dc.description.abstractKanser tüm Dünyada hızla yayılan, ölüm riski yüksek ve ülkemizde dolaşım sistemi hastalıklarından sonra en fazla ölüme sebep olan ikinci hastalıktır. Kanser tedavisinde cerrahi, radyoterapi ve/veya kemoterapi kullanılmaktadır. Cerrahi ve radyoterapi daha lokal bir alana uygulanabiliyorken kemoterapi sistemik etki sağlamaktadır. Kemoterapi tedavisinde kullanılan ilaçlar yüksek maliyetli ilaçlar olduğu için sağlığa ayrılan bütçede önemli bir yer tutmaktadır. Çalışmamız kemoterapide kullanılan ilaçlarda yarım doz imha maliyetlerini, maliyet tasarrufu sağlayarak kemoterapi ilaçlarına ayrılan gideri azaltmayı ve maksimum düzeyde ilaç, hasta ve çalışan güvenliğini hedeflemektedir. Çalışmamızda üç eğitim araştırma hastanesinin ilaçları ortak kemoterapi ilaç hazırlama cihazlarında hazırlanmaktadır. Hastane otomasyon programları üzerinden fatura edilen ilaç miktarları ve yarım doz ilaç imha miktarları raporlanmıştır. Excelde raporlar pivot tabloyla birleştirilerek analiz yapılmıştır. Hastanelerden ilaç hazırlama ünitesine görevlendirilen eczacılar, sorumlu eczacılar, hekimler ve hastane yönetimleriyle birlikte ortak çalışmalar ve takipler yapılmıştır. Hafta içerisinde patolojiye göre kemoterapi seanslarının rasyonel dağılımı, makul doz yuvarlaması yapılmış ve en önemli unsur da bir hastanede kalan yarım doz ilacın diğer hastane için kullanımı sağlanmış ve ilaç imhalarının önüne geçilerek gider azaltılmıştır. 18 aylık süreci kapsayan çalışmamızda imha maliyetlerinde yaklaşık 255.979,20 ₺ tutarında gider azalması gerçekleşmiştir. Çalışma yapılan merkezde ilaç hazırlama hizmetinin onkoloji alanında profesyonel, donanımlı eczacılar tarafından takibinin ve organizasyonun yapılmış olmasının, ilaç, hasta güvenliği ve düşük maliyet açısından olumlu sonuçlar verdiği görülmüştür. İstanbul, Ankara, İzmir gibi büyükşehirlerde büyük kompleks onkoloji merkezleri kurulmasının mevcut uygulamada olan ssitemlerden daha etkin olacağı düşünülmektedir.en_US
dc.description.abstractCancer incidence, rising rapidly in the World, carries a high mortality rate and causes the most deaths after cardiovascular diseases in our country. Surgery, radiotherapy and chemotherapy are different ways in which cancer is treated. While surgery and radiotherapy can be used in treatment of a local area, chemotherapy yields a systemic effect. Drugs used in chemotherapy treatment occupy an important place in the budget allocated to health, given their high costs. This study aims that reduce the half dose costs, reducing the chemotherapy drugs expense by the cost saving and drug, patient and employee safety in maximum level. In our work, drugs of three educational research hospitals are prepared in shared chemotherapy drug preparation devices. Drug amounts, invoiced through hospital automation programs, and the disposal cost of half dose drugs were reported. These reports were then consolidated into pivot tables in Excel and analyzed. Together with pharmacists assigned to drug preparation units by hospitals, authorized pharmacists, physicians and hospital managers studies and follow-up analyses were undertaken. Chemotherapy sessions on weekdays based on pathology reports were rationally distributed, reasonable dose rounding was conducted and most notably, half dose drugs left unused in hospitals were put to use in others, whereby drug disposals were prevented and costs were reduced. Throughout this work spanning across 18 months a cost reduction of approximately 255.979,20 TL was generated in the amount of disposal costs. This work observed that, at the center where this study was conducted, having well-equipped pharmacists specialized in the field of oncology serve in and watch drug preparation services yields positive results both in drug and patient safety as well as in reducing costs. It is suggested that establishing large-scale, complex oncology centers in major cities like Istanbul, Ankara, Izmir will prove to be more effective than existing systems.en_US
