Kondromalazi patella, patella eklem yüzündeki kıkırdağın yumuşaması, saçaklanması ve ülserasyonudur. Etiyolojisinde lateral fasete binen aşırı yük, eklem içi enjeksiyon uygulamaları, immobilizasyon gibi birçok faktör rol oynamaktadır. Tedavi edilmediği takdirde patellofemoral osteoartrite ilerleyebilmektedir. Literatürde patellofemoral geometriyi ölçen birçok yayın bulunmasına rağmen tibiofemoral geometrinin kondromalazi patella'ya etkisi hala belirsizliğini korumaktadır. Amacımız koruyucu hekimlik açısından bireylerin gelecekte patellofemoral osteoartrite eğilim olup olmayacağını ölçümlerle öngörmektir. Çalışmamızda ölçeceğimiz açılar Medial Tibial Slope, Lateral Tibial Slope, Sulcus Derinliği, Sulcus Açısı, Lateral Patellar Tilt Açısı, Lateral Patellofemoral Açı, Insall-Salvati Index, Caton-Dechamp Index, TT-TG mesafesi ve Lateral Trochlear Inklinasyon'dur. Modifiye Outerbridge evreleme sisteminde grade-1 ve 2'de kıkırdak lezyonları yüzeyel iken grade-3 ve 4'te derin ülserasyonlarla beraber subkondral kemiğin görünür hale gelmesi söz konusudur. Klinikte aynı semptomları vermesinden dolayı grade-1 ve 2'ye erken evre, grade-3 ve 4'e ileri evre kondromalazi olarak tanımlanmaktadır. Bu nedenle çalışmamıza, Medipol Mega Hastaneler Kompleksi'nde diz MRG yapılmış farklı yaş gruplarındaki (27-63), kondromalazi patella Modifiye Outerbridge derecelendirmesine göre grade-1 ve 2 olan yüz kontrol grubu olgusu ve grade-3 ve 4 olan yüz deney grubu olgusu dahil edilmiştir. Yapılan ölçümler sonucunda vaka grubunda Medial Tibial Slope, Sulcus Derinliği, Lateral Patellar Tilt Açısı, Lateral Patellofemoral Açı, kontrol grubundan anlamlı (p<0.05) olarak daha düşüktü. Vaka grubunda Lateral Tibial Slope, Sulcus Açısı, Insall-Salvati Index, Caton-Dechamp Index, TT-TG mesafesi kontrol grubundan anlamlı (p<0.05) olarak daha yüksekti. Vaka ve kontrol grubu arasında Lateral Trochlear Inklinasyon anlamlı (p>0.05) farklılık göstermemiştir. Sonuç olarak bu ölçümlerin hepsi kondromalazi patella prognozunda bize yön gösterici olabilir. Chondromalacia patella is the softening, fringing and ulceration of the cartilage on the articular surface of the patella. Many factors such as excessive load on the lateral facet, intra-articular injection applications and immobilization play a role in its etiology. If left untreated, it may progress to patellofemoral osteoarthritis. Although there are many publications in the literature measuring patellofemoral geometry, the effect of tibiofemoral geometry on chondromalacia patella still remains unclear. Our aim is to predict whether individuals will have a tendency to patellofemoral osteoarthritis in the future in terms of preventive medicine, with measurements. The angles we will measure in our study are Medial Tibial Slope, Lateral Tibial Slope, Sulcus Depth, Sulcus Angle, Lateral Patellar Tilt Angle, Lateral Patellofemoral Angle, Insall-Salvati Index, Caton-Dechamp Index, TT-TG distance and Lateral Trochlear Inclination. In the modified Outerbridge staging system, cartilage lesions are superficial in grades 1 and 2, while subchondral bone becomes visible with deep ulcerations in grades 3 and 4. Since it gives the same symptoms in the clinic, it is defined as grade-1 and 2 as early stage, and grade-3 and 4 as advanced stage chondromalacia. Therefore, one hundred control group patients with chondromalacia patella grade-1 and 2 according to Modified Outerbridge grading and one hundred experimental group patients with grade-3 and 4 in different age groups (27-63) who underwent knee MRI at Medipol Mega Hospitals Complex were included in our study. As a result of the measurements, Medial Tibial Slope, Sulcus Depth, Lateral Patellar Tilt Angle, Lateral Patellofemoral Angle were significantly (p<0.05) lower in the case group than in the control group. Lateral Tibial Slope, Sulcus Angle, Insall-Salvati Index, Caton-Dechamp Index, TT-TG distance were significantly (p<0.05) higher in the case group than in the control group. Lateral Trochlear Inclination did not differ significantly (p>0.05) between the case and control groups. In conclusion, all of these measurements can guide us in the prognosis of chondromalacia patella.
