dc.description.abstractBu çalışma somatosensöriyel sistemin vestibüler sistem üzerine etkisini incelemek amacıyla yapılmıştır. İstanbul Medipol Üniversitesi'nde okuyan, yaşları 18-25 arası değişen 27 kadın ve 27 erkek olmak üzere 54 birey üzerinde yapılmıştır. Bu kişilere Video Baş İttirme Testi (vHIT) ve Vestibüler Uyarılmış Kas Potansiyeli (VEMP) testi uygulanmıştır. Bu testler sırasıyla oturarak, ayakta ve sünger üzerinde ayakta yapılmıştır. Ayrıca bireylere çalışma esnasında kendilerini dengeli hissedip hissetmedikleri anket yoluyla sorulmuştur. Veriler SPSS 22.0 istatistik paket programı ile değerlendirilmiştir. Çalışmaya katılan bireylerin yaklaşık olarak yüzde 33'ü doldurdukları anket sonuçlarına göre süngerde kendilerini dengede hissetmediğini, yaklaşık yüzde 8'i de ayakta kendi dengeli hissetmediğini belirtmiştir. vHIT sonuçlarına göre oturarak-ayakta VOR kazançları arasında hem sağ hem de sol tarafta, ayakta- sünger üstünde ayakta vHIT karşılaştırması ise sağ posterior kanal VOR kazancında anlamlı bir farklılık gözlenmiştir (p<0.05). vHIT sonuçlarında lateral ve anterior kanal VOR kazançlarının karşılaştırmalarında anlamlı bir fark gözlenmemiştir. VEMP sonuçlarında ise sağ tarafta ayakta ve sünger üzerinde ayakta P1-N1 amplitüdü karşılaştırmasında istatistiksel olarak anlamlı bir fark gözlenmiştir (p<0.05). Bunun dışında kalan VEMP parametrelerinden latans, asimetri ve geri kalan amplitüd karşılaştırmalarında anlamlı bir fark bulunamamıştır. Ayağa kalkma ve sünger üzerinde ayakta durma sırasında değişen somatosensöriyel girdinin, inferior vestibüler sinir aracılığıyla posterior kanal VOR değişikliğine sebep olduğu düşünülmüştür. VEMP sonuçlarının yalnızca sağ tarafta anlamlı fark oluşturması ise, vestibüler-somatosensör multisensör duyuların baskın olduğu hemisfer tarafı (sağ) ile uyumlu çıkmıştır. Sonuç olarak bu iki sistemin birbirleriyle uyumlu çalışması ve birbirlerinin yerini doldurabilmesi; aynı zamanda bu özellikleri sayesinde mekânsal algı ve ağrı gibi genel duyular üzerinde ortak etkilenim yaratmaları, vestibüler rehabilitasyon konusunda somatosensöriyel sistemin önemini daha iyi vurgulamıştır.en_US
dc.description.abstractThe aim of this study was to investigate the effects of the somatosensory system on the vestibular system. The study was conducted on 54 individuals (27 females and 27 males) aged between 18-25 years studying at Istanbul Medipol University. The subjects were tested using Video Head Impuls Test (vHIT) as well as Vestibular Evoked Myogenic Potentials (VEMP) test. Tests were carried out sitting, standing on flat and standing on a foam respectively. In addition, individuals were asked whether they felt unstable or dizzy during these positions using a questionnaire. Collected data was analyzed with SPSS 22.0 statistical package program. Approximately 33% of the individuals who participated in the study stated that they did not feel balanced on the foam and 8% stated that they did not feel balanced on their feet. According to the posterior vHIT results, there was a significant difference between VOR gains obtained sitting and standing conditions in right as well as left side. However, when VOR gains in standing on flat and standing on foam results were compared to each other, statistical significance were found only in the right posterior canal (p<0.05). For in VEMP results, a statistically significant difference was seen in the comparison of P1-N1 amplitudes of the right side on standing-standing on foam. There was no significant difference between the latencies, asymmetries or amplitudes of the remaining VEMP parameters (p<0.05). It was thought that the responses to different conditions, such as standing om flat surface and standing on foam, may be initiated through inferior vestibular nerve, which have a common pathway with posterior semisircular canal and sacculus. The fact that the VEMP results were obtained significantly only on the right side was consistent with the fact that the vestibular-somatosensor multisensor senses are dominant on the right hemisphere. As a result, these two systems work in harmony with each other and can replace one another; at the same time, due to these characteristics, they create a common influence on general senses such as spatial perception and pain, emphasizing the importance of somatosensory system in vestibular rehabilitation once again.en_US
