Gelişen teknolojilerin etkisiyle radyoterapide doz hesaplama doğruluğu önemli hale gelmiştir. Küçük hacimler için hesap yapmak ve doğru ölçüm zorlaşmıştır bu sebeple farklı algoritmalar geliştirilmiştir. Radyoterapinin temel amacı belirlenmiş tümör alanını tam doğrulukta ışınlayıp sağlıklı dokuları gereksiz ışınlardan korumaktır. Solunuma bağlı lezyon hareketini, tedavi esnasında doğru takip edebilmek önemlidir. Yapılan bu çalışmada; farklı hesaplama algoritmaları kullanılarak akciğer SBRT planları hesaplatılmış, farklı tedavi cihazları ve hareketli akciğer fantomu kullanılarak dozimetrik doğruluğun tayini hedeflenmiştir. Fantomun serbest solunum ve 4DCT taramaları yapılarak görüntüler oluşturuldu. Görüntüler Multiplan ve Eclipse Tedavi Planlama Sistemine (TPS) aktarılıp, konturlamaları yapıldı. Eclipse Tedavi Planlama Sisteminde Analitik Anizotropik Algoritma (AAA) ve Acuros XB, Multiplan Tedavi Planlama Sisteminde Monte Carlo ve Ray-Tracing algoritmaları ile tedavi planları hesaplatıldı. Tedavi tekniği olarak Eclipse Tedavi Planlama Sistemi için Volümetrik Ayarlı Ark Radyoterapi (VMAT), Multiplan Tedavi Planlama Sistemi için non- coplanar uygulandı. Oluşturulmuş planlar Accuray Cyberknife M6 ve Varian Truebeam STx 2.0 cihazlarında ışınlandı. EBT-3 Gafkromik film sonuçları ile TPS' de hesaplanan dozlar karşılaştırıldı. Doz hesaplama algoritmaları doğruluklarına göre değerlendirildi ve karşılaştırıldı. Ölçülen ve hesaplanan doz dağılımlarının birbiri ile uyumlu olduğu bulundu. Akciğer tümörü tedavisinde algoritmalar arasında Monte Carlo ve Acuros XB algoritmalarının tercih edilebilir olduğu gözlemlenmiştir. The accuracy of dose calculation algorithms in radiotherapy has become more and more important with the advances in engineering software. It has become difficult to calculate and accurately measure small volumes. Therefore, to overcome this diffuculty different algorithms have been established. The main purpose of radiotherapy is to irradiate the determined tumor area with full accuracy and to protect healthy tissues from unnecessary radiation. It is important to follow the respiratory lesion movement accurately during treatment. In this study, lung SBRT plans were calculated by using different calculation algorithms, and it was aimed to determine the dosimetric accuracy by using different treatment devices and a mobile lung phantom. Images were created by performing free breathing and 4DCT scans of the phantom. Images were transferred to Multiplan and Eclipse Treatment Planning System (TPS) and contoured. Treatment plans were calculated using the Analytical Anisotropic Algorithm (AAA) in the Eclipse Treatment Planning System and Ray-Tracing and Monte Carlo algorithms in the Multiplan Treatment Planning System. As a treatment technique, Volumetric Modulated Arc Treatment (VMAT) was used for the Eclipse Treatment Planning System and non-coplanar was applied for the Multiplan Treatment Planning System. Accuray Cyberknife M6 and Varian Truebeam STx 2.0 devices were used to beam the plans that were created. The EBT-3 Gafchromic film results and doses calculated in TPS were compared. Dose calculation algorithms were evaluated and compared according to their accuracy. It was found that the measured and calculated dose distributions were compatible with each other. It has been observed that Monte Carlo and Acuros XB algorithms are preferable among the algorithms in the treatment of lung tumors.
