Dijital hastane çalışmalarının yatan hasta işlemlerinde sağladığı kağıt tasarrufu ve hemşirelik bakım hizmetlerinin süresine etkisinin analizi

Abstract

Sağlıkta yeni hedef, teknolojiyi kullanarak hastanelerdeki idari ve klinik süreçleri dijitalleştirmek ve sağlık bakım kalitesini arttırmaktır. SBSGM tarafında başlatılan 'Hastanelerimizde Dijital Dönüşüm' projesi doğrultusunda, Türkiye'deki kamu hastanelerinde teknolojinin kullanım ve benimsenme oranını ölçmek amacıyla HIMSS EMRAM modelinin bir araç olarak kullanılmasına karar verilmiştir. Dijitalleşmenin ve benimsenmenin ölçüldüğü bu modelde 6. veya 7. Seviyeye kadarki tüm süreçlerini tamamlayan hastaneler belgelendirilmektedir. Çalışmamızın iki temel amacı vardır. Birincisi; dijital hastane çalışmalarının yatan hasta işlemlerinde hemşirelik bakım hizmetlerinin süresine etkisinin tespit edilmesidir. İkincisi; yatan hasta işlemlerinde sağladığı kâğıt sarfiyatının belirlenmesidir. Çalışmamızda Bahçelievler Devlet Hastanesi'nde 2017 HIMSS EMRAM kriterlerine göre Seviye 6 olarak sertifikalandırılan 4 serviste, dijital hastane çalışmalarının öncesi ve sonrası yerinde gözlemlenmiştir. Her iki amaca yönelik yapılan gözlemsel çalışmada veriler Excel yardımı ile kayıt altına alınmıştır. Yapılan analizler sonucunda dijitalleşmenin yıl içerisinde 1.153 yatan hasta için sağladığı toplam süre tasarrufu 117 bakım günü, toplam kâğıt sarfiyatı ise 41.289 sayfa olarak tespit edilmiştir. Ayrıca, toplam 25 yatağı bulunan 4 serviste yıllık 8,257 TL kâğıt ve 13.734 TL toner tasarrufu da sağlandığı tespit edilmiştir. Türkiye projeksiyonunda ise, %10,8 ile %13 oranında değişen süre tasarrufu dikkate alındığında Türkiye genelindeki yıllık tasarruf 398 ila 559 arasında bir hemşirelik zamanına denk gelmektedir. Sonuç olarak dijital hastane çalışmalarının bir çıktısı olan hemşirelik bakım formlarının elektronik olarak doldurulması ile bakım süresinde dokümantasyona ayrılan sürenin azalması beklenmektedir. Bunun yanı sıra formların dijitalleşmesi ile kağıt sarfiyatının önemli derecede önlenmesi ve kağıt ve toner masraflarının azalması beklenmektedir.

The new goal in healthcare is to digitize administrative and clinical processes in hospitals using technology and improve health care quality. Initiated by SBSGM 'Digital Transformation in Our Hospitals' project in line and use of technology in public hospitals in Turkey to measure its rate of adoption has been decided to use EMRAM HIMSS model. In this model, where digitalization and adoption are measured, hospitals that complete all their processes up to level 6 or 7 are documented. Our study has two main purposes. First; This study aims to determine the effect of digital hospital studies on the duration of nursing care services in inpatient procedures. Second; It is the determination of paper consumption in inpatient procedures. In our study, it was observed on-site before and after the digital hospital studies in 4 clinics which are certified as Level 6 according to the 2017 HIMSS EMRAM criteria at Bahçelievler State Hospital. In both observational studies, data were recorded with Excel. As a result of the analysis, the total time savings of 1,153 inpatients during the year were 465 care days and total paper consumption was 41,983 pages. Besides, it has been found that the annual savings of paper are 8,257 TL and toner saving is 13,734 TL have been achieved in 4 clinics which have a total of 25 beds. According to our study; Considering saving time from 13% to 10.8% which corresponds to an annual saving of Turkey in general nursing day time between 398 to 559. As a result, it is expected that the time allocated to documentation in the care period will be reduced by the electronic filing of the nursing care forms, which are the outputs of the digital hospital studies. In addition to this, it is expected that the digitalization of the forms will significantly reduce paper consumption and reduce paper and toner costs.
