dc.description.abstractÇalışmanın amacı, Johnstone basınç splinti ve kas elektrik stimülasyonunun denge üzerine ne oranda etki ettiğini ve hangisinin daha etkili olduğunu araştırmaktır. Çalışmaya denge problemi olan akut/subakut 33 hemipleji olgusu dahil edilmiştir. Grup 1 (Johnstone) ve grup 2 (elektrik stimülasyon) olarak randomize kontrollü iki gruba ayrıldı. Her iki grup 6 hafta, haftada 5 gün, günde 1 saat rehabilitasyona alındı. Grup 1'e rehabilitasyona ek olarak her seansta 20dk Johnstone uzun bacak splinti ile öne-arkaya adım alma, ağırlık aktarma, yürüme eğitimi verildi. Grup 2'ye ise her seansta rehabilitasyona ek olarak 20dk elektrik stimülasyonu ile adım alma, ağırlık aktarma, yürüme eğitimi verildi. Yaş ve VKİ ortalamaları, 63,29±12,63 ve 24,57±4,26 idi. Akut/subakut hemipleji hastalarında; duyusal ve motor iyileşmenin belirlenmesi amacıyla Fugl Meyer testi, Brunnstrom, Motrisite İndeksi, Berg Denge Testi, on metre yürüme testi, zamanlı kalk yürü testi, günlük yaşamdaki bağımsızlık ölçümü için Fonksiyonel Bağımsızlık Ölçeği (FBÖ) kullanıldı. Ölçümler 1.seans öncesi, 3.hafta ve 6.hafta sonrası yapıldı. Tüm parametrelerde her iki grupta da gelişmeler kaydedildi. Başlangıç ve 3 hafta sonraki değerler karşılaştırıldığında sadece Motrisite kol indeksi kısmında Johnstone basınç splinti uygulanan grupta istatistiksel olarak anlamlı fark bulundu (p=0,043). 6 haftalık tedavi sonrasındaki değerlere bakıldığında ise; on metre yürüme testi, Berg Denge Testi, Brunstrom nörofizyolojik gelişim, FBÖ, zamanlı kalk yürü testi, Fugl Meyer Testi, Motrisite kol, bacak, yan taraf ve gövde testinde Johnstone basınç splinti uygulanan grupta istatistiksel olarak anlamlı fark bulundu (sırasıyla; P=0,001, p=0,017, p=0,001, p=0,010, p=0,006, p=0,001, p=0,006, p=0,003, p=0,004, p=0,004). Erken dönem inme tablosu olan hastalarda kullanılan Johnstone basınç splintinin, hastanın iyileşme tablosunu hızlandırdığı, motor gelişimi ve fonksiyonel gelişimi arttığını ve yürüme fonksiyonlarını iyileştirdiği düşünmekteyiz.en_US
dc.description.abstractAim of the study, how Johnstone pressure splint and muscle electrical stimulation affect balance and see which is more effective. 33 acute/ subacute hemiplegia patients who had balance problem were included in the study. Participants divided to two group randomly, group 1(johnstone) and group 2 (electric stimulation). Both group had rehabilitation program for 6 weeks, 5 days a week, 1 hour a day. In addition, group 1 had therapy with Johnstone pressure splint (take front step, side step and walking training) for 6 weeks, 5 days a week, 20 minutes a day and group 2 had same exercises with electric stimulation at the same time. The mean age and BMI were 63.29 ± 12.63 and 24.57 ± 4.26, respectively. In patients with acute / subacute hemiplegia; Brunstrom, Motricity Index, Berg Balance Test, ten meter walking test, timed up and go test, Functional Independence Measurement (FIM) for the measurement of independence in daily life were used to determine sensory and motor healing. Measurements were made before 1st session, 3rd week and 6th week. There was a statistically significant difference in the group with Johnstone pressure splint in the motricity arm index when baseline and after 3 weeks are compared (p = 0.043). When the values after 6 weeks of treatment are examined; we found a statistically significant difference in the group that applied Johnstone pressure splint in the ten meter walking test, Berg Balance Test, Brunstrom neurophysiological development, FIM, time up and go test, Fugl Meyer Test, Motricity arm, leg, side and trunk test (p = 0,001, p = 0,017, p = 0,001, p = 0,010, p = 0,006, p = 0,001, p = 0,006, p = 0,003, p = 0,004, p = 0,004; respectively). We think that the Johnstone pressure splint used in patients with early stroke table accelerates the healing table, improves motor development and functional development and improves walking function.en_US
