Kök hücre nakli yapılan hastalardaöz bakım gücü ile umutsuzluk düzeyi arasındaki ilişki

Abstract

Bu çalışma, kemik iliği nakli ünitesinde yatan hastaların öz bakım gücü ile umutsuzluk düzeyi arasındaki ilişkinin belirlenmesi amacıyla kesitsel, tanımlayıcı ve ilişki arayıcı olarak gerçekleştirildi. Araştırmanın örneklemini İstanbul'da bir vakıf üniversitesinin ve İstanbul'da bir özel hastanenin kemik iliği nakil ünitesinde yatmakta olan 153 hasta oluşturdu. Çalışmanın verileri, hastalara yönelik "Sosyodemografik ve Hastalık Özelliklerine İlişkin Veri Toplama Formu", "Beck Umutsuzluk Ölçeği" ve "Öz Bakım Gücü Ölçeği" kullanılarak araştırmacı tarafından yüz yüze görüşme yöntemiyle toplandı.Araştırmada elde edilen veriler IBM SPSS (Statistical Package for Social Sciences) for Windows 22.0 programı kullanılarak analiz edildi. Verilerin değerlendirilmesinde tanımlayıcı istatistiksel yöntemleri olarak sayı, yüzde, ortalama, standart sapma, T-Testi, Tek yönlü varyans analizi (Anova), korelasyon ve regresyon analizi uygulandı. Araştırmaya katılan hastaların yaş ortalaması 44,160±15,079 olup, yarısından fazlası erkek (%55,6) ve yarısına yakını (%47,7) ilköğretim mezunu idi.Eğitim durumuna göre umutsuzluk ölçeğinin gelecekle ilgili duygular alt boyutunda, psikolojik desteğe ihtiyaç duyma durmuna göre umutsuzluk ölçeğinin motivasyon kaybı alt boyutunda ve nakil konusunda bilgilendirilme durumuna göre öz bakım gücü ölçeğinde gruplar arasında istatistiksel olarak anlamlı fark bulundu. Araştırmaya katılan hastaların öz bakım gücü puan ortalaması 104,490±25,007, umutsuzluk puan ortalması ise 4,990±4,664 olarak saptandı. Öz bakım ile umutsuzluk arasında orta düzeyde ve negatif ilişki olduğu bulundu. Öz bakım gücü ile umutsuzluk arasındaki neden sonuç ilişkisini belirlemek üzere yapılan analizde hastaların öz bakım gücü düzeyinin umutsuzluk düzeyini azalttığı belirlendi. Bu sonuçlar doğrultusunda, kök hücre nakli yapılan hastaların öz bakım gücünü azaltan ya da arttıran faktörlerin belirlenmesi, hastaların psikososyal açıdan etkin şekilde değerlendirilmesi, umutsuzluk düzeylerini azaltmak için kapsamlı hemşirelik girişimlerinin planlanması ve uygulanması önerilmektedir.

This study was developed as cross sectional, descriptor and searcher to determine the relationship between the self-care strenght and hopelessness level of the patients who are in the bone marrow unit. 153 patients in the bone marrow transplant unit of a foundation university anf a private hospital in istanbuli formed a sample of the research. The data of the study were collected through face to face interviews by the researcher using "The Data Collection Form of Sociodemographic and Disease Characteristics", "The Beck Hopelessness Scale and Self-Care Power Scale". The data obtained in the study were analyzed using the IBM SPSS (Statistical Package for Social Sciences) for Windows 22.0 program. Descriptive statistical methods were applied in the evaluation of the data such as number, percentage, average, standard deviation, T-Test, One way variance analysis (Anova), correlation and regression analysis . The age average of the patients participating in the research was 44,160 ±15,079 and more than half of them were male (%55,6) and nearly half of them were primary school graduates (%47,7). A statistically significant difference was found between the groups in the emotional sub-dimension of future of hopelessness scale according to their educational status, in the motivation loss sub-dimension of hopelessness scale according to the need for psychological support and in self-care power scale according to the situation of being informed about transfer. The average score of self-care power of the patients participating in the study was 104,490±25,007 and the average score of hopelessness was 4,990±4,664. A moderate and negative was found between self-care and hopelessness. In the analysis which was carried out to determine the cause effect relation between self-care and hopelessness, it was determined that the level of self-care power of the patients decreased the level of hopelessness. In line with these results, it is advised to determine the factors that decrease or increase self-care power of patients with stem cell transplantation, to evaluate the patients psychosocially in an effective way and to plan and implement comprehensive nursing initiatives to reduce their level of hopelessness.
