Türkiye temel hastane hizmetlerine erişimin evrensel sağlık kapsayıcılığı çerçevesinde stratejik açıdan incelenmesi

Abstract

Beslenme, barınma ve giyinme tüm insanların temel yaşam ihtiyaçlarıdır. Sağlıklı bir yaşam sürdürmek ise her bireyin en temel insan hakkıdır. Ancak, dünya üzerindeki milyonlarca kişi halen temel ihtiyaçları ile en temel hakkı arasında seçim yapmak zorunda kalmaktadır. Dünya nüfusunun yarısının ihtiyaç duydukları sağlık hizmetine erişimi yoktur. Evrensel sağlık kapsamı (ESK), tüm bireylerin maddi bir külfete girmeden kaliteli sağlık hizmetlerini ihtiyaç duyduğu anda almasını ve sağlığını tehdit eden risk faktörlerinden korunmasını amaçlar. ESK, Dünya Sağlık Örgütü (DSÖ)'nün en önemli hedeflerinden birisidir ve bu nedenle DSÖ, 2018 yılı Dünya Sağlık Günü'nde "herkes için, her yerde sağlık" teması ile ESK'ye odaklanmıştır. Ülkelerin ESK' de başarılı olmalarının yolu, toplumun tüm fertlerinin "sosyal sağlık güvencesi kapsamına alınmasına" ve "sağlık hizmet sunumuna ayrılan kaynakların adil, eşit ve dengeli tahsisine" bağlıdır. Bu tezin amacı; ESK çerçevesinde temel hastane hizmetlerine erişimi ve hastane hizmet yeterliliklerini kalite evi temelinde bölgesel düzeyde tespit etmek ve tüm bireylerin kaliteli temel hastane hizmetlerine erişimlerine katkı sunmaktır. Çalışmada; Türkiye İstatistik Kurumu (TÜİK) İstatistik Bölge Birimleri Sınıflaması-1 (İBBS-1) kullanılmıştır. 2009-2018 dönemine ait veriler Bulanık TOPSIS yöntemi ile analiz edilmiştir. Çalışmanın sonucunda sosyal sağlık güvencesi kapsamının ülke genelinde oldukça eşit ve kapsayıcı olduğu, ancak sağlık hizmet sunumu ile ilgili kaynakların dağılımının bölge bazında adil ve dengeli olmadığı, bölgeler arasında temel hastane hizmetleri açısından önemli farklılıklar ve eşitsizlikler bulunduğu tespit edilmiştir.

Nutrition, shelter and clothing are basic life needs of all people. Maintaining a healthy life is the most basic human right of everyone. However, millions of people around the world still have to choose between their basic needs and their most basic rights. Half of the world's population does not have access to the healthcare they need. Universal Health Coverage (UHC) aims to ensure that all individuals receive quality health services whenever they need without incurring a financial burden and to protect them from risk factors that threaten their health. UHC is one of the most important goals of the World Health Organization (WHO).That's why WHO focused on UHC with the theme of "health for all, everywhere" on World Health Day 2018. The way for countries to be successful in the UHC depends on the "inclusion of all members of the society under social health insurance" and "fair, equal and balanced allocation of resources allocated for health service delivery". The aim of this thesis is; within the framework of the UHC, to determine the access to basic hospital services and quality hospital service at the regional level on the basis of the quality house and to contribute to the access of all individuals to quality basic hospital services. In the study, Turkey Statistical Institute Statistical Classification of Territorial Units-1 was used and the data of the 2009-2018 period. The data were analyzed with the Fuzzy TOPSIS method. As a result of the study, it was determined that the coverage of social health insurance is quite equal and inclusive throughout the country. However the distribution of resources related to health service delivery is not fair and balanced on the basis of the region, and there are significant differences and inequalities in terms of basic hospital services among regions.
