dc.description.abstractÇalışmanın amacı, sırt bölgesini etkileyen ağrılarda ESWT'nin etkinliğini araştırmak ve ağrının eklem hareket açıklığına, kas gücüne, esnekliğe, depresyona ve yaşam kalitesine olan etkisini değerlendirmektir. alışmaya sırt ağrılı 40 kişi gönüllü olarak katıldı. Çalışmaya alınma kriterlerine uyan hastalar, geliş sıralarına göre randomizasyon yapılarak iki gruba ayrıldı. Tedavi öncesi değerlendirmesi tamamlandıktan sonra 20 kişilik Grup I'e 3 hafta boyunca haftada 2 gün 6 seans ekstrakorporeal şok dalga tedavisi (ESWT) ; 20 kişilik Grup II'ye ise 3 hafta boyunca haftada 5 gün 15 seans geleneksel tedavi yöntemi (Transkutanöz elektriksel sinir stimülasyonu+Hot pack+Ultrason) uygulandı. Her iki gruba ev egzersiz programı verildi. Değerlendirmeler tedavi öncesi ve sonrası yapıldı. Ağrı durumlarını değerlendirmek için algometre (ağrı eşiği ölçümü), Görsel Analog Skala (VAS), McGill Ağrı Anketi kullanıldı. Boyun eklem hareket açıklığı gonyometre ile; skapula kas çevresi ise manuel kas testiyle ölçüldü. Gövde ekstansörlerinin esnekliğini değerlendirmek için mezura kullanılarak kısalık testi yapıldı. Günlük yaşam aktivitelerini değerlendirmek için Nottingham Sağlık Profili (NSP) kullanılırken; depresyon durumunu değerlendirmek için Beck Depresyon Ölçeği (BDÖ) kullanıldı. Çalışmanın veri analizinde "Statistical Package for Social Sciences" (SPSS) Version 20.0 (SPSS inc., Chicago, IL, ABD) istatistik programı kullanıldı. Tüm analizlerde p<0,05 (iki yönlü) değerler istatistiksel olarak anlamlı kabul edildi. Tedavi sonrası VAS, BDÖ skorları, boyun eklem hareket açıklığı ölçümleri, skapular depresyon ve adduksiyon, skapular adduksiyon ve aşağı rotasyon, skapular elevasyon sağ (R) ve sol (L) kas gücü, McGill Ağrı Anketi Değerlendirici Kategori, McGill Ağrı Anketi Çok Yönlü Kategori ve McGill Ağrı Anketi şimdiki ağrı şiddeti bölümü, NSP Enerji, NSP Ağrı, NSP Duygusal Reaksiyonlar, NSP Sosyal İzolasyon ve NSP Fiziksel Hareketlilik skorlarında ve gövde ekstansörlerinin esneklik ölçümlerinde her iki grupta da tedavi sonrası istatistiksel olarak anlamlı bir düzelme vardı (p<0,05). Algometre ölçümü sonucunda sırtın sağ taraf ağrı eşiği değerlerinde tedavi sonrası her iki grupta da anlamlı değişim görülmezken; sol taraf ağrı eşiği değerlerindeki değişimler, Grup I'de istatistiksel olarak anlamsız, Grup II'de ise anlamlıydı. McGill Ağrı Anketi Duysal Kategori, McGill Ağrı Anketi seçilen kelime sayısı, NSP Uyku değerlerinde Grup I'de tedavi sonrası istatistiksel olarak anlamlı fark yokken (p>0.05) Grup II'de anlamlı fark vardı (p<0,05). McGill Ağrı Anketi Tesir Edici Kategori, sırt ekstansör, skapular adduksiyon R ve L, skapular abduksiyon ve yukarı rotasyon R ve L kas gücü skorlarında Grup I'de tedavi sonrası istatistiksel olarak anlamlı fark varken (p<0.05); Grup II'de anlamlı bir fark yoktu (p>0.05). Bu parametrelerin çoğunda Grup I'deki değişimler Grup II'e göre daha fazlaydı ve bu fark istatistiksel olarak anlamlıydı (p<0.05). Çalışmanın sonucunda, sırt ağrılı hastaların birçok ağrı, fonksiyon, yaşam kalitesi ve depresyon parametrelerinde daha anlamlı düzelme sağlayan ve yeni bir tedavi yöntemi olan ESWT'nin, kliniklerde sıklıkla kullanılan geleneksel tedavi yöntemlerine alternatif olabileceği görüşüne varıldı.en_US
dc.description.abstractThe purpose of this study is to research the activity of ESWT over the pain affecting the upper back and to assess effect of pain over joint's range of motion, muscle strength, flexibility, depression and life quality. 40 people with back pains joined the study voluntarily. Patients that fit to the criteria were accepted into the study separated into two groups through randomizing on the basis of their order of participation. After the completion of pre-treatment evaluation, 2 days a week of 6 ESWT sessions throughout 3 weeks were applied to Group I with 20 people; 3 days a week of 15 traditional treatment method sessions (Transkutan nerve stimulation + hotpack+ ultrasound) throughout 3 weeks were applied to Group II with 20 people. Each group was given an exercise program for home. The evaluations were conducted before and after the treatment. In order to assess their pain conditions, algometer (pain threshold measurement), Visual Analog Scale (VAS) and McGill Pain Questionnaire were used. Neck joint's range of motion was measured with goniometer; scapular muscle circumference was measured with manual blood test. In order to analyze the flexibility of the body extensors, shortness test was performed via tape measure. In order to evaluate the daily life activities Nottingham Health Profile (NHP) was used while Beck Depression Inventory was used to assess the depression condition. In the data analysis of the study, a statistical program called "Statistical Package for Social Sciences" (SPSS) Version 20.0 (SPSS inc., Chicago, IL, USA) was used. Throughout the whole analyses values with p<0,05 (two-way) were accepted as statistically significant. There was a statistically significant improvement in both groups after the treatment (p<0,005) in post-treatment VAS, Beck Depression Inventory scores, neck joint's range of motion measurements, scapular depression and adduction, scapular adduction and lower rotation, scapular elevation R and L muscle strength, McGill Pain Questionnaire Rating Category, McGill Pain Questionnaire Multifaceted Category and McGill Pain Questionnaire current pain level region, NSP Energy, NSP Pain, NSP Emotional Reactions, NSP Social Isolation and NSP Physical Mobility scores and in the flexibility measurements of the body extensor. While there was not a significant change in both of the groups in the values of the right side of the upper back pain levels after the treatment; the changes in the values of the left side of the upper back pain levels were statistically insignificant for Group I and statistically significant for Group II. McGill Pain Questionnaire Sensorial Category, McGill Pain Questionnaire number of chosen words, NSP Sleep values had no statistically significant difference in Group I after the treatment (p>0,05), there was a statistically significant difference in Group II (p<0.05). In McGill Pain Questionnaire Effective Category, upper back extensor, scapular adduction R and L, scapular abduction and upper rotation R and L muscle strength scores, there was a statistically significant difference in Group I after the treatment (p<0.05); while there was not a statistically significant difference in Group II (p>0.05). In most of these parameters, the changes in Group I was higher than the changes in Group II and this difference was statistically significant (p<0.05). At the end of the study, it was decided that for the ESWT, which provides a more significant improvement in many pain, function, life quality and depression parameters and which is a fairly new method of treatment, could be an alternative for the traditional treatment methods (TENS, hotpack, ultrasound) that are used very often in the clinics.en_US
