Bu çalışmanın amacı karpal tünel sendromlu (KTS) bireylerde konvansiyonel fizyoterapiye ek olarak uygulanan dereceli motor imgeleme eğitiminin (DMİE) ağrı ve fonksiyonellik üzerine etkisinin incelenmesidir. Çalışmaya dahil edilen 18-65 yaş arası 30 KTS'li kişi dereceli motor imgeleme grubu (DMİG; n=15) ve konvansiyonel fizyoterapi grubu (KG; n=15) olarak rastgele 2 gruba ayrıldı. Çalışmadaki tüm katılımcılara 4 hafta boyunca haftada 5 kez terapi uygulandı. KG'ye Ultrason, Transkutaneöz Elektriksel Sinir Stimülasyonu, Hotpack modaliteleri ve egzersiz programını içeren konvansiyonel fizyoterapi uygulandı. DMİG'ye konvansiyonel fizyoterapiye ek olarak dereceli motor imgeleme eğitimi uygulandı. T üm katılımcıların çalışma öncesi ve sonrası Ağrı Tespit Anketi (ATA) ağrı şiddeti, Recognise uygulaması el lateralizasyonu, Boston Karpal Tünel Sendromu Anketi (BKTSA) ile semptom şiddeti ve fonksiyonel durumu, Jamar el dinamometresi ile kavrama kuvveti; gonyometrik ölçüm ile eklem hareket açıklığı (EHA); Tampa Kinezyofobi Ölçeği (TKÖ) ile hareket korkusu ve Ağrı Felaketlendirme Ölçeği (AFÖ) ile ağrı inançları değerlendirildi. Çalışma sonrasında yapılan değerlendirmeler sonucu EHA, BKTSA, ATA, AFÖ, TKÖ değerlerinde her iki grupta da iyileşme görüldü (p<0,05). Tedavi sonrası el lateralizasyonunda KG'de iyileşme görülmezken (p>0,05), DMİG'de etkilenmiş taraf elde iyileşme bulundu (p<0,05). El kavrama kuvvetinde etkilenmiş taraf elde her iki grupta da iyileşme oldu (p<0,05). İki grup arasında tedavi öncesi ve tedavi sonrası değerlendirmelerde ise el lateralizasyonu, EHA, el kavrama kuvveti, BKTSA alt başlıkları, ATA, AFÖ, TKÖ değerlendirmesi açısından istatistiksel olarak anlamlı farklılıklar bulunmadı (p>0.05). Bu çalışma ile KTS'li bireylerde konvansiyonel fizyoterapiye ek olarak uygulanan DMİE'nin sonucu ağrı ve fonksiyonellik üzerindeki etkileri arasında fark bulunmamıştır. The aim of this study is to examine the effect of graded motor imagery training (GMİT) applied in addition to conventional physiotherapy on pain and functionality in individuals with carpal tunnel syndrome (CTS). Thirty people with CTS aged 18-65 years included in the study were randomly divided into 2 groups as graded motor imagery group (GMIG; n=15) and conventional physiotherapy group (CG; n=15). All participants in the study received therapy 5 times a week for 4 weeks. Conventional physiotherapy including Ultrasound, Transcutaneous Electrical Nerve Stimulation, Hotpack modalities and exercise program was applied to CG. In addition to conventional physiotherapy, graded motor imagery training was applied to GMIG. Before and after the study, pain severity with the Pain Detection Questionnaire (PDQ), hand lateralization with Recognise application, symptom severity and functional status with Boston Carpal Tunnel Syndrome Questionnaire (BCTSQ), grip strength with Jamar hand dynamometer; joint range of motion (ROM) by goniometric measurement; Fear of movement was evaluated with the Tampa Kinesiophobia Scale (TKS) and pain beliefs were evaluated with the Pain Catastrophizing Scale (PCS). As a result of the evaluations made after the study, improvement was observed in both groups in the ROM, BCTSQ, PDQ, PCS, TKS values (p<0.05). While there was no improvement in CG in hand lateralization after treatment (p>0.05), improvement was found in the treated hand in DMIG (p<0.05). Hand grip strength improved in both groups in the treated hand (p<0.05). There were no statistically significant differences between the two groups in terms of hand lateralization, ROM, hand grip strength, BKTSA sub- headings, PDQ, PCS, and TKS evaluations before and after treatment (p>0.05). In this study, there was no difference between the effects of GMIT applied in addition to conventional physiotherapy on pain and functionality in individuals with CTS.
