dc.description.abstractAçık kalp cerrahisi uygulamalarında dokularda oluşabilecek oksijenasyon sorunları postoperatif dönemde morbidite ve mortaliteye neden olabilir. Doku oksijenlenmesi NIRS (Near-Infrared Spectroscopy) cihazıyla takip edilmektedir. Aynı zamanda kardiyovasküler cerrahinin standart takibi içinde olan hemoglobin değeri NIRS ile birlikte peroperatif anlamlı öngörüler sağlayabilir. KPB'de postoperatif dönemde oluşabilecek komplikasyonları önlemek için çeşitli yöntemler geliştirilmiştir. Bu yöntemlerin arasında KPB'de en sık kullanılan uygulamalardan birisi hipotermidir. Çalışmamızda; KPB ile açık kalp cerrahisi uygulanan hastalarda normotermik ve hipotermik bypass yöntemlerinin hemoglobin ve serebral NIRS üzerine etkilerini araştırmayı amaçladık. Açık kalp cerrahisi uygulanan 40 erişkin koroner arter hastası hipotermi uygulanan (grup1) n=20, normotermi uygulanan (grup2) n=20 olarak gruplandırılarak çalışmaya dahil edilmiştir. Çalışmaya dahil edilen hastalar için kan gazı örneği ve serebral NIRS takibi preoperatif, intraoperatif P1(KPB giriş), intraoperatif P2(kross klemp), intraoperatif P3(kross klemp sonrası), postoperatif '0'. ve '24'. saatler dahil olmak üzere incelenmiştir. Ancak, retrospektif bir çalışma olduğu için serebral NIRS 24. saat takibi incelenmemiştir. Elde ettiğimiz veriler istatistiksel olarak değerlendirildiğinde gruplara göre pompa öncesi, P2, P3 ve pompa sonrası NIRS değerleri açısından istatistiksel olarak anlamlı farklılık göstermemiştir (p>0,05). Gruplara göre P1 NIRS değerleri açısından istatistiksel olarak anlamlı farklılık tespit edilmiştir (t=2,086; p=0,044). Hipotermi uygulanan hastaların P1 NIRS değerleri, normotermi uygulananlara göre istatistiksel olarak anlamlı düzeyde daha yüksek tespit edilmiştir. Gruplara göre pompa öncesi, P1, P2, P3, pompa sonrası ve 24.saat hemoglobin değerleri açısından incelendiğinde istatistiksel olarak anlamlı farklılık bulunamamıştır(p>0,05). Elde ettiğimiz veriler doğrultusunda KPB' da uygulanan hipotermi ve normoterminin değişkenler üzerinde farklı etki gösterdiği tespit edilmiştir.en_US
dc.description.abstractOxygenation problems that may occur in tissues in open heart surgery applications may cause morbidity and mortality in the postoperative period. Tissue oxygenation is monitored with NIRS(Near-Infrared Spectroscopy) device. At the same time, hemoglobin value, which is in the standard follow-up of cardiovascular surgery, can provide significant preoperative predictions together with NIRS. Various methods have been developed to prevent complications that may occur in the postoperative period in KPB. Among these methods, one of the most frequently used applications in KPB is hypotermia. In our study; We aimed to investigate the effects of normothermic and hypothermic bypass methods on hemoglobin and cerebral NIRS in patients undergoing open heart surgery with KPB. Fourty adults coronary artery patients who underwent open heart surgery were grouped as n=20 who underwent hypotermia (group1), and n=20 with normotermia (group2). Blood gas sample were preoperative, intraoperative P1 (KPB entry), intraopertive P2 (cross-clamp), intraoperative P3 (post-cross-clamp), postoperative '0'. and '24'. Including watches. However, since it is a retrospective study, cerebral NIRS 24-hour follow-up has not been studied. When the data we optained were evaluated statistically, there was no statistically significant difference in terms of NIRS values before the pump, P2, P3 and after the pump (p>0,05). There was a statistically significant difference in terms of P1 NIRS values according to the groups (t= 2.086; p= 0.044). P1 NIRS values of patients who underwent hypotermia were found to be statistically significantly higher than those who recevied normothermia. According to the groups,no statistically significant difference was found when examined in terms of pre-pump, P1, P2, P3, post-pump and 24-hour hemoglobin values (p>0,05). In line with the data we optained, it was determined that hypotermia and normothermia applied in KPB had different effects on the variables.en_US
