Hemiplejik omuz ağrısında manuel terapinin ağrı ve fonksiyonellik üzerine etkisi

Abstract

Çalışmanın amacı; hemiplejik omuz ağrısı olan hastaların omuz ağrısı, üst ekstremite motor fonksiyonel kapasitesi ve eklem hareket açıklığı üzerine manuel terapi tekniklerinin etkisini incelemektir. Çalışmaya 18-75 arası 30 hasta dahil edildi. Katılımcılar 15 hasta manuel terapi grubunda ve 15 hasta kontrol grubunda olmak üzere iki gruba ayrıldı. Tüm hastalara germe, kuvvetlendirme, eklem hareket açıklığı, günlük yaşam aktivite egzersizleri, soğuk uygulama ve Transkutanöz Elektriksel Sinir Stimulasyonu (TENS)uygulandı. Manuel terapi grubunda klasik tedaviye ek olarak manuel terapi eklendi. Tedavi haftada 3 kez olmak üzere 4 hafta uygulandı.Değerlendirmeler tedavi öncesi ve sonrası yapıldı. Hastaların demografik ve klinik bilgileri kaydedildi.Omuz eklem-hareket açıklığı gonyometre-, ağrı Vizüel Analog Skalası, fonksiyonellik Fugl-Meyer üst ekstremite-motor fonksiyon skalası ve Barthel indeksi ile değerlendirildi. Çalışma sonuçlarının analizi SPSS23.00 istatistik programı aracılığıyla yapıldı. Anlamlılık p≤0,05 olarak alındı. Grupların her ikisinde de tüm klinik değerlerde istatistiksel olarak anlamlı fark saptandı (p<0,05).Gruplar arası karşılaştırmada ağrı, fonksiyonellik ve eklem hareket açıklığı değerlerinde istatistiksel yönden anlamlı fark saptanmadı (p>0,05).-Sonuç olarak manuel terapi yöntemi eklenen klasik tedavi metodunun, klasik tedaviye göre ağrı, fonksiyonellik ve eklem hareket açıklığı yönünden üstünlüğünün olmadığı görüldü. Rehabilitasyon süreci planlanırken iki tedavi yönteminin de kullanılabileceği, gerekli görüldüğü durumlarda manuel terapi yöntemi kullanılması önerildi.

The aim of this study is to examine the effect of manual therapy techniques on the improvement of parameters such as shoulder pain and motor functionality of upper extremity and range of motion with hemiplegic shoulder pain. Thirty patients between 18-75 were included in the study. The participants were divided into two groups as 15 patients in Manual Therapy Group and 15 patients in Control Group. Stretching, strengthening, Range of Motion, Activities of Daily Living Exercises, cold application and Transcutaneous Electrical Nerve Stimulation were applied to all patients. Manual therapy was applied to Manual Therapy Group in addition to classical physiotherapy. Assessments were made before and after treatment. The demographic and clinical data of the patients were recorded. Goniometer for a rangeof motion of the shoulder, Visual Analogue Scale for pain, Fugl-Meyer upper extremity motor functionality scale and Barthel index for functionality were used for assessments and treatments. SPSS v23.0 was used for statistical analysis. p ≤ 0.05 was considered a statistically significant difference. Statistically significant differences were found in all clinical values in both groups (p < 0.05).In the comparison between the groups, there was no statistically significant difference was found in all clinical evaluations (p> 0.05).As a result, it was seen that manual therapy had no superiority over other modalities in the point of pain, functionality, range of motion.These two methodscan be applied to the patients when planning of rehabilitation progress and manual therapy can be used for treatment if physiotherapist considers it as a necessity.
