An integrated framework for non-traditional machining process technology selection in healthcare applications

In spite of continuous progress in technical advancement, the conventional machining process became unsatisfactory in the healthcare field due to its disadvantages. This inadequacy lead researchers to consider using the application of nontraditional machining that can machine extremely hard and brittle materials into complicated shapes such as medical devices and implants in healthcare. In this study, the three most popular nontraditional machining process technologies: Laser Beam Machining, Water Jet Machining, and Electrocautery are evaluated to determine the most appropriate technology using the Health Technology Assessment based Multi-criteria Decision-Making framework. HTA is organized evaluation of effects and properties of health technology that enables the application of systematic skills to solve a health problem. HTA's main goal is to raise awareness of new health technologies among decision makers. For these reasons, the HTA core model that enables the production of HTA-related information was utilized.The comparison of selected technologies was carried out via integrating the HTA core model, Best Worst, and Evaluation Based on Distance from Average Solution methods. Finally, a comparison was made to find the most suitable technology to create the necessary infrastructure. As a result, evaluation scores were computed as 0,673; 0,538 and 0,500 for WJM, LBM, and EC, respectively.

Sağlık sektöründe yer alan ilaçlar, tıbbi tedavi prosedürleri, tıbbı cihazlar, cerrahi teknikler ve sağlık sistemleri Sağlık Teknolojisi Değerlendirme (STD) kapsamında değerlendirilmektedir. Bu tip sağlık teknolojilerinin çeşitli yönlerden ele alınarak belirlenen kriterler ve istenen hedef doğrultusunda değerlendirilmesinde STD gittikçe popülerleşen bir yöntemdir. Sağlık sektöründe kaynakların kısıtlı olması ve maliyetlerin son yıllarda artış göstermesi, bu sektördeki karar vericileri etkili, güvenilir ve rasyonel adımlar atmaya itmiş ve STD’ye olan ilgiyi artırmıştır. STD’ye olan ilginin artmasıyla beraber kapsamı da çok kriterli ve paydaşlı olacak şekilde genişlemiştir. Çok Kriterli Karar Verme (ÇKKV) yöntemleri birden fazla kriter altında alternatifler arasından en iyi alternatifin seçilmesinde önemli rol oynar. Sağlık sektöründe yaşanan gelişmeler, sağlıkta eniyileme ve kanıta dayalı tıp uygulamalarının yaygınlaşması STD çalışmalarında ÇKKV yöntemlerine olan ilgiyi her geçen gün artırmaktadır. Bu derleme çalışmasında, literatürde son 10 yılda yapılanSTD çalışmaları ve ÇKKV yaklaşımlarıyla yapılan STD çalışmaları kapsamlı bir biçimde özetlenmiş ve sınıflandırılmıştır. Ayrıca derleme çalışmasının sonucunda, STD çalışmalarında ÇKKV yöntemlerinin kullanımının 2008 yılından sonra hızlı bir şekilde arttığı gözlemlenmiştir. En sık kullanılan ÇKKV yöntemlerinin ise AHP (Analitik Hiyerarşi Prosesi), EVIDEM (Evidence and Value: Impact on DecisionMaking) ve ağırlıklı çarpım olduğu sonucuna ulaşılmıştır.; Drugs, medical treatment procedures, medical devices, surgical techniques and health systems in the healthcare field are in the scope of Health Technology Assessment (HTA). HTA is becoming a popular method to evaluate these health technologies by considering determined target and criteria. Limited resources and increasing costs in the healthcare field have prompted healthcare decision-makers to make effective, reliable and rational decisions and raised the interest on HTA. In this context, the importance of HTA has increased and its scope has expanded to include multiple criteria and stakeholders. Multiple criteria decision making (MCDM) plays an important role in selecting the best alternative from a set of available alternatives with respect to multiple criteria. Developments in the healthcare field, widespread utilization of optimization and evidence-based medicine studies are increasing the interest in utilization of MCDM methods in HTA studies day by day. In this study, HTA studies and HTA studies integrated with MCDM methods are reviewed and classified in a comprehensive manner. As a result of this review study, it is observed that utilization of MCDM methods in HTA studies has increased rapidly after 2008. In addition, AHP(Analytic Hierarchy Process), EVIDEM (Evidence and Value: Impact on DecisionMaking) and weighted product method are found to be the most commonly used MCDM techniques.

