 31 December 2019 in 60 patients, 30 in the experimental group and 30 in the control group, who underwent colorectal surgery in the Kocaeli Derince Training and Research Hospital under the University of Health Sciences and stayed in the service for at least 72 hours. The Patient Personal Information Form, Enhanced Recovery Protocol Implementation Checklist, Standard Care Implementation Checklist, Perianesthesia Comfort Scale (PCS), General Comfort Scale (GCS) and Visual Analog Scale were used in data collection. Enhanced recovery protocol was applied to the patients in the experimental group and the patients in the control group were given standard care. Data were tested using mean, standard deviation, frequency, percentage, t test, Pearson correlation analysis, Mann Whitney U test and Spearman correlation analysis. In the study, the mean score of the PCS was found to be 4,8±0,521 in the experimental group and 3,967±0,761 in the control group. It was determined that the mean score for the PCS in the experimental group was significantly higher than that in the control group (U=246; p=0,001). In the study, the mean score of the GCS was found to be 3,067±0,254 in the experimental group and 2,7±0,466 in the control group. It was determined that the mean score for the GCS in the experimental group was significantly higher than that in the control group (U=294; p=0,001). It was determined that the mean scores of all levels and dimensions of the GCS in the experimental group were significantly higher than the control group (p<0,05). A weakly significant positive correlation was found between GCS and PCS scores in the experimental group (r=373, p=0,042). In the control group, a positive, weak, significant relationship was found between the GCS scores and PCS scores (r=445, p=0,014). It was found that the level of pain in the experimental group were lower when compared to the control group. The first evacuation of flatus and stool and first oral intake, first mobilization duration, catheters removal period and length of hospital stay of the experimental group were found to be shorter than the control group. As a result, enhanced recovery protocol application in colorectal surgery increases both perianesthesia comfort and general comfort.
