Prematüre retinopati tedavisi almış çocuklarda görsel motor gelişim ve koordinasyonun değerlendirilmesi

Çalışmamızın amacı, farklı Prematüre Retinopati (PR) tedavisi almış çocukların göz yapısı, görme bozukluğu, motor gelişimi, görsel motor bütünlüğü, görsel algısı ve motor koordinasyonunu incelemekti. Çalışma, intravitreal Anti-VEGF (n:16) (Anti-Vascular Endothelial Growth Factor), DLF (n:23) (Diode Lazer Fotokoagulasyon) tedavisi almış ve Regrese PR'li (n:30) 2 ile 6 yaş arasındaki 3 grup çocuk üzerinde gerçekleştirildi. Değerlendirme yöntemleri olarak Beery - Buktenica Gelişimsel Görsel Motor Bütünlük Testi, Peabody Gelişimsel Motor Skala - 2, Titmus Stereo Fly Testi, Otorefraktometre, Snellen "E" eşeli, Optik Koherens Tomografi, Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği - Kısa Form ve Durumluk-Sürekli Anksiyete Ölçeği kullanıldı. Gruplara göre çocukların yaş ortalamaları bakımından istatistiksel olarak anlamlı bir fark yoktu (p>0,05). Regrese, Anti-VEGF ve Lazer gruplarındaki çocukların kronolojik yaşları ile görsel motor bütünlük ve motor koordinasyon yaş eşdeğerlikleri arasında istatistiksel olarak anlamlı fark vardı (p<0,05). Görme keskinliği sonuçları bakımından Regrese grubu ile diğer gruplar arasında anlamlı fark mevcuttu (p<0,05). Gruplar arasında ortalama optik sinir kalınlığı istatistiksel olarak anlamlı farklılık göstermedi (p>0,05). Kaba motor, lokomosyon ve görsel motor beceri sonuçları bakımından Anti-VEGF grubu ile diğer gruplar arasında istatistiksel olarak anlamlı fark vardı (p<0,05). Görsel motor bütünlük ve motor koordinasyon açısından Anti-VEGF ile Regrese grupları arasında istatistiksel olarak anlamlı fark bulundu (p<0,05). Anti-VEGF tedavisi almış çocuklarda daha belirgin olmakla birlikte PR'li erken doğan çocuklarda görme bozukluğu ile beraber görsel motor gelişim ve motor koordinasyon açısından gelişimsel gerilik görülme oranları yüksekti. PR tedavisinde kullanılan Anti-VEGF'in uzun vadeli sonuçları ve fizyoterapi programlarının etkinliği hakkında daha fazla araştırmaya ihtiyaç vardır. The aim of this study was to investigate eye structure, visual impairment, motor development, visual motor integration, visual perception and motor coordination in children who received different Retinopathy of Prematurity (ROP) treatments. The study was carried out on three groups of children, aged between 2-6 years who were treated with intravitreal Anti-VEGF (n:16) (Anti-Vascular Endothelial Growth Factor), DLF (n:23) (Diode Laser Photocoagulation) and who were diagnosed as Regrese ROP (n:30). The groups were compared by using Beery-Buktenica Developmental Visual Motor Integration Test, Peabody Developmental Motor Scale - 2, Titmus Stereo Fly Test, Autorefractometer, Snellen "E" chart, Optical Coherence Tomography, World Health Organization Quality of Life Scale - Bref Form and State-Trait Anxiety scale. There was no statistically significant difference between the groups in terms of mean age of the participants (p> 0.05). There was a statistically significant difference between the chronological age and visual motor integration, motor coordination' age equivalents of the participants in the Regrese, Anti-VEGF and Laser groups (p <0.05). There was a significant difference between Regrese group and other groups in terms of visual acuity results (p <0.05). The mean optic nerve thickness did not differ significantly between the groups (p> 0.05). There was a statistically significant difference between Anti-VEGF and other groups in terms of gross motor, locomotion and visual motor skill results (p <0.05). There was a statistically significant difference between Anti-VEGF and Regrese groups in terms of visual motor integrity and motor coordination (p <0.05). Although it was more prominent in children who received Anti-VEGF, preterm children with ROP had high developmental retardation incidence in terms of visual impairment, motor coordination and visual motor integration. Further research is needed on the long-term results of Anti-VEGF and the effectiveness of physiotherapy.
