Kronik böbrek hastalığı (KBH) çocuklarda ve yetişkinlerde fonksiyonel kapasite, kas kuvveti, yaşam kalitesinde azalmaya, depresyon ve yorgunlukta artışa neden olur. Aerobik ve kuvvetlendirme egzersizlerinin ve egzersiz oyunlarının KBH semptomlarının tedavisinde etkili olduğu bildirilmektedir. Çalışmanın amacı pediatrik kronik böbrek hastalarında Nintendo Wii Fit tabanlı video oyunları ve ev tabanlı eğlenceli video egzersizlerinin fonksiyonel kapasite, kas kuvveti, fiziksel aktivite, depresyon, yorgunluk, solunum fonksiyonları, denge ve inflamasyon üzerindeki etkilerinin karşılaştırılmasıdır. Çalışmaya KBH olan 23 çocuk katıldı. Gruplar Grup I (Nintendo Wii Fit), Grup II (ev tabanlı eğlenceli video egzersizleri) olarak randomize edildi. Her iki gruba 6 hafta boyunca haftada iki kez egzersiz programı uygulandı. Tedavi öncesi ve sonrası hastaların fonksiyonel kapasiteleri 6DYT ile, kas kuvveti manuel dinamometre ile, fiziksel aktivite seviyeleri 7 günlük ortalama adım sayıları ile, yaşam kalitesi Çocuklar İçin Yaşam Kalitesi Ölçeği (PedsQoL çocuk ve ebeveyn formları), yorgunluk seviyesi PedsQoL formu fiziksel alan alt başlığı ile, depresyon Çocuk Depresyon Ölçeği, denge Pediatrik Denge Ölçeği ve Nintendo Wii Fit Balance Board ile, solunum fonksiyonu Spirometre ile değerlendirildi. Çalışmayı on altı hasta tamamladı. Tedaviden sonra 6DYT, kas kuvveti, ortalama günlük adım sayısı, PedsQoL çocuk formu fiziksel sağlık toplam skoru ve Görsel Yorgunluk Ölçeği'nde her iki grupta da anlamlı farklılıklar gözlendi. Sol diz fleksör kas kuvveti ve ortalama adım sayısı Grup I'de daha üstün bulundu. Video oyunları ve ev tabanlı eğlenceli video egzersizler, KBH olan pediatrik hastalarda fonksiyonel kapasite, kas kuvveti, yorgunluk, fiziksel aktivite, depresyon, yaşam kalitesi, solunum fonksiyonları üzerinde olumlu etkiler sağlar. Bu egzersiz yöntemlerinin hastalığın erken dönemlerinden itibaren tedaviye dahil edilmesi gerektiğini düşünüyoruz. Chronic kidney disease (CKD) causes decrease in functional capacity, muscle strength, quality of life, increase in depression and fatigue in children and adults. It is reported that aerobic and strengthening exercises and exercise games are effective in the treatment of CKD symptoms. This study aimed to compare the effects of Nintendo Wii-based video games and home-based fun video exercises on functional capacity, muscle strength, physical activity, depression, fatigue, respiratory functions, balance in pediatric patients with CKD.23 children with CKD participated in the study. Groups were randomized into Group I (Nintendo Wii Fit) and Group II (home-based fun video exercises). An exercise program was applied to both groups twice a week for 6 weeks. Functional capacities of patients were assessed before and after treatment with six minute walk test (6MWT), muscle strength with manual dynamometer, physical activity levels with average 7-day step counts, quality of life with Children's Quality of Life Scale (PedsQoL child and parent forms), fatigue level with PedsQoL form physical field subheading. Depression was assessed with the Child Depression Scale, balance with the Pediatric Balance Scale and Nintendo Wii Fit sensors, and respiratory function with the Spirometer. Sixteen patients completed the study. After treatment, significant differences were observed in both groups in 6MWT, muscle strength, mean daily step count, PedsQoL child form physical health total score, and Visual Fatigue Scale. Left knee flexor muscle strength and mean step count were superior in Group I. Video games and home-based fun video exercises provide positive effects on functional capacity, muscle strength, fatigue, physical activity, depression, quality of life, respiratory functions in pediatric patients with CKD. We think that these exercise methods should be included in the treatment from the early stages of the disease.
