Kardiyopulmoner bypass'ta retrograd otolog primenın kan ve kan ürünleri kullanımı üzerindeki etkisi

Abstract

Açık kalp cerrahisinde kardiyopulmoner bypass başlamadan önce kullanılacak olan sistemin tamamen havasız bir şekilde olması için prime edilmesi gerekmektedir. Bu kullanılan prime miktarı kardiyopulmoner bypass esnasında hemodilüsyona sebep olmaktadır. Hemodilüsyonu ve hastada fazla volüm yükünü azaltmak için kullanılan yöntemlerden biri de retrograd otolog prime'dır (ROP). Bu çalışma ROP tekniğinin uygulandığı hasta gruplarında hemoglobin değerleri üzerindeki etkisini ve buna bağlı olarak kan ve kan ürünlerinin kullanım miktarının değişip değişmediğini araştırmaktır. Bu sebeple Başakşehir Çam ve Sakura Şehir Hastanesi' nde koroner arter bypass ameliyatı olan 60 kişi çalışmaya dahil edilmiştir. Bu hasta gruplarının EF %35 üstünde, preop hemogramda Hct değeri %25-45 arası, ilk kez opere olan ve herhangi bir kanamaya neden olan hemotolojik hastalığı olmayan hastalar randomize olarak seçilmiştir. 30 kişi standart prime (kontrol grubu) ve 30 kişi retrograd otolog prime ( ROP) tekniği (çalışma grubu) olacak şekilde uygulandı. Çalışma (ROP) grubunda ybü Hct değerleri kontrol grubuna göre anlamlı şekilde yüksek bulundu. Çalışma grubunda prime'da kullanılan kristalloid miktarı kontrol grubuna göre daha az olduğu bulunmuştur. Kontrol grubunda çalışma grubuna göre pompa dengesinin anlamlı bir şekilde yüksek olduğu saptanmıştır. İki grup arasında kan ve kan ürünü kullanımı (ES ,TDP ve Trombosit) kullanımı miktarında farklılık görülmemiştir.

Before starting cardiopulmonary bypass in open heart surgery, the system to be used must be primed to be completely airless. This amount of primen used causes hemodilution during cardiopulmonary bypass. Retrograde autologous priming (ROP) is one of the methods used to reduce hemodilution and excess volume load in the patient. This study aims to investigate the effect of the ROP technique on hemoglobin values in patient groups and, accordingly, whether the amount of blood and blood products used changes. For this reason, 60 people who had coronary artery bypass surgery in Başakşehir Çam and Sakura City Hospital were included in the study. Patients with EF above 35%, Hct value between 25-45% in preoperative hemogram, who were operated for the first time and who did not have any hematological disease causing bleeding were randomly selected from these patient groups. 30 people were applied to standard prime (control group) and 30 people to retrograde autologous prime (ROP) technique (study group). In the study (ROP) group, YBU Hct values were found to be significantly higher than in the control group. The amount of crystalloid used in the prime in the study group was found to be less than in the control group. Pump balance was found to be significantly higher in the control group compared to the study group. There was no difference in the amount of blood and blood product use (ES, TDP and Trombocyte) use between the two groups.