Potential criteria for frameworks to support the evaluation of innovative medicines in upper middle-income countries-A systematic literature review on value frameworks and multi-criteria decision analyses

Background Multicriteria Decision Analysis (MCDA), a formal decision support framework, has been growing in popularity recently in the field of health care. MCDA can support pricing and reimbursement decisions on the macro level, which is of great importance especially in countries with more limited resources. Objectives The aim of this systematic review was to facilitate the development of future MCDA frameworks, by proposing a set of criteria focusing on the purchasing decisions of single-source innovative pharmaceuticals in upper middle-income countries. Methods A systematic literature review was conducted on the decision criteria included in value frameworks (VFs) or MCDA tools. Scopus, Medline, databases of universities, websites of Health Technology Assessment Agencies, and other relevant organizations were included in the search. Double title-abstract screening and double full-text review were conducted, and all extracted data were double-checked. A team of researchers performed the merging and selection process of the extracted criteria. Results A total of 1,878 articles entered the title and abstract screening. From these, 341 were eligible to the full-text review, and 36 were included in the final data extraction phase. From these articles 394 criteria were extracted in total. After deduplication and clustering, 26 different criteria were identified. After the merging and selection process, a set of 16 general criteria was proposed. Conclusion Based on the results of the systematic literature review, a pool of 16 criteria was selected. This can serve as a starting point for constructing MCDA frameworks in upper middle-income countries after careful adaptation to the local context.

Son on yılda, sağlık hizmeti maliyetlerindeki çarpıcı artış, araştırmacıları ve sağlık politikacılarını maliyetleri düşürmek için yeni yollar bulmaya zorlamıştır. Simülasyon, sağlık hizmetlerinde verimliliği artırmak ve kaliteyi yükseltmek için kullanılan popüler ve etkili bir yöntem haline gelmiştir. Araştırmacıların sağlık hizmetlerinde gerçekleştirilen simülasyon çalışmalarına olan ilgileri, yayın sayısında sürekli bir artışa neden olmuştur. Bu derleme çalışmasında, sağlık alanında gerçekleştirilen ayrık olay simülasyonu (AOS) çalışmaları özetlenmiş ve sınıflandırılmıştır. Özellikle de yüksek düzeyde belirsizlik ve kısıtlı kaynaklar altında çalışan ameliyathanelerde gerçekleştirilen AOS çalışmalarına odaklanılmıştır. Ayrıca derleme çalışmasının sonucunda simülasyon tabanlı optimizasyon yöntemi ile ameliyathane çizelgeleme süreçlerinin iyileştirilmesi konusunda literatürde var olan eksiklikler belirlenmiştir. Bu doğrultuda, sağlık alanında çalışan araştırmacılar ve profesyoneller gelecek AOS araştırma ve uygulamaları hakkında yeni çalışmalarda bulunabilirler.; In recent decades, the dramatic increase in healthcare costs has compelled resarchers and healthcare policy makers to find out new ways to reduce costs. Simulation has become a popular and effective method for improving efficiency and enhancing quality in healthcare operations. The interest of researchers in the domain of simulation studies in healthcare induces a steady increase in the number of publications. In this regard, different discrete-event simulation (DES) studies employed in healthcare field are summarized and classified in this review. A particular attention is on the published studies that involve DES studies of operating rooms. The operating room is specifically the most complicated and expensive hospital resource that runs with high level of uncertainty and limited resources. Moreover, as a result of this review study, deficiencies in the literature about the improvement of the operating room scheduling processes with simulation based optimization method have been determined. In the literature, most of the simulation models only include demand and capacity constraints. However, they should include more constraints such as labor constraints, preferences of surgeons for surgical scheduling, number of available beds in intensive care unit, number of recovery beds, etc. Moreover, there is a need in the field to develop stochastic models, which involve uncertain surgery and recovery times. In most of the studies, it is assumed that these periods are known precisely. It is also determined that number of multi-objective models is quite low. In addition, only a few simulation studies have an online scheduling approach. Thus, these issues need to be addressed. In light of this review, healthcare professionals and researchers can make new suggestions and improvements regarding future research and applications of DES.
