Küçük hücreli dışı akciğer kanseri nedeniyle ameliyat olan ve adjuvan kemoterapi alan hastaların yaşam kaliteleri ve sosyal destek düzeylerinin değerlendirilmesi

Abstract

Araştırma küçük hücreli dışı akciğer kanseri nedeniyle ameliyat olan ve adjuvan kemoterapi alan hastaların yaşam kaliteleri ve sosyal destek düzeylerini değerlendirmek amacıyla yapıldı. Araştırmanın evrenini, 28.12.2015 - 06.06.2016 tarihlerinde Yedikule Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim Araştırma Hastanesi Kemoterapi Ünitesine tedavi amacıyla başvuran hastalar, örneklemi ise bu hastalardan araştırmaya alınma kriterlerini karşılayan ve çalışmaya katılmayı kabul eden hastalar oluşturdu. Veriler hasta bilgi formu, EORTC QLQ-C30 Yaşam Kalitesi ölçeği ve Çok Boyutlu Algılanan Sosyal Destek ölçeği kullanılarak toplandı. Hastaların 21'i (%14,0) 70 yaş ve üstü, 135'i (%90,0) erkek, 123'ü (%82,0) evli, 76'sı (%50,7) İlköğretim mezunu, 112'sinin (%74,7) sosyal güvencesi SGK, 138'i (%92,0) çekirdek aile ve 39'u (%26,0) 2 çocuk sahibi olarak bulundu. Olguların genel iyilik hali puan ortalaması 45,222 ± 18,276 olup fonksiyonel ölçeklerden en yüksek puanı bilişsel fonksiyondan (85,222) en düşük puanın da rol fonksiyondan (44,556) aldı. Bireylerin sosyal destek ölçeği puan ortalamasının 41,367 ± 14,243 olduğu, alt boyutlarından en yüksek puanı aile (20,800 ± 5,705), en düşük puanı özel insan alt boyutu (9,067 ± 5,218) olduğu saptandı. Yaşam kalitesi tüm alt boyutlarının yaş, eğitim, hastalığı algılama durumu, hastalık ve ilaç tedavisine uyum durumu ve ilaçlarla ilgili düşünceleri ile; yaşam kalitesi alt boyutlarından semptomların cinsiyet ile; yaşam kalitesi alt boyutlarından genel iyilik halinin sosyal güvence ile; yaşam kalitesi alt boyutlarından fiziksel güçlüklerin çocuk sayısı ve kemoterapi tedavi kür sayısı ile istatistiksel açıdan anlamlı bir ilişki olduğu görüldü (p<0.05). Algılanan sosyal destek toplam ortalamalarının hastalığı algılama ve eğitim durumu ile, arkadaş alt boyutu puan ortalamasının cinsiyet ile, aile alt boyutu puan ortalamasının alkol kullanım durumu ile, özel insan alt boyutu puan ortalamasının medeni durum ve kemoterapi tedavi kür sayısı ile aralarındaki ilişkinin anlamlı olduğu saptandı (p<0.05). Sosyal destek ölçeği ile yaşam kalitesi ölçeği alt boyutlarından genel iyilik hali ve fonksiyonel skorlar ile pozitif, semptomlar ile negatif yönlü bir anlamlılık olduğu görüldü (p<0.05).

This study aimed to assess the quality of life and level of percieved social support of the patients who underwent operation due to non-small cell lung cancer and recieved adjuvant chemotherapy in a descriptive manner. The patients who were treated in Yedikule Chest Diseases and Chest Surgery Research and Ecucation Hospital Chemotherapy Unit between 28.12.2015 and 06.06.2016 constituted the study universe and the sample group included the patients who met the inclusion criteria of the study and accepted to participate. Data were collected by using patient information form, EORTC QOL-C30 quality of life scale, and multidimensional percieved social suppor scale. Twenty-one of the patients (14%) were aged over 70, 135 (90%) male, 123 (82%) married; 76 (50,7%) graduated primary school, 112 (74,4%) had social security, 138 (92%) lived only with their spouse alone and 39 (26%) with spouse and 2 children. The mean overall wellness value of the cases was 45,222±18,276, and they took the highest point from the cognitive functions (85,222) and lowest point from the role functions (44,556). The mean value of multidimensional percieved social support scale of the cases was 41,367±14,243, and from the subdimensions family had the highest point (20,800±5,705) and special human subdimension had the lowest point(9,067±5,218). The relation between the mean scores of quality of life with age, educational status, perception of the disease, adaptation to disease and drug therapy, and opinions about the drug therapy; between symptoms with sex; general wellness with social security; physical difficulty with number of children and number of chemotherapy; percived social support with perception of the disease and educational status; percieved social support from friends with sex; percieved social support from family with alcohol use; percieved social support from special person with marital status and number chemotherapy were significant ( p<0.05 ). The relation between the subdimesions of CBASD with subdiemensions of EORTC QLQ-C30 namely general health status and functional scores was significantly positive, between the symptoms was significantly negative (p<0.05).
