Kanser hastalarında travma sonrası gelişim ve sosyal desteğin incelenmesi

Abstract

Bu çalışma kanser hastalarında travma sonrası gelişim ve sosyal destek ilişkisini belirlemek amacıyla yapılmış tanımlayıcı, kesitsel tipte bir çalışmadır. Araştırmanın örneklemini İstanbul'da bir özel üniversite hastanesinin onkoloji bölümünde takip edilen, belirlenen dahil edilme kriterlerine sahip, rastgele örnekleme yöntemi ile seçilen 120 hasta oluşturmuştur. Verilerin toplanmasında Travma Sonrası Gelişim Ölçeği, Çok Boyutlu Algılanan Sosyal Destek Ölçeği ve literatür ışığında araştırmacılar tarafından oluşturulan Sosyo-demografik Bilgi Toplama Formu kullanılmıştır. Verilerinin değerlendirilmesinde, bağımsız değişkenlerin ölçek puanları üzerine etkisi Independent-samples t test, Mann Whitney U ve Kruskal Wallis analizleri kullanılarak incelenmiş, ölçek/ölçek alt boyut puan ortalamaları arasındaki ilişki ise Spearman Korelasyon analizi kullanılarak değerlendirilmiştir. Çalışmamızda hastaların travma sonrası gelişim ölçeği puan ortalamalarının orta düzeyde (62.02±22.34) (min:0.00- max:135.00) olduğu saptanmıştır. Ölçeğin alt boyut puanları incelendiğinde hastaların en yüksek puanı 'Kişinin Kendisindeki Değişim' alanından aldıkları görülmüştür. Hastaların Çok Boyutlu Algılanan Sosyal Destek Ölçeği puan ortalaması ise 62.98±16.10 (min:22.00 – max:84.00) olarak saptanmış ve alt boyutlardan 'Aile'nin en yüksek algılanan destek alt boyutu olduğu saptanmıştır. Travma Sonrası Gelişim Ölçeği ile Çok Boyutlu Algılanan Sosyal Destek Ölçeği korelasyonları incelendiğinde; sosyal destek düzeyi ile travma sonrası gelişim düzeyi arasında pozitif yönlü korelasyon olduğu saptanmıştır. Çalışmamızda özellikle kişinin kendisindeki değişimin ve aileden algılanan sosyal desteğin travma sonrası gelişimi etkilediği dikkat çekici diğer bulgudur. Ayrıca iki ölçeğin de tüm alt boyutları ile pozitif yönde ilişkisinin olması bu iki kavramın da kültürümüzdeki yerinin anlaşılması adına büyük önem arz etmektedir. Çalışmamız zorluklarla dolu kanser sürecinin iyi yönetilmesi durumunda, bu deneyimin güçlendirici etkisinin olabileceğini ve bunun için sosyal desteğin bir kolaylaştırıcı olduğunu gözler önüne sermektedir. Bakım sırasında hasta ve hasta yakınlarının beraber ele alındığı multidisipliner bir ekip yaklaşımının benimsenmesi ve bir travma olarak kanseri, kanserde travma sonrası gelişimi kolaylaştıran faktörleri hasta ve bakım verenler gözünden inceleyebilmek için kalitatif çalışmaların yapılması önerilmektedir.

This study is a descriptive, cross-sectional study to determine the relationship between post-traumatic growth and social support in cancer patients. The sample of the study consisted of 120 patients who were randomly sampled and had the specified inclusion criteria, who were followed in the oncology department of a private university hospital in Istanbul. In the collection of data, Post-Traumatic Growth Scale, Multidimensional Perceived Social Support Scale and Socio-demographic Information Collection Form created by the researchers in the light of the literature were used. In the evaluation of the data, the effect of independent variables on the scale scores was examined using Independent-samples t test, Mann Whitney U and Kruskal Wallis analyzes, and the relationship between the scale / scale subscale scores was evaluated using Spearman correlation analysis. In our study, it was determined that the mean score of post-traumatic growth scale of the patients was moderate (62.02 ± 22.34) (min:0.00- max:135.00). When subscale scores of the scale were examined, it was seen that the patients got the highest score from the 'Change in the self'. The mean score of the Multidimensional Perceived Social Support Scale of the patients was 62.98 ± 16.10 (min:22.00 – max:84.00) and it was determined that 'Family is the highest perceived support sub-dimension' from the subscales. When the Post Traumatic Growth Scale and Multidimensional Perceived Social Support Scale correlations were examined, there was a positive correlation between social support level and post-traumatic growth level. Another remarkable finding in our study is that the change in the individual itself and the social support perceived from the family influence the post-traumatic growth. In addition, the fact that both scales are positively related to all sub-dimensions is of great importance for the understanding of these two concepts. Our study shows that if the cancer process is full of difficulties, it can be the empowering effect of this experience and that it is an facilitator of social support for it. It is suggested that qualitative studies should be carried out in order to be able to examine the adoption of a multidisciplinary team approach in which patients and their relatives are treated together during care and to examine factors that facilitate cancer growth as a trauma and posttraumatic cancer in cancer patients and caregivers.
