Çalışmamızın amacı kronik inmeli bireylerde robotik rehabilitasyonun geleneksel terapiye kıyasla denge, gövde kontrolü, mobilite, spastisite, motor fonksiyon ve depresyon üzerine etkilerinin karşılaştırılmasıdır. Özel Avrasya Hastanesi Fizik Tedavi ve Rehabilitasyon Kliniği'ne başvuran, epikriz ve sağlık kurulu raporu ile hemipleji tanısına sahip, yaşları 40-70 arasında olan 40 hasta gönüllülük esası ile cinsiyet farkı gözetilmeksizin çalışmaya alındı. Katılımcılar demografik ve klinik bilgileri kaydedildikten sonra hekim kararı esas alınarak robotik rehabilitasyon ile kombine geleneksel tedavi (n=20) ve yalnızca geleneksel tedavi (n=20) olmak üzere 2 gruba dahil edildi. Gruplardan biri geleneksel tedavi alırken diğer grup geleneksel tedaviye ek olarak robotik yürüme eğitimi almıştır. Geleneksel tedavi haftada 3 gün olacak şekilde 4 hafta boyunca uygulanan güçlendirme, denge, eklem hareket açıklığı egzersizleri ile yürüyüş eğitimi içermektedir. Robot destekli yürüyüş eğitimi haftada 3 gün 20 dakika olacak şekilde planlandı. Değerlendirme yöntemleri olarak 10m Yürüme Testi, Brunnstrom motor evrelemesi, Fonksiyonel Ambulasyon Sınıflandırması, Fugl Meyer Değerlendirme Ölçeği (alt ekstremite bölümü), Modifiye Ashworth Skalası, Beck Depresyon Ölçeği, Tinetti Denge ve Yürüme Testi, İnme Hastalarında Postüral Değerlendirme Ölçeği ve İnme Etki Skalası kullanıldı. Gruplar arasında cinsiyet, yaş ve hastalık süreleri homojen dağılım gösterdi. Robot ve egzersiz grubunun tedavi sonrası adım sayısı ve yürüme hızı anlamlı olarak artmış, Fugl Meyer Testi ve TDYD denge alt testi ölçümleri anlamlı olarak arttı (p<0,05). Egzersiz grubunda tedavi sonrası adım sayısı, Beck Depresyon Ölçeği, TDYD yürüme alt testi ve PASS-Türk ölçümlerinde anlamlı bir farklılık bulundu (p<0,05). Robot destekli yürüme eğitimi kronik inmeli bireylerde geleneksel tedaviye ek tamamlayıcı bir tedavi yöntemi olarak kullanılabilir. The aim of this study is to compare the effects of robotic rehabilitation on balance, body control, mobility, spasticity, motor function and depression compared to traditional therapy in individuals with chronic stroke. Patients aged between 40-70 years, who applied to the Private Avrasya Hospital Physical Therapy and Rehabilitation Clinic, were diagnosed with hemiplegia based on an epicrisis medical board report, were included in the study on a voluntary basis, regardless of gender. After recording the demographic and clinical information of the participants, based on the physician's decision, they were included in 2 groups: conventional treatment combined with robotic rehabilitation (n=20) and conventional treatment only (n=20). While one of the groups received traditional treatment, the other group received robotic walking training in addition to conventional treatment. Traditional treatment includes strengthening, balance, range of motion exercises and gait training applied 3 days a week for 4 weeks. Robot-assisted walking training was planned for 20 minutes, 3 days a week. As assessment methods, the 10m Walk Test, Brunnstrom motor staging, Functional Ambulation Classification, Fugl Meyer Rating Scale (lower extremity section), Modified Ashworth Scale, Beck Depression Scale, Tinetti Balance and Gait Test, Postural Assessment Scale in Stroke Patients and Stroke Impact Scale were used. Gender, age and duration of illness showed homogeneous distribution between the groups. The number of steps and walking speed of the robot and exercise group increased significantly after the treatment, while Fugl Meyer Test and TDYD balance subtest measurements increased significantly (p<0.05). A significant difference was found in the exercise group in the post-treatment step count, Beck Depression Scale, TDYD walking subtest, and PASS-Turk measurements (p<0.05). Robot-assisted walking training can be used as a complementary treatment method in addition to traditional treatment in individuals with chronic stroke.
