dc.description.abstractBu araştırma, basınç yaralanmaları bakımında kullanılan topikal negatif basınç terapisinin iyileşmeye etkisini belirlemek üzere randomize kontrollü deneysel olarak yapıldı. Araştırmanın örneklemini İstanbul Başkent Üniversitesi Hastanesi yara bakım polikliniğine başvuran 30 hasta oluşturdu. Araştırma hakkında bilgi verilip onay alınan 30 hasta, deney (n=15), kontrol (n=15) olarak iki gruba ayrıldı. Deney grubuna TNBT, kontrol grubuna ıslak kuru pansuman uygulandı. Araştırmanın verileri, Hasta Tanıtım Formu, PUSH ve Yara Alan, Hacim, Doku Sınıflandırma Cihazı ile toplandı. Verilerin değerlendirilmesinde SPSS 22.0 programı kullanıldı. Veriler Kolmogorov-Smirnov testi, Anova ve Ancova testleri ile analiz edildi. Araştırmada grupların ilk terapi öncesi ve son terapi sonrası serum albümin ve hemoglobin düzeyleri arasında anlamlı bir farklılık bulunmadı (p>0,05). Yarada granülasyon dokusu oluşumunda iki yöntemin de etkili olduğu ancak deney grubunda granülasyon dokusu oluşumunun kontrol grubuna göre daha yüksek olduğu görüldü (p<0,05). Yara alanı küçülmesinde iki yöntemin de etkili olduğu ancak deney grubu alan ölçümünün kontrol grubu alan ölçümünden daha düşük olduğu görüldü (p<0,05). İki yöntemin de PUSH toplam puanı azalttığı ancak deney grubunda PUSH toplam puandaki düşmenin daha anlamlı olduğu bulundu (p<0,05). Araştırmada yara verileri hem PUSH ile hem de Yara alan, hacim, doku sınıflandırma cihazı ile toplandı. Cihaz ölçümleri ile PUSH ölçeği bulgularının korele olduğu belirlendi (p<0,05). Kontrol grubunun antibiyotik kullanım oranının deney grubu antibiyotik kullanım oranından yüksek olduğu görüldü (p<0.05). Sonuçta, TNBT'nin yara iyileşmesinde etkin olduğu, yeni bir teknoloji olan yara alan, hacim, doku sınıflandırma cihazının yara değerlendirilmesinde güvenle kullanılabileceği düşünülmekle birlikte, daha geniş örnekleme sahip çalışmaların yapılması ve hemşirenin pansumana ayırdığı zaman ve işgücünün değerlendirilmesi gerektiği önerilmektedir.en_US
dc.description.abstractThis study has been performed with randomized controlled trials to determine effects of topical negative pressure therapy on healing, which is used to treat pressure wounds. Sampling of the study consists of 30 patients who were admitted to İstanbul Başkent University Hospital wound care clinic. Those 30 patients were briefed about the study and once their approval was taken, they were separated into two as treatment (n=15) and control (n=15) groups. Treatment group was given the topical negative pressure therapy, and control group was applied wet-to-dry dressing. Study data were gathered with Patient Identification Forms, PUSH and Wound Size, Volume, Tissue Classification Device. To assess the data, SPSS 22.0 program was used. Data were analyzed with Kolmogorov-Smirnov, Anova and Ancova tests. There was no significant difference between pre-first therapy and post-last therapy serum albumin and hemoglobin levels of the treatment and control groups ( p>0.05). While both methods are effective in forming granulation tissue, it is observed that more granulation tissue formed at treatment group compared to the control group (p<0.05). While both methods are effective in reducing the sore size, it is observed that treatment group had a smaller wound area measurement compared to the control group (p<0.05). While both methods reduce the total PUSH points, it is observed that the drop in treatment group's PUSH points are more meaningful (p<0.05). Sore data for the study was gathered with both PUSH and Wound Size, Volume, Tissue Classification Device. It is found that the device measurements and PUSH scale findings are in correlation (p<0.05). It is observed that the rate of antibiotic usage of control group is higher compared to the treatment group (p<0.05). In conclusion, while it is considered that TNPT is effective on healing of the sores and the new technology of Wound Size, Volume, Tissue Classification Device can be used safely for the evaluation of the sores, it is suggested that research should be conducted with a wider sample range and, time and work force spared by the nurse to apply the dressings should be assessed.en_US
