Çalışmamızın amacı, diz osteoartrit (OA) hastalarında kan akışını kısıtlayıcı egzersiz (BFRT) eğitiminin, konvansiyonel eğitime kıyasla oluşturduğu kuadriseps hipertrofisi, ağrı şiddeti, fonksiyonel seviye ve yaşam kalitesine etkisini araştırmaktı. Otuz sekiz katılımcının tamamladığı çalışmamızda, bireyler konvansiyonel egzersiz grubu (KEG) ve BFRT grubuna randomize edildi. KEG egzersizleri 1RM'nin %70-80'i dirençle, BFRT ise 1RM'nin %20-30'una denk gelen dirençle tamamladı. BFRT grubundaki katılımcılar uyluk proksimaline yerleştirilen pnömatik manşon ile arteryel oklüzyon basıncının %80'inde kan akışı kısıtlanarak egzersiz gerçekleştirdi. Tüm katılımcılara 8 hafta, 24 seanslık aynı programdan oluşan supervize egzersiz eğitimi verildi. Çalışma başlangıcı ve sonunda katılımcılar kuadriseps enine kesit alanı (CSA) ve kuadriseps hacmi (KV), ağrı şiddeti, fonksiyonellik, inflamatuar progresyon ve yaşam kalitesi açısından değerlendirildi. Ayrıca uyluk çevresi antropometrik ölçümleri, diz eklem hareket açıklığı (EHA), kreatin kinaz (CK) ölçümleri yapıldı. CSA, KV, ağrı şiddetleri, EHA, fonksiyonellik ve yaşam kalitesi değerleri tedavi sonunda başlangıca göre grup içi anlamlı farklılık gösterdi (p<0,05). CSA, KV, fleksiyon EHA, fonksiyonellik ölçümlerinden süreli otur kalk testi, yaşam kalitesi anketinin genel sağlık parametresi etki büyüklüklerine göre BFRT lehine, CK ise KEG lehine anlamlıydı (p<0,05). Çalışmamız ile BFRT uygulamasının konvansiyonel rehabilitasyonda kullanılan yüksek şiddetli direnç egzersizlerine benzer düzeyde etki yarattığı, daha fazla kuadriseps hipertrofisi, EHA, fonksiyonellik, genel sağlıkta iyileşme ve ağrıda azalma sağladığı, diz OA rehabilitasyonunda güvenli bir yöntem olarak kullanılabileceği sonucuna varıldı. The aim of the study was to investigate the quadriceps hypertrophy, pain intensity, functional level and quality of life provided by blood flow restriction exercise (BFRT) in patients with knee osteoarthritis (OA) compared to conventional training. In this study, which was completed by 38 participants, individuals were randomized to the conventional exercise group (KEG) and BFRT group. KEG exercises were completed with resistance of 70-80% of 1RM, and BFRT with resistance corresponding to 20-30% of 1RM. Participants in the BFRT group exercised by restricting blood flow at 80% of arterial occlusion pressure with a pneumatic cuff placed on the proximal thigh. Supervised exercise training consisting of the same program of 8 weeks and 24 sessions was given to all participants. At the initial treatment session and end of the treatment, the participants were evaluated for quadriceps cross-sectional area (CSA), and quadriceps volume (KV), pain severity, functionality, inflammatory progression, and quality of life. In addition, thigh circumference anthropometric measurements, knee joint range of motion (ROM), creatine kinase (CK) measurements were made. CSA, KV, pain severity, ROM, functionality and quality of life values showed significant differences between the groups at the end of the treatment compared to the baseline (p<0.05). According to the effect sizes of CSA, KV, flexion ROM, 30 seconds sit to stand test among functionality measurements, general health parameter of the quality of life questionnaire were significant between the groups in favor of BFRT. Only CK showed significant difference in favor of KEG (p<0.05). In our study, it was concluded that BFRT technic has a similar effect to the high-intensity resistance exercises used in conventional rehabilitation, provides more quadriceps hypertrophy, ROM, functionality, improvement in general health and decrement in pain, and can be used as a safe method in knee OA rehabilitation.
