Koklear implant, işitme cihazından fayda görmeyen ve çok ileri derece işitme kaybına sahip olan kişiler için iç kulağa yerleştirilen elektronik bir cihazdır. Koklear implant kullanıcılarında en sık karşılaşılan zorluklardan birisi gürültü varlığında konuşmayı anlayamamaktır. Gürültüde konuşmayı anlamada etkili olduğu söylenilen birçok faktör mevcuttur. Bu çalışmanın amacı, yetişkin koklear implant kullanıcılarında bilişsel işlevlerin gürültüde konuşmayı anlamadaki etkisini araştırmaktır. Katılımcıların bilişsel becerileri sayı menzil ve stroop testleri ile değerlendirildi. Sayı menzil testinin özellikle çalışma belleğini, stroop testinin ise inhibisyon becerisi ve dikkati değerlendirdiği bilinmektedir. Gürültüde konuşmayı anlama performansı ise Türkçe Matris Testi ile değerlendirildi. Katılımcılar ayrıca işitsel yoksunluk ve implant kullanım sürelerine göre karşılaştırıldı. Çalışmaya en az 1 yıl koklear implant deneyimine sahip 36 yetişkin koklear implant kullanıcısı katıldı. Verilerin tanımlayıcı ve nicel istatistiksel analizleri için "SPSS version 20.0" kullanıldı. İşitsel yoksunluk süresi 10 yıldan az olanların geri sayı menzil testi ile Türkçe matris testi arasında istatistiksel olarak anlamlı orta derecede korelasyon (p<0,05, r=-0,556), implant kullanım süresi 0-5 ve 10 yıldan fazla olanların sayı menzil testlerinde güçlü korelasyonlar gözlendi (p<0,05, r=-0,717, r=-0,828). Tüm katılımcıların Türkçe matris test sonuçları ile geri sayı menzil ve sayı menzil toplam skorlarında istatistiksel olarak anlamlı orta derecede korelasyonlar elde edildi (r=-0,434 ve r=-0,428). Koklear implant kullanıcılarının gürültüde anlama performansında işitsel yoksunluk ve implant kullanım süresinin gürültüde anlama üzerinde doğrudan bir etkisinin olmadığı gözlendi. Bilişsel becerilerin önemli olduğu ve özellikle çalışma belleğinin gürültüde konuşmayı anlama performansında etkili olduğu sonucuna ulaşıldı. A cochlear implant is an electronic device for people with profound hearing loss who cannot benefit from a hearing aid. One of the most common difficulties in cochlear implant users is the inability to perceive speech in the presence of noise. There are many factors that effect speech perception in noise. The aim of this study is to investigate the effect of cognitive functions on understanding speech in noise in adult cochlear implant users. Cognitive functions of the participants were evaluated with digit span and stroop tests. It is known that the digit span test especially evaluates working memory and stroop test evaluates inhibition skill and attention. Speech perception performance in noise was evaluated with the Turkish Matrix Test. Participants were also compared for duration of auditory deprivation and implant usage time. 36 adult cochlear implant users with at least 1 year of cochlear implant experience participated in the study. Descriptive and quantitative analysis of differences associated with independent variables were performed by SPSS version 20.0. A moderate correlation was observed between the digit span and Turkish matrix test in participants with a duration of auditory deprivation less than 10 years. (p<0.05, r=-0.556). In addition to, strong correlations were observed between the Turkish matrix test and the digit span test in patients with 0-5 years and more than 10 years of implant usage time (p<0.05, r=-0.717, r=-0.828). Furthermore, moderate correlations were obtained between the Turkish matrix test and the backward and digit span total scores (r=-0.434 and r=-0.428). We were observed that auditory deprivation and duration of implant use did not have a direct effect on speech perception in cochlear implant users. It was concluded that cognitive functions are important and especially working memory is effective in perception speech in noise.
